Dear Editor,
The authors have very comprehensively discussed the
subject with an emphasis on trauma triad, namely
hypothermia, coagulopathy, and acidosis.[1]
The vari-
ous preventive measures to be adopted are precisely
and clearly mentioned in a tabulated form. Respected
members of the fraternity will agree that such topics
should be presented in the CME programmes at the
Annual Conference of Association of Surgeons of In-
dia, where both senior and upcoming surgeons take
back home a message that, in a given trauma case, it is
not a must to complete the operative procedure as in a
planned surgery. The said approach thus should not
reflect an impression on the competency and efficien-
cy of the operating team. The relatives of the trauma
victim and the public at large (including politicians
and nontechnical administrators) must be made aware
of such an approach that is followed in the better in-
terest of the patient.

merous social and welfare organizations also provide
basic health services, as well as medical and financial
help to the needy.
It may be a good move for various medical associa-
tions to cooperate with the Government authorities and
plan out strategies to improve the existing health serv-
ices in our country. After all, improvements in public
health can be achieved only by a synergistic effort be-
tween the different players in the healthcare systems.
Thus, modern medicine is still in an evolutionary stage
and has a number of challenges ahead, before it can
be truly modern and available to all Indians.

The editorial by Dr. Shukla was thought provoking and
timely.[1]
Medicine, once considered a noble profession,
is now losing its supremacy and gradually evolving as
a business. The ‘service’ attitude has lost out to com-
mercial gains. The ‘brain-drain’ problem continues to
be on the rise. Increasing requirements of doctors in
‘areas-of-need’ in developed countries like UK and
Australia have lured many Indian doctors to serve in
places where native doctors might not prefer to work.
The Government healthcare infrastructure in India,
although well designed still faces many problems, the
most important one being lack of resources. Hence,
private health institutions and nursing homes form the
bulk of health providers in this country. Upcoming
corporate hospitals provide state-of-art medical facili-
ties that may well be beyond the reach of many. The
private-sector delivery of health care is therefore poorly
regulated, and prices are scaled according to the pay-
ing capacity of the patients. Health insurance schemes,
although nearly mandatory in developed nations, are
yet to reach the common people in India. Hence, in
spite of modern facilities being available in our coun-
try, they are inaccessible to most patients.
There are many instances of actions being undertaken
to improve the health facilities in our country. The ‘Aao

The researchers also explore the background of cultural competence and culturally competent health care frameworks that can take into existing health care programs to reduce disparities and health outcomes for all populations. This paper reveals that there are racial and ethnic disparities in the minority communities of India in the delivery of public health care services. There is a gap on how to remove theses critical problems and no attention have been paid to techniques for reducing them. Several studies demonstrated that cultural competence resulted in increased satisfaction with care, improved abilities to engage in treatment and recovery, reduced readmission rates and reduced mortality in health care. Previous studies also suggested that culturally competent organizations deliver improved quality health outcomes, increased respect and mutual understanding and fewer care disparities among various cultures. Researchers claim that cultural competence is the key strategy for reducing disparities care and inequalities in healthcare access and it is the effective tool to improve the quality and effectiveness of care for diverse cultures. Damage control resuscitation (DCR) evolved from the concept of damage control surgery. It involves early blood product transfusions using a 1:1:1 ratio of red blood cells, plasma, and platelets to address trauma-induced coagulopathy. It also focuses on immediately arresting hemorrhage, restoring blood volume, and correcting physiological abnormalities like hypothermia and acidosis. DCR can be applied anywhere to critically injured patients. Massive transfusion protocols that guide DCR have been shown to improve survival rates in patients with severe injuries and hemorrhage. Health care delivery to patients should be the best and of high quality. Health centers should ensure they work to ensure their clients gets maximum benefits from the services they are offering. Health services in the country have however been not to the best of their game with several factors affecting their efficiency. Most clients always complain of poor outcomes once they visit government health care facilities. The core of the establishment is poor and not able to help offer the best services the client expects. This is due to small amounts of funds allocated to health care sector by the government comparing to the needs of the citizens of the country (World Health Organization, 2003). This research paper will in deep length explain the problems that health care establishments faces in an effort to provide the best health care services to their clients. The stakeholders of healthcare sector, who are my audience, should ensure they serve their part effectively to ensure a smooth service delivery to their clients under all aspects. The technological, political and socioeconomic factors that pose these problems needs to be addressed and solutions provided to improve the health of the clients.
Healthcare sectors has faced all of these problems for over several centuries now. Technological advancement is continuous and the government is ought to keep up with the current advancement as it affects patient delivery. Infrastructure in hospitals should be well in cooperated with technology (Kaplan, Damuk, Lynch, Cohen, 1996). Government policies and regulations on healthcare sector affect service delivery. Most clients always complain of poor outcomes once they visit government health care facilities. The core of the establishment is poor and not able to help offer the best services the client expects. This is due to small amounts of funds allocated to health care sector by the government comparing to the needs of the citizens of the country (World Health Organization, 2003). This research paper will in deep length explain the problems that health care establishments faces in an effort to provide the best health care services to their clients. The stakeholders of healthcare sector, who are my audience, should ensure they serve their part effectively to ensure a smooth service delivery to their clients under all aspects. Patients who have complex health needs require not only medical. But also social services and support from a variety of caregivers and providers. Facility managers who are part of care coordination could assist patient in receiving optimal care by addressing the challenges in coordinating care for these patients, and offer programmatic changes and policies that help deliver the best services to all patients.
Facility managers can come up with strategic plans based on prior data and make necessary changes based on preexisting conditions. “Patient- centered, comprehensive, coordinated, and accessible care that continuously improved through a systems-based approach to quality and safety” (AHRQ, 2012) are what’s needed to achieve the highest quality care possible in any health care facility.
Patient centered care can’t be achieved with providers only. It requires team work and collaboration among all stakeholders. To improve the quality and safety of patients, health care facility managers can work hand and hand with the coordinated team to provide a system based approach by drawing on decision-support tools, taking into account patient experience, and using population health management approach. Patient preference and needs on what aspects of care to be improved.

