New Directions in Targeted Therapeutic Approaches for Older Adults With Mantl...i3 Health
i3 Health is pleased to make the speaker slides from this activity available for use as a non-accredited self-study or teaching resource.
This slide deck presented by Dr. Kami Maddocks, Professor-Clinical in the Division of Hematology and
Associate Division Director for Ambulatory Operations
The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.
Target Audience
Hematology/oncology fellows, attending faculty, and other health care professionals involved in the treatment of patients with mantle cell lymphoma (MCL).
Learning Objectives
1.) Identify clinical and biological prognostic factors that can guide treatment decision making for older adults with MCL
2.) Evaluate emerging data on targeted therapeutic approaches for treatment-naive and relapsed/refractory MCL and their applicability to older adults
3.) Assess mechanisms of resistance to targeted therapies for MCL and their implications for treatment selection

The prostate is an exocrine gland of the male mammalian reproductive system
It is a walnut-sized gland that forms part of the male reproductive system and is located in front of the rectum and just below the urinary bladder
Function is to store and secrete a clear, slightly alkaline fluid that constitutes 10-30% of the volume of the seminal fluid that along with the spermatozoa, constitutes semen
A healthy human prostate measures (4cm-vertical, by 3cm-horizontal, 2cm ant-post ).
It surrounds the urethra just below the urinary bladder. It has anterior, median, posterior and two lateral lobes
It’s work is regulated by androgens which are responsible for male sex characteristics
Generalised disease of the prostate due to hormonal derangement which leads to non malignant enlargement of the gland (increase in the number of epithelial cells and stromal tissue)to cause compression of the urethra leading to symptoms (LUTS

Editor's Notes
Nu Skin’s ageLOC approach is to target the sources of the problem. Rather than just trying to mask the signs and symptoms, which is a traditional approach used by nutritional supplement and skin care companies, we specifically target the sources of aging.
Through this ability to target the sources of aging, we identify changes in gene expression that accelerate the aging process.
Aging defense mechanisms exist naturally in all of our cells to help resist aggressors that accelerate aging. The expression of our genes communicates with our aging defense mechanisms to tell them how to behave to protect the body and keep it functioning youthfully.
However, from the moment we’re born, we’re exposed to external and internal aging aggressors. Aging aggressors are a broad class of factors including environmental factors like pollutants, toxins, sun, smoke, stress, and anxiety as well as internal factors like cellular metabolism, stress, debris, and damage accumulation.
Although we are all born with aging defense mechanisms, over time they decline in function and performance due to changes in gene expression. We may begin to feel the effects of age.
By maintaining the youthful activity of our aging defense mechanisms, we can help our bodies continue to feel and act young throughout our lives. Gene expression modulates aging defense mechanisms, which in turn elicit broad spectrum, systemic anti-aging benefits. Aging defense mechanisms are not genes or youth gene clusters; they are mechanisms that gene expression controls. Gene expression changes with age, leading to the loss of efficiency of aging defense mechanisms with age. ageLOC Youth targets the gene expression that controls aging defense mechanisms, reminding them to perform youthfully.
Some of the ways that we experience these youthful effects is through systemic health benefits such as DNA protection and repair, optimal physical performance, metabolic health, and healthy cognition.
Using our proprietary science, we have developed our most advanced anti-aging supplement to date. ageLOC Youth is a foundational product designed for all adults interested in wellness and youth preservation benefits.
Our bodies have an amazing array of systems designed to maintain health and the benefits of youth. As mentioned before, we call these aging defense mechanisms. Currently, we are focusing on systemic mechanisms in six categories:
Antioxidant Defense and Protection
Systemic Detoxification and Stress Response
DNA Protection and Repair
Tissue Renewal
Metabolism Regulation
Regulation of Inflammatory Balance
These aging defense mechanisms operate throughout the body and interact with one another, working synergistically to oppose aging aggressors.
Over time, the function of all six aging defense mechanisms diminishes and declines due to changes in gene expression, accelerating the aging process.
Through Nu Skin’s proprietary science we have the ability to identify ingredients that can positively modulate gene expression and restore our aging defense mechanisms to youthful activity.
Youthful gene expression is optimal gene expression and that is what is necessary to regulate the healthy and efficient function of ADMs.
We set out to identify ingredients that would positively modulate gene expression that regulates our ADMs. Here are the ADMs targeted by our anti-aging formula. All of these ADMs are defense mechanisms that we already have in our bodies and these particular ADMs work throughout our bodies.
