Introduction
For care provision to be effective in the nursing field today, people are turning more towards the provision of community-based care (Lundy, Janes & Lundy, 2016). This requires one to clearly understand the specific care needs of the community to come up with programs to meet these needs. In the paper, we are going to identify a focus area I could work on in my community, the various health risks they can are vulnerable to and how to carry out a community assessment on my focus area.
Identify an area of focus in community health within your own community.
The health area that I would be highly interested in my community is lifestyle improvement. Lifestyle diseases have become a major health concern in my community since we have recording a great number of diseases such as child and adult obesity, diabetes and gout among many other lifestyle diseases. The main cause of this is making wrong lifestyle choices. My interest in this area is to help provide education to the community on lifestyle choices and what they ought to avoid and therefore help reduce the number of lifestyle diseases being recorded and also ensure that they are able to access healthy food choices.
Using Gordon’s Functional Health Patterns framework (p. 130 in your e-text), assess the health risks in your community.
· Health perception-health management pattern: Individuals in the community have not embraced primary care interventions. They often seek medical care when they have already contacted diseases or when a disease is already in its chronic stages making management harder.
· Nutritional-metabolic pattern: Individuals are at higher risk of lifestyle disease due to the pattern of fast food consumption. The presence of so many fast foods joints has encouraged this behavior especially for the children

Health Needs Assessment
Health assessment can be defined as a care program which involves the identification of special needs of person or a group of people and the way those needs are addressed by health facilities or the entire health system. Health assessment also involves the evaluation of the health status of an individual(s) through the performance of a physical examination after recording their health history. Health assessments are different from diagnostic tests because the latter is carried out when a person is already exhibiting the signs and/or symptoms of a particular disease (Turnock, 2012).
Measure of Public Health
Measures used in assessing health are different and the first measure of public health is mortality. Mortality is the rate of deaths occurring in a particular population. It has been very common for the numbers and rates of death to be used in measuring public health. Globally, some diseases such as cancer, cardiovascular diseases, diabetes and hypertension among others have been observed to be the leading causes of death. In order for policies to be formulated mortalities which are specific on particular age groups are considered as they provide more awareness on health status of that age group. The same way, when mortality data is stratified on the basis of ethnicity or race, the health disparities available are quantified (Pennel, McLeroy, Burdine, Matarrita-Cascante & Wang, 2016).
Morbidity is the second measure that is used to measure public health. It can literally be said to mean the rate of incidence of a disease or illness in a specified group of individuals or a population. This rate of morbidity can be estimated through use of the rates of hospitalizations recorded among a group or a population. This kind of measure is easy and advantageous in that it is not difficult to get access to the rates of hospitalizations. Although they are of very good use when carrying out certain analyses, they can be biased indicators of the health status (Turnock, 2012). For example, in cases where there are increasing rates of outpatient treatment when handling conditions which require hospitalization can adversely and substantially affect the usefulness of the information or data recorded for assessing health status.
Measuring disability is another dimension of morbidity that looks into non-fatal health complications. Certain problems such as pain in joints and bones often a result of arthritis can be said to be main contributors of disability. Other chronic conditions such as lung problems, heart disease, stroke, diabetes etcetera are also known to be causers of disability. our support and empowerment. As professionals, social workers experience first‐hand
the effects of the met and unmet patient needs, which brings with it a responsibility to
insure that practice and policy decisions give full recognition to the impact of
psychosocial aspects and services that provide total care to chronically ill older adults
and their caregivers.
This section describes some of the most recent literature addressing the role of
social workers in managing chronic illness care specifically related to conducting
biopsychosocial assessments, providing interventions, and in designing and
implementing effective models of health services delivery such as care coordination.
Characteristics of Chronic Illness
as They Impact the Social Work Role
Three important characteristics of chronic illnesses among older adults need to be considered
as they affect the social work role and function.
1. The trajectory for many serious illnesses has changed from an acute terminal
course to a much longer chronic period, with episodes of exacerbations and
remissions interspersed with extended periods of good functioning.
2. The trajectory of advanced chronic and terminal illnesses has changed from a
relatively brief period to a longer period in which both curative and