As a facility manager, and part of the care coordination team, I would look into models of care that would assist our situation. With the Affordable Care Act in place, there are accountable care organizations (ACOs), which provide models of care (“Promise,” 2013). There are many different definitions and perspectives on care coordination, but all lead to the goal of meeting patient needs and providing adequate healthcare (“Care,” 2014).
Care coordination is essential because each patient can interact with a variety of professionals each visit. For example, for a routine physical appointment, the patient could meet with the scheduling staff, medical assistants, nurses, doctors, pharmacists, and the billing staff. If each one of these member fails to coordinate as a whole, the patient could be harmed or neglected. As a care coordinator, I would be responsible for discussing an individualized care plan with each patient and ensuring that they understand their responsibilities. All barriers should be identified, such as financial, social (language), psychological, and anything that would effect the patient from following their correct plan of care and interacting with the staff (“Promise,” 2013). Another key point is to ensure the medical staff has reviewed the patient’s medical records and ensure that everyone is on the same page. These are just a few examples, because each case is different and each patient will have different needs. .

PGx Analysis in VarSeq: A User’s Perspective
Since our release of the PGx capabilities in VarSeq, we’ve had a few months to gather some insights from various use cases. Some users approach PGx workflows by means of array genotyping or what seems to be a growing trend of adding the star allele calling to the existing NGS pipeline for whole genome data. Luckily, both approaches are supported with the VarSeq software platform. The genotyping method being used will also dictate what the scope of the tertiary analysis will be. For example, are your PGx reports a standalone pipeline or would your lab’s goal be to handle a dual-purpose workflow and report on PGx + Diagnostic findings.

Breast cancer: Post menopausal endocrine therapy
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

GASTROINTESTINAL INFECTIONS result from the ingestion of pathogens that cause infections at the level of this tract, generally being transmitted by food, water and hands contaminated by microorganisms such as E. coli, Salmonella, Shigella, Vibrio cholerae, Campylobacter, Staphylococcus, Rotavirus among others that are generally contained in feces, thus configuring a FECAL-ORAL type of transmission.
Among the factors that lead to the occurrence of gastrointestinal infections are the hygienic and sanitary deficiencies that characterize our markets and other places where raw or cooked food is sold, poor environmental sanitation in communities, deficiencies in water treatment (or in the process of its plumbing), risky hygienic-sanitary habits (not washing hands after major and/or minor needs), among others.
These are generally consequences (signs and symptoms) resulting from gastrointestinal infections: diarrhea, vomiting, fever and malaise, among others.
The treatment consists of replacing lost liquids and electrolytes (drinking drinking water and other recommended liquids, including consumption of juicy fruits such as papayas, apples, pears, among others that contain water in their composition).
To prevent this, it is necessary to promote health education, improve the hygienic-sanitary conditions of markets and communities in general as a way of promoting, preserving and prolonging PUBLIC HEALTH.
Gastritis and Gastric Health
Gastric Health is one of the most relevant concerns in human health, with gastrointestinal infections being among the main illnesses that affect humans.
Among gastric problems, we have GASTRITIS AND GASTRIC ULCERS as the main public health problems. Gastritis and gastric ulcers normally result from inflammation and corrosion of the walls of the stomach (gastric mucosa) and are generally associated (caused) by the bacterium Helicobacter pylor, which, according to the literature, this bacterium settles on these walls (of the stomach) and starts to release urease that ends up altering the normal pH of the stomach (acid), which leads to inflammation and corrosion of the mucous membranes and consequent gastritis or ulcers, respectively.
In addition to bacterial infections, gastritis and gastric ulcers are associated with several factors, with emphasis on prolonged fasting, chemical substances including drugs, alcohol, foods with strong seasonings including chilli, which ends up causing inflammation of the stomach walls and/or corrosion. of the same, resulting in the appearance of wounds and consequent gastritis or ulcers, respectively.

Nu- merous social and welfare organizations also provide basic health services, as well as medical and financial help to the needy. It may be a good move for various medical associa- tions to cooperate with the Government authorities and plan out strategies to improve the existing health serv- ices in our country. After all, improvements in public health can be achieved only by a synergistic effort be- tween the different players in the healthcare systems. Dear Editor, The editorial by Dr. Shukla was thought provoking and timely.[1] Medicine, once considered a noble profession, is now losing its supremacy and gradually evolving as a business. The ‘service’ attitude has lost out to com- mercial gains. The ‘brain-drain’ problem continues to be on the rise. Increasing requirements of doctors in ‘areas-of-need’ in developed countries like UK and Australia have lured many Indian doctors to serve in places where native doctors might not prefer to work. The Government healthcare infrastructure in India, although well designed still faces many problems, the most important one being lack of resources. Hence, private health institutions and nursing homes form the bulk of health providers in this country. Upcoming corporate hospitals provide state-of-art medical facili- ties that may well be beyond the reach of many. The private-sector delivery of health care is therefore poorly regulated, and prices are scaled according to the pay- ing capacity of the patients. Health insurance schemes, although nearly mandatory in developed nations, are yet to reach the common people in India. Hence, in spite of modern facilities being available in our coun- try, they are inaccessible to most patients. There are many instances of actions being undertaken to improve the health facilities in our country.
