2. Case – 1
• S/c engaged tipper, bearing Reg. No XX XX XXXX for
shifting L&T Formwork Materials from site to Yard.
• (The vehicle was certified by our P&M In charge and Safety
Engineer through PM 32 – B – Rev 00 and inspected on by
P&M In charge and SSC.)
• During Lunch hour, the driver parked the vehicle, in
between OB 1 and Cafeteria, by applying parking brake and
placing boulders in rear wheel as stoppers.
• After lunch the driver was advised to remove the vehicle
immediately to give way to the tractor accessing towards the
passage.
• Driver disengaged the parking brake and tried starting the
vehicle. As the vehicle was in slope, the vehicle began to
move before getting started.
• Since the Tipper was moving in the slope at a great speed,
the driver could not control the vehicle by brake and the
vehicle ran over victim, who was crossing the road.

Direct Cause;
• Brake was not effective due to no air in air tank,
• Boulders placed as stop – end was not effective,
• Vehicle was parked in the slope,
• Panic,
Proximity Cause;
• Induction & screening wasn’t done,
• System of Identifying Un - authorized drivers was not
implemented strictly,
5. Case – 1
Precautionary Measure ;
• Photo identification gate pass is being issued to avoid
usage of someone else's gate pass to gain entry.
• The photo pass shall be issued strictly,
• A sticker shall be placed on the helmet indicating that
the person is inducted.
• This can be checked on a periodical basis by the
Safety Engineer.
• Periodical training on Defensive Driving technique,

• 145 mm dia Drilling completed upto 35 m and casing
of 150mm installed upto 10 m, (Sketch 1)
• Lowering of 5” PVC piping into the drilled borehole,
but not succeeded beyond 10 m depth, hence
withdrawn
• Decided to abandon the borehole and retrieve the
installed 150mm casing
• Extracted the casing by 4.0 m and arrangements
started for cutting the casing
• Workers got trapped suddenly into the sand near the
casing.
• 2 workers were trapped, 1st was trapped upto chest
level and 2nd at 2.35 m below the ground level.
• Rescue operation commenced, 1st worker was rescued
but 2nd worker couldn’t.

Direct Cause
• Cavity below the ground level - the person sunk down
Proximity Cause
• Failure to predict the hazard and severity,
• Due to this reason no controlling measures was also
not provided while person stands near the rig inst
near the bore hole
Precautionary Measures
• Structural platform made of MS plate 2 x 2 M size
touching 1 m radius periphery which will be rested on
"I" beam placed across.
11. Case – 3
• The victim had gone to the top of the ICE
MAKER,
• Removed the inspection cover, made up of
transparent cover,
• Bail out water by putting hand inside the ICE
MAKER.
• Part of his hand inside the ICEPLANT got
entangled and he could not remove his hands

16. Case – 4
• The victim was engaged in fixing a post (size – H beam 244
x 252 x 11, 13.38 m long, 881 kgs) over the cantilever
beam at + 65.9 m elevation.
• The post was lifted (4 nos bolt) over the cantilever beam
(hand tightened).
• The post was already tied with guy rope (20 mm PP rope)
to the adjacent structural member for vertical support.
• After bolting he went to release the slings of winch rope.
• After releasing the sling, all of a sudden the post was
tilted one side twisting the cantilever beam where the post
was bolted.
• He was hanged on the safety belt along with the post
hitting against the adjacent beam where the post was
rested. He got his injury in his face.
• He was having safety helmet with chin strap on, safety
belt & shoe.

Direct Cause;
• Fastening was not sufficient to
maintain its balance under load &
hence got twisted,
• Failure to provide sufficient no of
Guy ropes,
Proximity Cause ;
• Safe work procedures not prepared,