[Examples of categories of ADMs, the speaker would choose 1 or 2 of the following ADM categories to highlight/give more detail on how ADMs preserve youth]
1) The blend targets both exogenous and endogenous antioxidant defense and protection mechanisms. It provides dietary (exogenous) antioxidants like the carotenoids lutein, lycopene, and astaxanthin, but our bodies also have the ability to synthesize our own antioxidants (e.g. glutathione) and antioxidant enzymes (like catalase and glutathione peroxidase. The blend delivers ingredients that stimulate the production of these endogenous antioxidants. Therefore, the blend promotes antioxidant status from within and without, maximally supporting our antioxidant protection and defense mechanisms.
2) Similarly, the blend delivers ingredients that promote the activity of our intrinsic Systemic Detoxification and Stress Response Mechanisms, supporting our cells’ abilities to neutralize and excrete cellular toxins and protect against damage from cellular stress and contaminants.
3) The blend includes ingredients that promote the protection and repair of our DNA.
4) The blend supports the Mechanisms Regulating Inflammatory Balance, as we observed in the ADM video, a healthy inflammatory response is critical to preserve youth.
5) The blend targets Tissue Renewal Mechanisms. For example, rather than providing the building blocks for healthy bone e.g. calcium and magnesium, the blend promotes bone remodeling, it targets the mechanisms responsible for the removal of old, weaker bone cells for new, strong, healthy bone structure.
6) The blend targets Mechanisms Regulating Metabolism. For example, the blend positively influences both glucose and lipid metabolism, both of which are critical for youth preservation.
This network of aging defense mechanisms works synergistically to defend against negative effects of aging, eliciting systemic youth preservation benefits.
Systemic youth preservation benefits contribute to broad spectrum systemic health benefits.
ADMs operate in all cells and interact with one another. Here is an example of several ADMs operating in the brain to maintain cognitive performance and neuroprotection. These are ADMs related to brain health and cognition.
In this example, when we say brain health, these are some of the benefits that the product produces in the brain.
These are the requirements for effective ADMs. With ageLOC Youth, we set out to identify ingredients that contribute to the effective function of ADMs.
We used our proprietary gene expression screening tool to filter through a library of potential ingredients—filtering down to those that demonstrate a positive influence on gene expression and had the most robust anti-aging scientific substantiation to develop our most advanced anti-aging supplement.
We then developed these ingredients into an optimal blend—a mixture that is the most beneficial for youth preservation benefits.
We carefully selected ingredients based on their unique health benefits and their ability to positively impact gene expression using our proprietary anti-aging scientific approach. The result is a powerful blend of ingredients that strengthen and support our aging defense mechanisms.
Our scientific approach with ageLOC Youth is different from multivitamins and mineral supplements that are focused primarily on filling nutritional gaps. ageLOC Youth provides targeted nutrients that are not common even in healthy diets.
After selecting these ingredients based on scientific literature and gene expression research, we noticed that many of these ingredients are found in diets from all over the world—many of which are well known for the extraordinary health benefits in the specific cultures where these ingredients are consumed.
These ingredients come from various regions all over the world, which is one of the reasons why this unique ingredient blend is not readily available even from a healthy diet.
Another reason this ingredient blend is difficult to obtain from your diet is due to dietary habits and lifestyle. For example, in most places around the world, people typically discard the peels or rinds of citrus fruit. But these are great sources of naringin, hesperetin, and d-limonene, which is why in some places, such as the Mediterranean, people eat them. Eating the citrus peel may turn out to be one of the secrets to the health benefits of the Mediterranean diet, but this is not a habit that most people around the world are likely to adapt. ageLOC Youth delivers these ingredients in standardized amounts and in consistent doses.
Other examples: anchovies, purple corn juice
Obtaining the consistent, precise doses of ingredients for optimal benefits is also very difficult through diet.
ageLOC Youth delivers a potent blend of anti-aging nutrients and phytochemicals that are difficult to obtain from diet alone.
Some powerful ingredients such as resveratrol, coenzyme Q10, alpha lipoic acid, and carnosic acid are found at such low levels in foods that it would be nearly impossible to get enough—or even a consistent amount—from your diet each day. For example, astaxanthin is a carotenoid responsible for the pink color in marine sources such as shrimp, salmon, and seaweed. You could consume the same amount of astaxanthin found in ageLOC Youth by eating a full serving of salmon, but would you want to eat salmon every single day? Probably not.
ageLOC Youth provides consistent levels of these powerful ingredients twice per day.
ageLOC Youth delivers all-encompassing anti-aging protection. It promotes a wide range of functions from your heart to your brain—your bones to your metabolism.
Everyone will benefit from this product. The benefits it provides are foundational and broad spectrum. This is the first product that everyone should reach for, regardless of diet, age, or lifestyle.
