• "The new rules would require crawler cranes to stop operating and go into safety
mode under two scenarios: when there is a forecast for steady wind speeds of 20
miles per hour or higher or gusts of at least 30 m.p.h., and when actual readings
reach those levels.

m.p.h. or gusts increased to 40 m.p.h., though individual manufacturers
sometimes set stricter standards.“

stand behind the safety of the
bearing, especially a weld holding
it to a spacer ring. “Honestly
speaking, we don’t have
confidence on this welding,”
Last August, DOB moved to shut him
down for a pattern of safety violations.
Lomma is fighting them.

documented and maintained by
the employer that conducts the
inspection:
• The items checked and the results
of the inspection.
• The name and signature of the
person who conducted the
inspection and the date.
• This document must be retained
for a minimum of three months.

equipment must be inspected by
a qualified person

• Bolts, rivets and other fasteners:

manual, must be readily
available in the cab at all times
for use by the operator.

  • 25. Operations • The employer must comply with all manufacturer procedures applicable to the operational functions of equipment, including its use with attachments. 1926.1417(a)

Operations • The procedures applicable to the operation of the equipment, including rated capacities (load charts), recommended operating speeds, special hazard warnings, instructions, and operator's manual, must be readily available in the cab at all times for use by the operator. 1926.1417(c)(1)

  1. One theory, Mr. Loughlin said, was that the pile had struck a very soft spot of river mud and had begun to sink rapidly, jolting the 60-ton hammer into a rapid drop and putting a particular strain on the boom, which buckled. Mr. Loughlin, emphasizing that he could only speculate about what had happened until the investigations were complete, said that an operator on an old-fashioned crane with a foot-operated brake might have taken his foot off the brake and allowed the load of the hammer to go into a free fall, relieving the strain before the buckling occurred.
  2. Terry Quirk a consultant and retired crane operator in Alaska, said in a telephone interview and follow-up email that the first thing he noticed in the video is that the “luffing jib,” the latticed boom which fell in a mangled red heap on Worth Street, was being lowered with its “headache ball,” a round object with a single cable and hook, still hanging high in the air off the jib. That was not done in accordance with standard procedure dictated by the crane manufacturer, he said. Quirk said that he also noticed from the video that the main boom, closest to the cab, was being lowered before the luffing jib was firmly on the ground or in a vertical position. “This is not in accordance with the manufacturer’s procedures for raising or lowering a luffing jib,” said Quirk, adding that those two factors in his opinion constituted operator error