1. Theories and Models
This project will be guided by both occupational based model Person-Environment- Person-Participation known as PEOP and other related knowledge theory bases related to immigration studies. All of these theories are explained as following. The detailed explanations about these theories are eliminated and the main features of them that are align with this project are illustrated.
1.1. Person-Environment-Occupation-Performance (PEOP) Model
The Person-Environment-Occupation-Performance (PEOP) Model was generated in 1985 and published in 1991. Development of this model was a response to need for more occupational based models over the paradigm shift. (Christiansen & Baum, 1991, 1997; Christiansen et al., 2005). It is envisioned that although there are some similarities with other models, yet different from other models in terms of its focus on occupational performance and participation, as well as using a top-down approach (Christiansen et al., 2005). The PEOP model is defined as a system model that looks at the function in the systems as a whole and considers the interaction over its components. Occupational performance has been received close attention in the PEOP model and contains three main components: (1) characteristics of the person (including physiological, psychological, motor, sensory/perceptual, cognitive, or spiritual), (2) characteristics of the environment (including cultural, social support, social determinants, and social capital, physical and natural environments, health education and public policy, assistive technology), and (3) characteristics of the activity, task, or role. The most recent edition is developed to provide therapists to identify the client’s enablers and barriers to occupational performance, and might be employed both individual and organizational/community based (Christiansen et al., 2005). PEOP pays close attention to the importance of occupational competency in order to attain occupational participation. attention it deserves Mental health. I
chose this topic because I am personally effected by it and so are many millions of Americans. Mental illness is also one of the leading causes of
death in our nation and one life is lost as a result of suicide, abuse or incarceration every 17mins in the United States. Mental illness has been my
area of focus throughout this program and the advocacy and participatory philosophy will be useful for the final project because it suggests that “
that research inquiry needs to be intertwined with politics and a political agenda” (Creswell, p.9). I do believe that mental health has a specific
agenda for a study and that there has been constant aim for reform in healthcare and mental health. This social issue is definitely pertinent right

Introduction
For care provision to be effective in the nursing field today, people are turning more towards the provision of community-based care (Lundy, Janes & Lundy, 2016). This requires one to clearly understand the specific care needs of the community to come up with programs to meet these needs. In the paper, we are going to identify a focus area I could work on in my community, the various health risks they can are vulnerable to and how to carry out a community assessment on my focus area.
Identify an area of focus in community health within your own community.
The health area that I would be highly interested in my community is lifestyle improvement. Lifestyle diseases have become a major health concern in my community since we have recording a great number of diseases such as child and adult obesity, diabetes and gout among many other lifestyle diseases. The main cause of this is making wrong lifestyle choices. My interest in this area is to help provide education to the community on lifestyle choices and what they ought to avoid and therefore help reduce the number of lifestyle diseases being recorded and also ensure that they are able to access healthy food choices.
Using Gordon’s Functional Health Patterns framework (p. 130 in your e-text), assess the health risks in your community.
· Health perception-health management pattern: Individuals in the community have not embraced primary care interventions. They often seek medical care when they have already contacted diseases or when a disease is already in its chronic stages making management harder.
· Nutritional-metabolic pattern: Individuals are at higher risk of lifestyle disease due to the pattern of fast food consumption. The presence of so many fast foods joints has encouraged this behavior especially for the children

· Activity-exercise pattern: Individuals are actively involved in recreation activities but have very poor physical exercise patterns. This paired with the consumption of fast food such as burgers has led to increase in obesity rates.

