Hydroquinone has been used by dermatologists for over 40 years to lighten hyperpigmentation and is considered the gold standard treatment. The FDA created an OTC monograph in 1982 that allows the sale of 1.5-2% hydroquinone products without a prescription. Hydroquinone works by inhibiting tyrosinase, a key enzyme in melanin production. While it can lighten dark marks, exposure to UV light can cause them to return, so daily sunscreen use is important. Hydroquinone is scrutinized due to rare cases of liver damage and skin discoloration from misuse of high doses, but is considered safe when used as directed in recommended amounts of 2% or less.
