Some of the scene w ill
be taking place during

mainly take place
around the Clevedon
pier. Reason being is
that the narrative fits
w ith the setting. A

may become a risk. To limit this, w e w ill:
 Avoid, w here possible, filming in
particularly slippery areas
 Encourage sensible, practical

dow n where necessary
 Wipe or mop up any liquid spillages
as soon as possible and notify
other members of cast and crew.

the sea. Also to be extra carefulif
filming next to the sea and bridges
so that the equipment’s don’t get

concerns we need to know . Seat
belts w orn by allif in car. If travelling
by bus there is a fire exist and use it
if need to be.
 All to remain in vehicle if safe to do
so, emergency contact lists
available. If there is an accident
w hich willdelay the schedule make
sure to contact the director to inform
her.

risk of slipping and

also a major risk for
the equipments
(Electrical
equipments).
All  Mobs to dry the floor. Make sure to
have first aid equipments near the
location and a fully charged phone.
 Dry the floor to avoid slipping,
tighten the taps, and use a
moderate w ater to avoid w asting the
w ater. Make sure everyone follows
the instruction given to them. To
take extra careful.
 In addition to speak to the actor at
all time to make sure if she is fine
and comfortable.

Risk Assessment Form – Part B Activity1 – Each individual activity you are proposing e.g. location lighting: As per Location Breakdown Sheet Location – As Per Part A and generalmovement around the complex Sand Bay Weston super Mare BS22 9UZ Dates / times: Shooting days commences on Monday 21st March + Tuesday 24th March Hazards2 List what could cause harm f rom this activ ity e.g. working at height, trip hazard, fire, etc. Who exposed List who might be harmed f rom this activ ity e.g. staff, contractors, contributors, public, etc. Risk3 For each hazard, decide lev el of risk as if y ou were to do the activ ity without y our controls Control measures For each hazard, list the measures y ou will be taking to minimise the risk identif ied e.g. appointing competent persons, training received, planning and rehearsals, use of personal protective equipment, provision of first aid, etc. Risk3 For each hazard, now decide lev el of risk once all y our controls are in place Cold Environment Cast & Crew LOW  Encourage all to bring w arm clothing and lots of layers  Put reminder in Location Notes on all Call Sheets  Ensure regular breaks are taken in a w armenvironment  Ensure hot drinks are available on location at all times VERY LOW Uneven grounds All MEDIUM  Warn everyone to be extra careful  Ensure thorough checks have been made to the area before using it.  LOW Night shooting- Some of the scene w ill be taking place during the evening. This w ill mainly take place around the Clevedon pier. Reason being is that the narrative fits w ith the setting. A Possible flashbackw ill be used at this location. Crew & Cast MEDIUM  Ensure that w e have sufficient amount of lighting, possibly a torch w ith us.  Wear appropriate clothes like a w armcoat. VERY LOW Object Falling, Moving or Flying All LOW  Make sure all objects that are at height are secured to the rigging or w eighted down at the base  Make sure all crew and actors know the set and w hat is at height.  Ensure that the vicinity surrounding the moving/flying object is clear. VERY LOW Slippery Surface Cast & Crew MEDIUM Due to adverse w eather, Slippery Surface may become a risk. To limit this, w e w ill:  Avoid, w here possible, filming in particularly slippery areas  Encourage sensible, practical footw ear  Ensure sand/grit/sawdust is put dow n where necessary  Wipe or mop up any liquid spillages as soon as possible and notify other members of cast and crew. VERY LOW Weather is unpredictable especially the British w eather and it may be rain or snow which can have an effect on All MEDIUM  To make sure that all of the equipment are safe and cover them. In addition all of the cast, crew must have raincoats/ umbrella w ith them at all times.  Ensure that w e checkthe w eather in VERY LOW

LOW Fire Crew & Cast LOW  When setting up shots, the relevant departments w illensure electrical equipment is given adequate space to keep cool and not overheat  Loose cable fromcables drums to eliminate riskof fire and spread cabling over a larger but safer area  Strictly no smoking or any naked flames on set at any time  In the event of a fire, area to be left immediately and all cast and crew to assemble in pre-arranged safe location.  Cast and crew to familiarise themselves w ith locations of fire extinguishers/blankets and locations of fire exits  Ensure that fire escape routes are not blocked VERY LOW Working in Confined Space Cast & Crew LOW  Cast and Crew to blockthrough moves  Constant communication to be kept up betw een everyone, so that everyone know swhat everyoneelse is doing and planning  Only necessary personneland equipment to be on location VERY LOW Noise Crew VERY LOW  Ensure noise levels monitored  If needed, w ear extraneousnoise cancelling headphones VERY LOW Cast & Crew LOW  VERY LOW Falling into the sea. The majority of the scenes willtake place either at Clevedon pier or sand bay. For this reason w e need to be carefulfor our safety and the equipment’s. Crew & Cast MEDIUM  Bridge to w alkon, pavement also safe footwearand to make sure that all equipment’s is kept aw ay from the sea. Also to be extra carefulif filming next to the sea and bridges so that the equipment’s don’t get damaged  Warn all the cast and crew to be extra carefuland if something arises to go to either (FATUMA OR LOW

Very Low Member of the crew/cast falls ill. Crew  Everyone must complete a health questionnaire; this w illhelps us indicate if w e need to take action. All of the crew / cast must informus beforehand if there is any health concerns we need to know . Seat belts w orn by allif in car. If travelling by bus there is a fire exist and use it if need to be.  All to remain in vehicle if safe to do so, emergency contact lists available. If there is an accident w hich willdelay the schedule make sure to contact the director to inform her. Very Low Bathtub scene - A risk of slipping and possibly drow ning, also a major risk for the equipments (Electrical equipments). All  Mobs to dry the floor. Make sure to have first aid equipments near the location and a fully charged phone.  Dry the floor to avoid slipping, tighten the taps, and use a moderate w ater to avoid w asting the w ater. Make sure everyone follows the instruction given to them. To take extra careful.  In addition to speak to the actor at all time to make sure if she is fine and comfortable. Medium