Health Needs Assessment
Health assessment can be defined as a care program which involves the identification of special needs of person or a group of people and the way those needs are addressed by health facilities or the entire health system. Health assessment also involves the evaluation of the health status of an individual(s) through the performance of a physical examination after recording their health history. Health assessments are different from diagnostic tests because the latter is carried out when a person is already exhibiting the signs and/or symptoms of a particular disease (Turnock, 2012).
Measure of Public Health
Measures used in assessing health are different and the first measure of public health is mortality. Mortality is the rate of deaths occurring in a particular population. It has been very common for the numbers and rates of death to be used in measuring public health. Globally, some diseases such as cancer, cardiovascular diseases, diabetes and hypertension among others have been observed to be the leading causes of death. In order for policies to be formulated mortalities which are specific on particular age groups are considered as they provide more awareness on health status of that age group. The same way, when mortality data is stratified on the basis of ethnicity or race, the health disparities available are quantified (Pennel, McLeroy, Burdine, Matarrita-Cascante & Wang, 2016).
Morbidity is the second measure that is used to measure public health. It can literally be said to mean the rate of incidence of a disease or illness in a specified group of individuals or a population. This rate of morbidity can be estimated through use of the rates of hospitalizations recorded among a group or a population. This kind of measure is easy and advantageous in that it is not difficult to get access to the rates of hospitalizations. Although they are of very good use when carrying out certain analyses, they can be biased indicators of the health status (Turnock, 2012). For example, in cases where there are increasing rates of outpatient treatment when handling conditions which require hospitalization can adversely and substantially affect the usefulness of the information or data recorded for assessing health status.
Measuring disability is another dimension of morbidity that looks into non-fatal health complications. Certain problems such as pain in joints and bones often a result of arthritis can be said to be main contributors of disability. Other chronic conditions such as lung problems, heart disease, stroke, diabetes etcetera are also known to be causers of disability. 1. Theories and Models
This project will be guided by both occupational based model Person-Environment- Person-Participation known as PEOP and other related knowledge theory bases related to immigration studies. All of these theories are explained as following. The detailed explanations about these theories are eliminated and the main features of them that are align with this project are illustrated.
1.1. Person-Environment-Occupation-Performance (PEOP) Model
The Person-Environment-Occupation-Performance (PEOP) Model was generated in 1985 and published in 1991. Development of this model was a response to need for more occupational based models over the paradigm shift. (Christiansen & Baum, 1991, 1997; Christiansen et al., 2005). It is envisioned that although there are some similarities with other models, yet different from other models in terms of its focus on occupational performance and participation, as well as using a top-down approach (Christiansen et al., 2005). The PEOP model is defined as a system model that looks at the function in the systems as a whole and considers the interaction over its components. Occupational performance has been received close attention in the PEOP model and contains three main components: (1) characteristics of the person (including physiological, psychological, motor, sensory/perceptual, cognitive, or spiritual), (2) characteristics of the environment (including cultural, social support, social determinants, and social capital, physical and natural environments, health education and public policy, assistive technology), and (3) characteristics of the activity, task, or role. The most recent edition is developed to provide therapists to identify the client’s enablers and barriers to occupational performance, and might be employed both individual and organizational/community based (Christiansen et al., 2005). PEOP pays close attention to the importance of occupational competency in order to attain occupational participation. attention it deserves Mental health. I
chose this topic because I am personally effected by it and so are many millions of Americans. Mental illness is also one of the leading causes of
death in our nation and one life is lost as a result of suicide, abuse or incarceration every 17mins in the United States. Mental illness has been my
area of focus throughout this program and the advocacy and participatory philosophy will be useful for the final project because it suggests that “

1).Over the last few centuries numerous historical events have taken place that resulted in a major impact on the contemporary juvenile justice network in the United States. The notion of age of responsibility and maturity level has been the central focus since the inception of recorded history. The question remains today of when and under what circumstances children are capable of forming criminal intent ("Juvenile Justice", p. 5) which has led to continuous debates still occurring today.
Up until the early 1800s juvenile offenders in the United States received the same treatment and punishment as adult offenders. It was very common for juveniles to be housed with adults as well. In 1818, a New York City commission initiated the term "juvenile delinquency", and brought to the attention of the public eye and identified pauperism, or poverty, as a root cause of misbehavior amongst juveniles. In 1825, the Society for the Prevention of Juvenile Delinquency began to campaign for the separation of juveniles from adults. Over the course of the next few years, numerous juvenile institutions were established. These institutions, known as houses of refuge, primarily focused on education and treatment, instead of solely focusing on punishment ("Juvenile Justice", pp. 6-7).
With public warning from the New York City police chief, that violent juvenile offenses were steadily increasing, in 1849 the United States transitioned from houses of refuge to concentrating on reform schools and preventive agencies ("Juvenile Justice", p. 7) Industrial Schools for dependent children were founded in 1879, as a result of the Industrial School Act, preceded by the Chicago Reform School Act, but were later held unconstitutional. As a result of unsuccessful reform schools , the child savers movement developed during the post-Civil War era, with high and genuine concern for the welfare of children, leading to the establishment of the first juvenile/family court in 1899 ("Juvenile Justice", p. 8).
By 1932 the number of independent juvenile courts across the United States, exceeded 600. All states had enacted laws developing separate juvenile courts by 1945. Because juvenile courts were not criminal courts , juvenile offenders did not possess the same constitutional rights as accused adult offenders. Historical U.S. Supreme Court cases such as

