Discussion Although the general population has concerns about who has the ability to access their medical records, data has been put into place

and insurance companies have benefited from EHRs because insurance companies do not have to pay for duplicate testing as well as patients and

benefits, critics have suggested that the use of EHRs worry people will have to endure more privacy breaches and that utilizing the EHRS will be
cumbersome for physicians especially if the physicians are using the EHR in the room with a patient (Knooper, 2008). To elaborate on the negative
aspect of using the EHR, a Luddite who does not want to embrace technology feels that using an EHR will only cause him/her to miss diagnostic
clues if he/she does not take a person's medical history by hand with pen or paper which could potentially lead to a misdiagnosis (Knooper, 2008).

A hospital in Orlando, FL has decided to switch from a paper–based system to an electronic health record system. An electronic health record is a
computerized, accessible record that contains multimedia data (scanned images, digital, video, voice, and so on) about the patient; it is basically the
patient's paper chart. EHR's are real–time records which allow instant availability to those who have authorized access to the EHR (Bowie, 2011).
There are many reasons for a hospital to want to change from paper records to an EHR; the usual primary reason is to improve medical documentation.
First and foremost it is important to establish what the hospital's primary reason is to establish an EHR. Knowing this will make the transition
smoother and faster because of comprehensive EHR software and hardware installation that is comprised of a number of different components
(Medflow).

The data collected through computers during single research are used to synthesize evidence to guide the best care. After the synthesis of evidence, the
evidence has to be translated into practice. The incorporation of EBP into the information system in healthcare has been possible to aid the clinician
with their decision making. There are many clinical practice guidelines available electronically that can be utilized at the point of care (Upshur, 2014).
Personally, I have used guidelines on hypertension, hyperlipidemia, and diabetes many times to aid with my decision to treat patient with during my
clinical. In my first clinical at the UW Internal Medicine, the modified guidelines were available to clinicians through intranet. In this way, the EBP are
easily accessible during point of care and enhance its utilization during practice. Once again, the data collected into the EHR are used to determine its
impact on patient outcomes, cost, and effectiveness of the treatment. This is a way to evaluate whether the implementation of clinical practice

the quality and efficiency of that care. Most articles written about PHRs have been published since 2000.
