Diode laser applications in periodonticsMinkle Gulati
Periodontics extensively deals with soft tissue surgeries. However, pain, trauma, bleeding, postoperative
edema and scarring are often associated with conventional methods of surgery which are
problematic for both the patient and the clinician. Hence, to overcome these drawbacks, 940nm Diode
laser was used to perform various surgical procedures including frenectomy, gingivectomy,
vestibuloplasty, depigmentation and second stage implant surgery. The intra-operative and postoperative
advantages i.e. lack of swelling, bleeding, pain or scar tissue formation, and good wound
healing were observed in all the cases, verifying diode laser as an appropriate and beneficial device for
soft tissue surgeries.

Optical coherence tomography (OCT) is an established medical imaging technique that uses light to capture micrometer-resolution, three dimensional images from within optical scattering media (e.g., biological tissue). Optical coherence media (e.g., biological tissue). Optical coherence tomography is based on low coherence interferometry, typically employing near-infrared light. The use of relative long wavelength light allows it to penetrate into the scattering medium. Confocal microscopy, another optical technique, typically penetrates less deeply into the sample but with higher resolution.

Bilateral vocal fold paralysis is a rare condition. The commonest cause identified is mostly iatrogenic. It can be extremely debilitating for the patient who usually suffers from severe breathlessness on slightest exertion. Many of them requires a tracheostomy to maintain airway. The treatment of bilateral vocal cord palsy is a balance between phonation, airway and swallowing. Several surgical modalities have been described for cases which doesn’t improve with conservative management. However transoral CO2 laser endoscopic aryte-noidectomy has become the standard of management today for this condition. CO2 laser is arguably the most appropriate tool for cordectomy with the advantage of increased pre-cision, better hemostasis and minimal tissue handling. We describe the procedure of posterior cordectomy with partial arytenoidectomy using transoral CO2 laser in two pa-
tients who were successfully managed for this condition in our centre.

This procedure demanded precision and control. The Digital AcuBlade scanning micromanipulator provided the
highest degree of both for accessible areas. For areas inaccessible to the DAB, the FiberLase flexible CO2 laser fiber still
enabled a high level of precision and control.

Procedure
Because of retrognathia, it was very difficult to expose by direct laryngoscopy with
conventional laryngoscopes. Visualization of anterior 1/3 part of vocal folds and of
anterior commissure was impossible through the operative microscope.
Procedure performed was CO2 Laser-Assisted Microlaryngoscopy using Digital
AcuBlade for posterior and mid-parts of the cords and fiber-delivered CO2 laser
ablation using 30° rigid endoscope visualization for anterior cords and anterior
commisure.
Anesthesia Procedure performed under general anesthesia because of the extent of the
papillomatosis involving the two cords

Procedure Part A: Laser System #1 was used to ablate papillomatosis of posterior
and mid-parts of vocal cords under microscope visualization
Procedure Part B. Laser System #2 was used to ablate lesions of anterior parts of
vocal cords and of anterior commissure under 30° rigid scope visualization. The
distance from the tip of the fiber to tissue was 1-2 mm.
The end-point for ablation was when epithelium seemed fully ablated. Char was
removed with saline-soaked cottons to ensure there was no remaining epithelium.
Cidofovir injection (7.5 mg/ml) to affected areas at conclusion of procedure
Hemostasis Cautery was available, but was not used.
Hemostasis was accomplished by the laser or cottonoids soaked with saline
solution and adrenaline.

The goal of this surgery was superficial ablation of the epithelium. For controlling
the depth of ablation without risk of thermal damage in the Reinke’s space, the
REPEAT DELAY was used between Digital AcuBlade scans.

When using the FiberLase CO2 laser fiber, REPEAT mode was used to maintain
control and minimize thermal trauma to delicate tissues.
Visualization Use saline-soaked cottonoids to periodically wipe away char and permit good
assessment of treatment planes. Removal of char also helps minimize thermal
build up.

The patient experienced pain in the throat for 5-6 days
The patient has been back once for retreatment. His disease was significantly
lessened.
The voice was well improved
Discussion
In this patient, surgery was impossible to perform through the microscope. Another fiber –delivered laser
wavelength could have been used, but with the CO2 laser hollow fiber, you really see what you are doing
without risk of deep thermal damages – even at the level of the anterior commissure.
