This retrospective study analyzed 1178 blood samples from cancer patients with suspected bloodstream infections at a tertiary cancer hospital in India from 2016. The study found a blood culture positivity rate of 27.35% and Gram-negative bacteria accounted for over half of infections. Escherichia coli was the most common organism isolated. Antibiotic resistance was high, especially for cephalosporins. Sensitivity to amikacin was the highest for most Gram-negative bacteria. The study provides important local data on infectious organisms and antibiotic susceptibility to help guide effective treatment of bloodstream infections in cancer patients.

Most cases of ACCs are sporadic but can also occur in association with several hereditary syndromes, including Li-Fraumeni, Beckwith-Wiedemann, multiple endocrine neoplasia (MEN) 1, congenital adrenal hyperplasia, familial polyposis coli, and germline β-catenin or p53 mutations. Patients with ACC present with either symptoms due to hormone hypersecretion or manifestations of tumor mass effect, although an increasing percentage is discovered as incidentalomas during abdominal imaging.

This study analyzed 1020 patients with mild to moderate COVID-19 infections and found that 210 (20.6%) had newly diagnosed diabetes based on blood glucose or HbA1c levels. Nearly all (90.4%) of these 210 patients had an HbA1c of 6.5% or higher, suggesting they likely had previously undiagnosed diabetes. The study aims to determine if COVID-19 infection itself increases the risk of diabetes.
Familial Mediterranean Fever (FMF) is an autoinfl ammatory disease caused by mutations in the MEFV
gene and characterized by recurrent fever, polyserositis and arthritis. It is transmitted in an autosomal
recessive pattern. FMF has been predominantly found in ethnic groups living around the Mediterranean
basin (Jews, Arabs, Turks, and Armenians). Although accompanying infl ammatory diseases have been
reported in FMF, the coexistance of fi bromyalgia syndrome (FMS) is very rare. MEFV gene analysis that the
fi rst patient had compound heterozygous mutation (M680I+V726A) and the other patient had heterozygous

Introduction: Bloodstream infections (BSIs) are associated with a high mortality rate of 20%-50%. Blood culture is paramount to identify causative agents of BSIs to choose an appropriate antimicrobial therapy. Objectives: The present study was undertaken to analyze the various microorganisms causing BSIs and study their antimicrobial resistance patterns in a tertiary care hospital, Eastern India. Materials and Methods: A total of 239 blood specimens from clinically suspected cases of BSIs were studied for 6 months from July 2015 to December 2015. Blood specimens were incubated in BacT/ALERT ® 3D system (bioMerieux, Durham, NC, USA) a fully automated blood culture system for detection of aerobic growth. Identification and antimicrobial susceptibility testing were conducted on VITEK ® 2 (bioMerieux, Durham, NC, USA) as per Clinical Laboratory Standards Institute guidelines. Results: Out of 239 specimens, 41 (17.2%) yielded growth of different microorganisms. From these isolates, 20 (48.8%) were Gram-negative bacilli, 18 (43.9%) were Gram-positive cocci and rest 3 (7.3%) were yeasts. Among Gram-negative bacilli, Klebsiella pneumoniae sub spp. pneumoniae (70%) was most commonly isolated. Coagulase-negative staphylococci (88.9%) were the most common isolate among Gram-positive cocci. All three Candida spp. isolated were nonalbicans Candida (two Candida tropicalis and one Candida krusei). Gram-negative isolates were least resistant to tigecycline and colistin. All Gram-positive cocci were sensitive to linezolid. Conclusion: Monitoring of data regarding the prevalence of microorganisms and its resistance patterns would help in currently prescribing antimicrobial regimens and improving the infection control practices by formulating policies for empirical antimicrobial therapy.
A 35-year-old male patient with HIV was admitted with complaints of painful swelling in his right lower limb. Doppler imaging showed deep vein thrombosis in the right leg. The patient had no other risk factors for DVT. He was treated with anticoagulation therapy and his symptoms improved. Deep vein thrombosis is more common in HIV-infected individuals due to factors causing a hypercoagulable state induced by the virus. The report examines the case of DVT in an HIV-positive patient without other risk factors.

