This document summarizes a study examining surgical site infections following caesarean sections through post-discharge surveillance across multiple hospitals in the UK. The study found that 13.6% of women who had c-sections developed wound problems, with 84% occurring after discharge. Applying standardized definitions, 8.9% met criteria for surgical site infections. However, rates varied significantly between hospitals, from 2.9% to 17.9%. The study identified several risk factors associated with higher infection rates, including BMI, age, blood loss during surgery, wound closure method, and emergency procedures. The results suggest c-section morbidity has been underestimated without post-discharge monitoring, and that antibiotic prescribing after discharge could be reviewed given

ABSTRACT- Introduction: Surgical Site Infections (SSI) still remains a significant problem following an operation
and the third most frequently reported nosocomial infections. SSI contributes significantly to increased health care costs in
terms of prolonged hospital stay and lost work days.
Objective: The current study was undertaken to identify incidence of SSI and the risk factors associated with it, and the
common organism isolated and its antibiotic sensitivity and resistance.
Material and Methods: A total number of 3211 patients admitted in general surgical wards for elective surgery in the
study period, out of which 1225 were clean and clean contaminated cases, fulfilling our study criteria. Totally 56 cases
had surgical site infections which had been taken up for this study. Wound discharges were sent for culture and sensitivity.
Results and Conclusions: The overall infection rate was 4.57%. The SSI rate was almost equal in clean surgeries and
clean contaminated ones. Superficial surgical site infections in the most commonest type and accounted for about 66.07%
of all the SSI’s and deep surgical site infection accounted for about 25% with 8.92% was organ space. The most
commonly isolated organism from surgical site infections was staphylococcus aureus followed by pseudomonas and then
E. coli. Drains, prosthesis usage and other risk factors of SSI have been identified. Most of the organisms which were
isolated were multidrug resistant. The high rate of resistance to many antibiotics underscored the need for a policy that

with a community midwife after discharge. This provided an opportunity to
assess whether a collaborative surveillance approach between hospital and
community staff was feasible using routinely available information. Follow-
ing a successful pilot study, 11 maternity units in the East Midlands partici-
pated in an extended study. Complete records were available for 5563
(88%) women. Overall, 758 (13.6%) wound problems were reported, 84%
of which developed after discharge. Of these, 488 (8.9%) met national def-
initions for surgical site infection (SSI); however, there was a marked inter-
unit difference in incidence, ranging from 2.9% to 17.9%. Statistical models
were used to examine these differences using 12 possible risk factors. Five
risk factors were found to be significantly associated with the development
of a surgical site infection: body mass index, age, blood loss, method of
wound closure and emergency procedures. These results suggest that
caesarean section is associated with high infectious morbidity, the extent
of which would have been considerably underestimated without post-
discharge monitoring. Almost all women with wound problems were
treated with antibiotics, regardless of how minor the problem, with 97%
