This document establishes the procedure for completing supplemental investigation reports for field operations personnel. It specifies that any time an investigation is initiated, a supplemental report must be completed. The report must be typed and include details of the incident, individuals involved, violations, and other essential information. It provides examples of properly formatted supplemental reports.

I. PURPOSE
To establish a uniform procedure for the initiation and completion of the Supplemental
Investigation Report.
II. POLICY
Field Operations personnel are responsible for completing a Supplemental Investigation Report
anytime an investigation is initiated.

VII. PROCEDURE
A. The Supplemental Investigation Report may be used in conjunction with other forms
utilized by Field Operations. The Supplemental Investigation Report is also used
independently for special reports.
B. All Supplemental Investigation Reports are to be typed and completed as follows (refer
to numbered sections on the sample):
1. Enter the date and time the incident was reported, expressed in numerals to
indicate month, day, year and the twelve-hour designation (e.g., 09/12/05 at 8:30
AM).

2. Enter the date and time the incident occurred, expressed in numerals to indicate
month, day, year and the twelve-hour designation (e.g., 09/12/05 at 8:30 AM.)
3. Enter the Detectives Report number or Incident number assigned by the
investigating police agency or sheriff’s department, if applicable.
4. Enter the Chameleon Activity number.
5. Enter the citation control number of any citation issued during the course of the
investigation if applicable (e.g., police, sheriff or OCAC citation number).
6. Enter the entire violation code section(s) and title accurately and verbatim,
including capital letters (e.g., California Penal Code 597 Crimes Against
Animals).
a. Enter any additional explanation of section 5 or if no violation code is
applicable, enter the justification for the Supplemental Investigation
Report.
7. Check the appropriate box indicating if the Animal Owner is the victim, suspect
or complainant. The box labeled as Other must be completed in the same
fashion as the Animal Owner, if applicable.
a. Enter the full name(s) of the victim(s), complainant(s) or suspect(s)
involved in the investigation.
8. Enter the complete addresses of the Animal Owner and/or Other persons
involved in the investigation. The address is to be written out per the US Postal
Service’s guidelines.
9. Enter the telephone number, including area code, of the Animal Owner and/or
Other persons involved in the investigation.
10. Provide a detailed, clear, and concise report of the events pertaining to the
investigation. The report is to be written in the first person and contain no radio
codes, jargon, symbols, or abbreviations (e.g., @, Hrs., OCCO, CPC,
OCAC, ACO, Sgt., Lt., DR, etc.) The abbreviated OC in OC Animal Care is
acceptable as this is the official name of the agency.
a. If the suspect is arrested and/or issued a citation by the Police
Department the citation number and the suspect’s date of birth must be
obtained and included in your report. (Obtain the suspect’s driver’s
license number if and when possible).
11. Enter the first initial, last name and badge number of the officer completing the
report and manually sign your signature next to the type.

12. Enter the date and time the report was completed. The date and time are to be
expressed in numerals to indicate month, day and year (e.g. 11/21/05 at 7:30
PM.)
13. The supervisor reviewing the report is to sign their name here.
14. The supervisor reviewing the report is to enter the date the report was reviewed
by the supervisor expressed in numerals to indicate month, day and year (e.g.
11/21/05).
15. Enter the person, place and/or section to whom or where additional copies were
sent (e.g. Brea Police Department, Business Licensing).
C. Additional Information:
1. Enter N/A (not applicable) or Unknown for any blank windows as required.
2. Space down 2 or 3 times on the second and third page etc. before typing text so
no text is removed by the hole-punch when filed.
3. When a report is completed and ends a portion of the way down the page, space
down to ensure the signature boxes are at the bottom of the last page.
4. A well written report will allow the reader to see the scene through verbal
images.

On Friday, January 21, 2011, at 2:18 PM while working as a uniformed Animal
Control Officer for the County of Orange and driving a marked Animal Control truck, I
received a report of a dog locked in a white Honda Accord, California License Plate

reporting party was the Sunset Harbor Patrol. The Orange County Sheriff Department
later reported that the bird was run over intentionally and the witness was still on
scene.

On August 30, 2010, at 1:35 PM, while working as a Senior Animal Control Officer for
the County of Orange and driving a marked Animal Control truck, I received a report
for possible animal cruelty against an opossum at 16505 Caballero Drive in the City of
Huntington Beach.
