1) National Taiwan University Hospital established a telehealthcare center in 2009 to provide continuous healthcare services for patients at home. 2) The center uses an integration system to provide patient self-management support, extract recommendations from patient data, encourage engagement, and provide educational and treatment advice. 3) Case managers closely contact patients, provide health advice, and alert physicians if needed, while physicians provide instruction and support.

5. Conclusion The Department of Health of Taiwan has been establishing a national health insurance that includes continuous care service. A new service infrastructure is needed to support post-hospital community service, and provide continuous and comprehensive care service Our project demonstrates an infrastructure to enabling integrated personal healthcare service.