Bernal in this journal attempts to determine how surveillance affects human rights which has raised a lot of concerns of late among many workers. Looking at how surveillance has gained a lot of significance in many companies, most people have raised concerns regarding how this technology denies them their privacy. He goes on to summarize the various issues and how they can be mitigated.

1. Informal settings for meetings and to exchange information can be very useful in breaking down barriers. Often, people feel very restricted to voice their opinions when in a formal office setting or in a boardroom with their superiors. I have seen it often in the military where people are given an option to speak up, but no one truly feels comfortable because the setting does not fell appropriate. Wearing casual clothes, drinking a beer, and socializing before business begins can often make leaders feel more "human" to their employees and in return lead to more honest feedback.
2. It is possible that the consultants could have influenced the agenda in more formal ways, but several issues point to the fact that employees of the company did not feel comfortable with the leadership. Originally, the marketing and sales manager was already skeptical of the CEOs attempt to have dinner at his own house, believing that the CEO would attempt to influence the agenda of the consultants. Also, when the consultants invited middle management to the Italian restaraunt, it was noted that middle management had several good ideas but were always shut out and their opinions were not hear. By breaking down those formal barriers, the consultants were able to do their job. I believe more formal ways would have came up short of the desired effect.
3. If I was one of the managers at the Italian restaraunt I would have been relieved that someone was finally listening to us. It was noted that they had ideas and knew why the company was in trouble. Middle management is the where the rubber meets the road between the employees and supervisors. They must be involved when trying to find out better ways to run the business. CURT

During the reading of Illustration 15.3 question 1, Why are informal settings such as dinners useful? It is custom sometimes during business dealings to set the first meeting in a relaxed environment to break the ice and ensured that everyone gets a chance to relax and express their opinions. In this scenario that was the thought of some people but, not all had the same conclusion for a mutual meeting spot. The CEO had other plans, and some saw it as an opportunity to persuade the consultant company in his favor.
Question 2, Could the consultant have influenced the agenda in more informal ways? How? Although I understand having meetings off-site to avoid everyone at the company going crazy until the plan for the consultant is set. Having a more formal meeting is the best practice to set the tone on how serious the situation is. I do believe that the consultant firm could have influenced a more of a formal setting. However, it is a very fragile meeting that the consultan.

Published classroom materials form the basis of syllabuses, drive teacher professional development, and have a potentially huge influence on learners, teachers and education systems. All teachers also create their own materials, whether a few sentences on a blackboard, a highly-structured fully-realised online course, or anything in between. Despite this, the knowledge and skills needed to create effective language learning materials are rarely part of teacher training, and are mostly learnt by trial and error.
Knowledge and skills frameworks, generally called competency frameworks, for ELT teachers, trainers and managers have existed for a few years now. However, until I created one for my MA dissertation, there wasn’t one drawing together what we need to know and do to be able to effectively produce language learning materials.
This webinar will introduce you to my framework, highlighting the key competencies I identified from my research. It will also show how anybody involved in language teaching (any language, not just English!), teacher training, managing schools or developing language learning materials can benefit from using the framework.

We are bombarded daily with spiraling thoughts(both negative and positive) creating all-consuming feel , making us difficult to manage with associated suffering. Good thoughts are like our Mob Signal (Positive thought) amidst noise(negative thought) in the atmosphere. Negative thoughts like noise outweigh positive thoughts. These thoughts often create unwanted confusion, trouble, stress and frustration in our mind as well as chaos in our physical world. Negative thoughts are also known as “distorted thinking”.
