Outline of risk assessment
Summary of what is proposed in
terms of sequences and set ups.
Main set up w illbe, tripods and a clear space around the recording place, I w illalso setup the camera in
houses as w elland I w illhave to use the tripods 24/7.
There w illbe hand held motions in the clips but it w illvery rarely be actually done at the moment

Risk matrix – use this to determine risk for
each hazard i.e. ‘how bad and how likely’ Likelihood of Harm
Severity of Harm
Remote
e.g. <1 in 1000 chance
Very unlikely
e.g. 1 in 200 chance
Unlikely
e.g. 1 in 50 chance
Possible
e.g. 1 in 10 chance
Likely
e.g. >1 in 3 chance
Negligible e.g. small bruise Trivial Trivial Trivial Low Low
Slight e.g. small cut, deep bruise Trivial Trivial Low Low Medium
Moderate e.g. deep cut, torn muscle Trivial Low Medium Medium High
Severe e.g. fracture, loss of consciousness Low Medium High High Extremely high
Very Severe e.g. death, permanent disability Low Medium High Extremely high Extremely high

Activity1
– Each individual activity you are proposing: Location – w here this activity willtake place: Dates / times:
Hazards2
List what could cause harm f rom this activ ity
e.g. falls from height, trip hazard, fire, etc.
Who exposed
List who might be harmed
f rom this activ ity
e.g. staff, contractors,
contributors, public, etc.
Risk3
For each hazard, decide
lev el of risk as if y ou were to
do the activ ity without y our
controls
Control measures
For each hazard, list the measures y ou will be taking to minimise the risk identif ied
e.g. appointing competent persons, training received, planning and rehearsals, use of personal protective
equipment, provision of first aid, etc.
Risk3
For each hazard,
decide lev el of risk
once all y our controls
are in place
Manual Handing
Krasimir Marinov
Daniel Parushev
Gonçalo penica
Slight
Most of the things used in my production aren’t heavy, they are easy to move but things like
tables and chairs w illneed more people than one to move. To prevent any problems
everything is on the script. Negligible
Driving long distances
Krasimir Marinov
Daniel Parushev
Gonçalo penica
Slight
Everything for the place and how it looks like, is on the scrip it w illbe easy to find and access.
There are not many problems on the road. The only thing w illbe long distance of traveling
that’s it. Negligible
Trip hazards
Krasimir Marinov
Daniel Parushev
Gonçalo penica
Slight
There w illbe any cables on the floor or other things that w e use and can create a trip hazard.
They w illbe taped to the floor so its un seen in scenes and also its smoother to go over.
Negligible
Cold / Hot environment
Krasimir Marinov
Daniel Parushev
Gonçalo penica
Slight
The rooms w illget hot, probably from the lights or any other systems. If it’s too hot, Me as a
director w illjust open the room and let the lights/ cast rest or continue shooting another
scene. It w illbe the same if the room is cold. Negligible
Object falling, moving or flying
Krasimir Marinov
Daniel Parushev
Gonçalo penica Slight
There is only one object that can fall, it’s the tripod if it’s not an equal and safe ground, it w ill
be mostly hand held by me. If not, I w illmake sure to use my gorilla tripod w hich can place
the camera anyw here. If not, I w illtape it to the floor so there is no w ay a shake can affect it. Negligible
Fire
Krasimir Marinov
Daniel Parushev
Gonçalo penica
Slight
In everyplace we willbe, there is a chance thing to set on fire because of the systems that we
use, such as computers and lights. If fire comes to place, there is a fire exit everywhere no
matter w hen we are. Also, there is a fire extinguisher in everyplace. Negligible
Physical damage
Krasimir Marinov
Daniel Parushev
Gonçalo penica Slight
There w illbe only one scene that w illbe physicalenough to create damage. And that’s
because of the scene, otherwise, nothing that w illbe used in the movie w illw eight over 20kg.
Negligible
