I. PURPOSE
To establish a uniform procedure for completing the Chameleon Database Activity
window.
II. POLICY
An Activity window shall be initiated and completed for every service requested of OC
Animal Care. The Activity window shall be completed as accurately and detailed as
possible to provide a clear and concise account of the services performed.

VII. PROCEDURE
A. Creating an Activity (attachment A)
1. Activity No.: This box should be blank when storing a new activity. The
number will be automatically generated upon storing the window. The
first two digits indicate the year the activity was initiated.
2. Seq.: This field is to be left blank unless the activity requires an additional
sequence number. If so, type in the next sequential number prior to storing
the new sequence. Otherwise, this field will be automatically generated
upon storing the window.

3. Type: Press F4 and select the appropriate type of call to be created.
4. Subtype: Press F4 and select the appropriate subtype of call to be created.
5. Priority: Enter the activities priority based on the attached priority list.
a. PD: (Priority 1) Person in Danger or at Risk (Code 2).
b. AD: (Priority 2) Animal in Danger or at Risk. (Code 2).
c. UR: (Priority 3) Urgent-Next Available Officer. (Code 2).
d. TO: (Priority 4) To Be Done Today Per Supervisor.
e. NO: (Priority 5) Normal Priority.
f. LO: (Priority 6) Low Priority.
6. Danger: If the call has a Priority status of PD, AD or UR as noted above,
enter ‘Y’ to indicate the call is urgent.
7. Total: Enter the total number of animals being reported.
8. Type: Press F4 and enter the appropriate type of animal(s) being reported.
9. Tag No.: Enter the OC Animal Care dog license number, if available. The
Extra 1 field to the right of the Tag No. box is to be left blank.
10. Phone: Enter the areas code and telephone number of the caller if known.
11. Caller ID: Complete Person Window (Policy and Procedure 500.01) for
the person reporting the activity.
12. No: Enter the numeric street address of the activity address.
13. Sfx: Leave blank
14. Dir.: Enter the street direction (N, S, E, W.) of the activity address.
15. Name: Enter the street name of the activity address.
16. Type: Enter the street type (Ln, Dr, Cir, etc.) of the activity address.
17. Qdt: Enter the address quadrant if necessary (apartment building number).
18. Apt.: Enter the apartment, space, etc., of the activity address, if known.
19. Enter the complete city name of the activity address.
20. Default is CA.
21. Enter the zip code of the activity address.
22. This is the “Address Checker” or “Address Verifier”. After entering the
street numerics and name, click on the house icon. A Dialog Box should
come up if there is an exact match. Click the correct address in the Dialog
Box and boxes 19 thru 21 as well as 28 will be populated. If there is no
match you must enter the information manually.
23. Common Place Name: this is where you enter the name of the apartment
complex, condominium complex name, business name, etc.
24. Cross Street Low: Enter a major street that intersects with #25
25. Cross Street High: Enter another major street that intersects with #24

26. Comment: This is to be used to complete the appropriate sequence
comments for an officer’s electronic log. Any comments that may assist
in the handling of the call or lengthy comments must be entered in a
Memo Window and attached to the activity.
27. Geo: Press F4 and enter the geographical area of the activity address.
28. Jurisdiction: Press F4 and enter the city of the activity address. The
jurisdiction should be a city within OC Animal Care’s jurisdiction. In
the event an officer transports an animal to a city outside of the city where
it was originally impounded, the jurisdiction must reflect the city where
the animal was originally impounded for billing purposes. In the event an
officer is required to provide mutual aid to an outside animal control
agency, that city should be listed in the Jurisdiction field. If the city is not
listed, “out of jurisdiction” can be used. Also remember to take note of
the freeway calls and calls that are in unincorporated areas. These should
be either listed as “CO” after the city we service or “Orange Co.”
29. Map Page: Enter the appropriate Thomas Brothers map page.
30. Latitude: Leave blank
31. Longitude: Leave blank.
32. Map Icon: After the activity is stored, clicking on the map icon should
show you a map of the area surrounding the activity address.
33. Animal ID: If an animal is impounded, the person impounding will fill in
this field. If you have run a license tag or microchip # in Chameleon and
are sure it is the same animal, you can enter that Animal ID here. DO
NOT create an animal ID when simply initiating an activity. The animal
might not be located or the animal may already have a microchip or tag.
NOTE: Only one animal ID number may be attached to each activity
sequence. If multiple animals are impounded, additional sequences will
need to be created. Additionally, each activity number will only accept
100 sequences. In the event you need additional sequences, you must
create a new activity and associate it with the previous activity in a
memo.
34. Owner ID: If the person contacted has an existing PID, enter that PID in
this field, again do not create a PID on an activity when writing it up.
35. Status: Press F4 and choose the appropriate selection. If the call is stored
with a status of ‘New,’ the call will be sent directly to dispatch.
NOTE:All Field Initiated Calls (FIC’s) shall be stored with a status of
‘Completed’.
36. Call: The date/time the request for service was originated.
37. New: The date/time the request for service is to be handled.

