This document provides a risk assessment matrix for hazards during a film shoot. It rates probability of occurrence from 1-5, consequence from 1-5, and calculates risk exposure by multiplying probability and consequence. Risk levels are classified as high, medium, or low. Four hazards are identified and rated: trip hazards pose low risk; traffic accidents medium risk; dropping cameras low risk; and other hazards medium risk. Control measures are suggested to mitigate each risk.

4. Probable - Less frequent, but expected

1. Negligible - no injury/damage to property
2. Minor - Minor injuries(cuts/bruises), minor damage to
property.
3. Serious - More serious injuries/damage to property.
4. Major - Major injury (hospitalized)
5. Disaster - fatality, catastrophic damage to property.

Trip Hazards 3 2 6 L Make sure there is no ice on floor, and walk slowly
Traffic Accident 2 5 10 M Drive safely, and use crossings for safety.
Dropping camera 3 1 3 L Hold camera carefully, make sure it’s safely attached to
tripod.
Other 3 3 9 M Be generally safe whilst filming to avoid risks.