Current options for active drugs in these patients include maraviroc, ibalizumab, fostemsavir, and enfuvirtide, which have novel mechanisms of action. Introduction: Bloodstream infections (BSIs) are associated with a high mortality rate of 20%-50%. Blood culture is paramount to identify causative agents of BSIs to choose an appropriate antimicrobial therapy. Objectives: The present study was undertaken to analyze the various microorganisms causing BSIs and study their antimicrobial resistance patterns in a tertiary care hospital, Eastern India. Materials and Methods: A total of 239 blood specimens from clinically suspected cases of BSIs were studied for 6 months from July 2015 to December 2015. Blood specimens were incubated in BacT/ALERT ® 3D system (bioMerieux, Durham, NC, USA) a fully automated blood culture system for detection of aerobic growth. Identification and antimicrobial susceptibility testing were conducted on VITEK ® 2 (bioMerieux, Durham, NC, USA) as per Clinical Laboratory Standards Institute guidelines. Results: Out of 239 specimens, 41 (17.2%) yielded growth of different microorganisms. From these isolates, 20 (48.8%) were Gram-negative bacilli, 18 (43.9%) were Gram-positive cocci and rest 3 (7.3%) were yeasts. Among Gram-negative bacilli, Klebsiella pneumoniae sub spp. pneumoniae (70%) was most commonly isolated. Coagulase-negative staphylococci (88.9%) were the most common isolate among Gram-positive cocci. All three Candida spp. isolated were nonalbicans Candida (two Candida tropicalis and one Candida krusei). Gram-negative isolates were least resistant to tigecycline and colistin. All Gram-positive cocci were sensitive to linezolid. Conclusion: Monitoring of data regarding the prevalence of microorganisms and its resistance patterns would help in currently prescribing antimicrobial regimens and improving the infection control practices by formulating policies for empirical antimicrobial therapy.
A 35-year-old male patient with HIV was admitted with complaints of painful swelling in his right lower limb. Doppler imaging showed deep vein thrombosis in the right leg. The patient had no other risk factors for DVT. He was treated with anticoagulation therapy and his symptoms improved. Deep vein thrombosis is more common in HIV-infected individuals due to factors causing a hypercoagulable state induced by the virus. The report examines the case of DVT in an HIV-positive patient without other risk factors.

This document discusses management of HIV in heavily treatment-experienced patients with multiclass resistance and limited treatment options. It provides an overview of the problem, including that some older patients were treated early in the HIV epidemic with less potent regimens, resulting in resistance. Younger patients may have congenital HIV and been treated long-term. Assessment of virologic failure and resistance testing are important to select an effective new regimen. Current options for active drugs in these patients include maraviroc, ibalizumab, fostemsavir, and enfuvirtide, which have novel mechanisms of action. Adherence assessment is also critical to determine if the current regimen may still be effective if taken as prescribed.

ABSTRACT
Objective: To determine the frequency of hepatitis C virus infection in patients with type 2 diabetes mellitus and to look for the common risk factors leading to this infection in diabetics. Study Design: Descriptive cross sectional study design. Place and Duration of Study: Department of Medicine, Combined Military Hospital (CMH) Kharian, from Jan 2015 to Jun 2015. Patients and Methods: This study was conducted in the department of Medicine, Combined Military Hospital Kharian. Through a descriptive cross sectional study design, a total of 140 patients with type 2 diabetes mellitus, admitted through casualty, OPD or private clinics were selected and tested for Hepatitis C virus infection. The common risk factors leading to such infection among positive cases were also scrutinized. Results: The mean age of patients was 48.82 ± 10.14 with 60.7% female gender predominating the overall sample of diabetics. Using 3rd generation ELISA method, hepatitis C virus was found in 45 (32.1%) of patients with 41-50 years of age group most commonly affected age group (34.7%) and female (57.8%) commonly affected gender. The distribution of risk factors leading to hepatitis C virus in diabetics are: 21 (46.7%) had history of surgery in the past, 13 (28.9%) had history of blood transfusion in the past, 7 (15.55%) had history of hemodialysis while only 4 (8.9%) had history of tattooing in the past. Conclusion: Hepatitis C virus infection is still a common problem in diabetic patients of our local population and we recommend further research work over its risk factors so that the guidelines for its control may be formulated. Hemorheological indexes, living habits, medical history and genetics factor are primary risk factors in Coronary Heart Disease (CHD). In the present study the relation of all factors to the severity of CHD was examined. This study aimed to identify determinants of illness severity for COVID-19 patients in China. Medical records from 598 COVID-19 patients admitted to four hospitals in China between January and March 2020 were analyzed. Patients were divided into moderate (n=400), severe (n=85), and critical (n=113) illness groups based on their condition. Ordinal logistic regression was used to identify predictors of more severe illness. The analysis found that older age, hypertension, abnormal liver enzymes and cardiac markers, longer time from illness onset to diagnosis and admission were associated with increased risk of more severe illness.

1) The study found that filaggrin (FLG) gene mutations are common in white patients with severe atopic dermatitis in the United States, with 48.5% of patients carrying a mutation. Compound heterozygous and homozygous mutations were found in 18.8% of patients.
2) Patients with homozygous or compound heterozygous FLG mutations had significantly higher serum immunoglobulin E levels than patients with heterozygous or wild-type FLG genes.
3) Screening for additional FLG mutations beyond the two most common identified more mutations than previous studies, showing the genetic basis of severe atopic dermatitis is more complex than previously understood.

The course of hepatitis B virus (HBV) infection is variable depending on many factors. In this study,
we investigated single nucleotide polymorphisms of interleukin-28B and interleukin-16 as possible host factors
which may determine the occurrence of hepatocellular carcinoma (HCC) in Saudi patients. Chronic hepatitis B
(CHB) patients (75), HCC patients (42), and healthy controls (70) were analyzed for polymorphisms of the IL16
and IL28B genes using PCR and restriction fragment length polymorphism (RFLP). Results showed that HCC
and chronic HBV patients had higher prevalence of rs11556218TG genotype than controls. The rs11556218GG
genotype was higher among HCC (14.4%) compared to chronic HBV (2.7%) patients. The IL-16 genotype
rs4072111CT was higher among HCC (47.6%) and chronic HBV (46.7%) patients than controls (28.6%). The
rs4072111TT genotype was higher among HCC patients compared to the other two groups. The T allele
frequency was higher among HCC patients than controls. The CT and TT of the IL-28B rs12979860 genotype
were significantly less frequent in chronic HBV and HCC patients. The IL-28B rs8099917 TG genotype was
more frequent among HCC (19%) compared to chronic HBV (8%) patients. However, no significant difference
was detected in the allele distribution.