38. Dispatch: The date/time the request for service is dispatched.
39. Working: The date/time the officer arrived on scene.
40. Complete: The date/time the officer completed the call.
NOTE: Fields 36 thru 40 will generally be completed by the system based
on the routing of the call through the Dispatch Window. However, it is the
responsibility of the officer handling the request for service to ensure that
the dates and times for fields 36 thru 40 are correct and consistent with
when the service was provided.
41. Clerk ID: Leave blank. When the Activity window is stored, this box will
show user’s name
42. Officer ID: When the call has been completed, the officer handing the
request for service shall enter his/her Person ID number. If the call is new
or is to be placed in pending, leave area blank.
43. Cancel By: When a call is cancelled through the Dispatch window, the
user’s name will show up here.
44. Reason: When a call is cancelled through the Dispatch window, the
reason is to be entered and it will show up here.
45. ChamAlert: Leave blank.
46. Qty: Enter the numeric value that directly corresponds with the ‘Result”
field. Only one AID can be attached per sequence. If more than 1 animal
(including a litter) is impounded, the Qty shall be 1 for each AID. Each
AID and its corresponding impound Result code(s) will be on a separate
sequence.
47. Result: Press F4 and enter the appropriate result typ.
48. Activity Search: Type in a street name and click on this box. It will show
you all activities on that particular street. If you have the exact address,
you can enter that as well.
49. License Search: Again, by entering an address, you can click on this box
to find any animals licensed or linked to that particular address.
B. Specific Information
1. Field Initiated Calls (FIC). For each FIC completed, the officer shall
create and complete an Activity Window. FIC Activity windows follow
the same instructions as above with the following exceptions:
a. Caller ID: Enter the Person ID number of the officer who initiated
the call.
b. No.: If no specific address is appropriate, enter 0.

c. Name: If no specific address is appropriate, enter one of the major
cross streets.
d. Comment: Enter the other major cross street and/or any other
pertinent information here.
e. See 35 above.
2. Vicious/Potentially Dangerous Dog Investigations. A separate sequence
shall be initiated for each date that an officer works on the investigation
and for each location that is contacted when the officer is canvassing. It
will be that officer’s responsibility to close out the call with the
appropriate result code(s) and generate the next sequence to be placed in
‘pending’. All completed Vicious/Potentially Dangerous Dog
Investigations shall have a supplemental report written using Microsoft
Word. The supplemental report is not to be written in the Chameleon
Memo Window.
3. Bite Reports. All bite reports not initiated in the field (e.g., by rabies desk
or clerical staff) will have a new activity sequence generated for each
officer that attempts service for that bite report activity. The officer who
attempts service for his/her sequence will be responsible for the
disposition (result and times) and/or the re-filing of the call for the
following shift.
4. Compliance Checks. Officers who are unable to successfully complete a
compliance check shall have the activity re-filed through dispatch with the
appropriate result code. This will generate a new sequence for the
following officer who will then be responsible for the proper disposition
of his/her activity sequence.
5. Re-files. With the exception of B.2 and B.3., all calls requiring re-file for
the following shift or officer follow-up shall be placed in pending by the
Dispatch Services Operator (DSO) utilizing the Dispatch View only. The
DSO is required to input the appropriate result(s) in the Results Window.
Officers shall not personally place calls in ‘pending’ without prior
authorization from the Watch Commander (WC). (The WC’s name and
the purpose of the re-file will be noted in the Activity Memo by the officer
placing the call in pending).
6. Completed Calls. All officers shall close out each Activity, prior to
handling the next Activity. They should enter the results themselves
whenever possible and must advise the DSO that they have completed that
particular call. Officers shall not call the DSO on the Dispatch Service line
or use their personal cell phones to close out Activities unless specifically

requested to do so by the DSO or supervisor. Officers are responsible for
ensuring that all dispositions are accurate and any needed corrections are
made in Chameleon prior to the end of their shift.
7. Memos. All activity Memos shall be entered prior to the end of shift. An
Activity Memo is required any time the Activity Result code does not
sufficiently explain the outcome of an Activity. (Examples: Cruelty
Investigations, Dog vs. Dog, Agency Assists, etc.).
8. When creating an Activity for a cruelty call, a memo must be generated.
Any information the RP provided is to go on memo, i.e. what the RP tells
you, why they called, what they say the nature of the complaint is, etc.
These memos become part of the package in case of litigation etc.