Abstract—
Aim: The aim of the present work was to evaluate associations between the risk of endometriosis and -460C/T (rs833061) and +405G/C (rs2010963) polymorphisms in the VEGF gene.
Methodology: In the present study, we examined group of 100 patients with endometriosis and 100 controls. Genomic DNA was extracted from peripheral blood. Determination of genes polymorphic variants was made using polymerase chain reaction-restriction fragment length polymorphism technique (PCR-RFLP).
Results: Presented study showed statistically significant increase in the endometriosis development risk for the -460T/T genotype (OR 3.39; 95% CI, 1.60-7.13; p = 0.002) and for the -460T allele (OR 2.49; 95% CI, 1.64-3.78; p <.0001), as well as for the +405C/C genotype (OR 2.16; 95% CI, 1.047-4.48; p = 0.035) in patients with endometriosis in comparison with healthy control group. We also observed positive association of the +405C/C genotype (OR 0.26; 95% CI, 0.08-0.79; p = 0.019) as well as the +405C allele occurrence with an increased endometriosis development risk (OR 0.31; 95% CI, 0.19-0.71; p = 0.005), assessed by the degree of rASRM classification stages.
Conclusion: The results support the hypothesis that the -460C/T and +405G/C polymorphisms of the VEGF gene may be associated with endometriosis occurrence in Poland.

A 20-year-old woman presented with fever, fatigue, and weight loss. She had a history of Holt-Oram syndrome and primary EBV infection a year prior. Testing showed pancytopenia, hepatosplenomegaly, and elevated ferritin. A bone marrow biopsy showed hemophagocytosis. She was diagnosed with EBV-associated hemophagocytic syndrome and died despite treatment. Hemophagocytic syndrome is caused by impaired NK and cytotoxic T cell function leading to uncontrolled immune activation and cellular damage. It can be genetic, infection-associated, malignancy-associated, or associated with autoimmune diseases. Treatment involves controlling inflammation, the underlying trigger, and supportive care.

Background: The prevalence of toxoplasmosis in the general population of Guadalajara, Mexico, is around 32%.
Toxoplasmosis can cause ocular lesions and slowing of reaction reflexes. Latent toxoplasmosis has been related
with traffic accidents. We aimed to assess the prevalence of anti-Toxoplasma gondii antibodies and visual
impairments related with traffic accidents in drivers from the metropolitan Guadalajara.
Methods: We prospectively evaluated the prevalence of IgG and IgM anti-T. gondii antibodies in 159 individuals
involved in traffic accidents, and in 164 control drivers never involved in accidents. Cases of toxoplasmosis
reactivation or acute infection were detected by PCR in a subset of 71 drivers studied for the presence of T. gondii
DNA in blood samples. Ophthalmologic examinations were performed in drivers with IgG anti-T. gondii antibodies
in search of ocular toxoplasmosis.
Results: Fifty-four (34%) traffic accident drivers and 59 (36%) controls were positive to IgG anti-T. gondii antibodies
(p = 0.70). Among the 113 seropositive participants, mean anti-T. gondii IgG antibodies titers were higher in traffic
accident drivers than in controls (237.9 ± 308.5 IU/ml vs. 122.9 ± 112.7 IU/ml, respectively; p = 0.01 by Student’s t
test, p = 0.037 by Mann–Whitney U test). In multivariate analyses, anti-T. gondii IgG antibody titers were consistently
associated with an increased risk of traffic accidents, whereas age showed an inverse association. The presence of
IgM-anti-T. gondii antibodies was found in three (1.9%) subjects among traffic accident drives, and in two (1.2%)
controls. Three (4.2%) samples were positive for the presence of T. gondii DNA, all among seropositive individuals.
No signs of ocular toxoplasmosis were found in the entire cohort. Moreover, no other ocular conditions were found
to be associated with the risk of traffic accidents in a multivariate analysis.
Conclusions: Anti-T. gondii antibody titers are associated with the risk of traffic accidents. We could not determine
any association of ocular toxoplasmosis with traffic accidents. Our results warrant further analyses in order to clarify
the link between toxoplasmosis and traffic accidents.
This study examined the association between blood group antigens, CD4 cell count, and hemoglobin patterns in HIV-infected patients. 240 newly diagnosed HIV-positive patients and 120 healthy controls were analyzed. There was no significant association found between ABO/Rh blood groups or hemoglobin genotypes between the two groups. However, a significant association was observed between CD4 cell count and ABO blood group, with blood groups A and AB having higher average CD4 counts. The study suggests blood group A may confer some immunity against HIV infection.
