The Affordable Care Act was established to put America on the path to good health. It was founded on the premises of providing high quality and affordable care for each and every one. When it comes to the community and public health, the important elements in the act supports the transformation of health care from a “sick” model to a “healthy” model, provision of care by addressing the nursing shortage, improvement in quality and patient safety, provide community-based interdisciplinary/ inter-professional teams to support primary care practices, support nurse-managed health centers (centers operated by advanced practice nurses that provide comprehensive primary care and wellness services to underserved or vulnerable populations), and improve the choice of affordable health to individuals in order to make sure all American have health insurance coverage.(American Nurses Association, 2014) Today, nurses play an integral and leadership role implementing the laws of Affordable Care Act. They provide improvement in quality of care and safety through nursing research which help shape the scientific base for clinical practice, processes, prevention, and improved patient outcomes. The nurse also serves as a valuable asset when it comes to educating the community on illness prevention, wellness promotion, available resources and cost containment. It is the duty of all nurses to equalize the health disparities among lower socioeconomic and minorities groups in the health care setting. Defining a Culturally Competent Organization Culturally competent health care, broadly defined as services that are respectful of and responsive to the cultural and linguistic needs of patients, is increasingly viewed as essential in reducing racial and ethnic disparities, improving health care quality, and controlling costs. The U.S. government considers cultural competence as a method of increasing access to quality care for all patients. The aim should be to develop systems more responsive to diverse populations. Managed care organizations view cultural competence as driving both quality and business. By embedding cultural competence strategies into quality improvement initiatives to make care more efficient and effective, clinical outcomes are improved while costs are controlled. Those in academic settings agree that cultural competency education is crucial for preparing future health care workers, although appropriate education on the topic is provided in only half of the medical schools in the United States (Betancourt, Green, Carrillo, & Park, 2005). According to the Office of Minority Health, cultural competence refers to the ability of health care providers and organizations to understand and respond effectively to the cultural and linguistic needs of patients (Office of Minority Health, 2001). Cultural competence encompasses a wide range of activities and considerations. It includes providing respectful care that is consistent with cultural health beliefs of the clients and family members. Competent interpreter services and programs to promote staff diversity are other ways in which health care organizations can increase cultural competence (Clancy & Stryer, 2001). Because communication is a cornerstone of patient safety and quality care, every patient has the right to receive information in a manner he or she understands. Effective communication allows patients to participate more fully in their care. Communicating effectively with patients is also critical to the informed consent process and helps practitioners and hospitals give the best possible care. For communication to be effective, the information provided must be complete, accurate, timely, unambiguous, and understood by the patient. Many patients of varying circumstances require alternative communication methods: patients who speak and/or read languages other than English; patients who have limited literacy in any language; patients who have visual or hearing impairments; patients on ventilators; patients with cognitive impairments; and children. The hospital has many options available to assist in communication with these individuals, such as interpreters, translated written materials, pen and paper, communication boards, and speech therapy. It is up to the hospital to determine which method is the best for each patient. Various laws, regulations, and guidelines are relevant to the use of interpreters. These include Title VI of the Civil Rights Act, 1964; Executive Order .

The times call for the high spirit of the courageous pioneers among physicians, scientists, and nurses.” Lillian Wald This is a time of rapid transformation in health care, one in which community health nursing has a critical role in advancing individual and public health. As the United States integrates the mandates of the Affordable Care Act (ACA), community health organizations have a pivotal role in affecting the health status of the nation, particularly for vulnerable populations. The Institute for Healthcare Improvement, through the construct of the Triple Aim, calls on all members of the health care team to improve the health of the population, improve the consumer experience and reduce the cost of care. The Institute of Medicine's (IOM) report on The Future of Nursing has charged nurses to become equal partners in the development of health policy and practice (IOM, 2011). The IOM report Public Health and Primary Care has challenged practitioners to coordinate efforts for the betterment of patients (IOM, 2012a). Community-based organizations are strategically positioned to provide the leadership as well as the integration and coordination of services necessary to carry out these aims. Further, the community-based sector of the nursing profession is poised to influence the transformation of health care delivery by drawing on principles that are core to the discipline. By partnering with communities, creating innovative approaches to care as the system evolves, and engaging the communities they serve, community health nurses can deliver on the promise of quality health care for all. This chapter discusses the approaches of the Visiting Nurse Service of New York (VNSNY) to mobilize the strengths of the community to improve public health, establish cross-continuum interprofessional teams to affect the continuum of the patient care journey, and promote public policy to advance funding methodologies that more adequately consider risk factors of vulnerable populations. Community as Partner and the Community Anchor Community Anchor is a concept that is being developed by the VNSNY as a way to build healthier communities. DQ 3-1 response 1 Working in Oncology care, the Oncology Nursing Society or ONS is a national professional specialty nursing association that I have been an ongoing member of since 2010. As an organization, their mission is “to advance excellence in oncology nursing and quality cancer care” with a vision “to lead the transformation of cancer care” (Oncology Nursing Society, 2017). Innovation, excellence and advocacy is what guides ONS to improve the quality of life and outcomes for patients with cancer and their families as well as improving the nursing profession (Oncology Nursing Society, 2017). They not only engage in providing education and training for nurses but also engage in international outreach and partnering with other organizations that share common goals to enhance quality of care for people with cancer. Cancer care is complex and continuously changing. As a member of the health care team, I feel the values of excellence, innovation and advocacy are reflected in their mission and vision statements. I also feel their mission and vision statements also mirrors the organization I work for. As a health care provider, Aurora Health Care, helps people to live well, providing the best care possible during and after a patient’s hospitalization. They support and encourage their medical/oncology nurses to become oncology and chemotherapy certified. They provide annual mandatory cancer education and offer monthly oncology seminars for all health care providers. The also encourage and support local, state and national seminars as well for their nurses. As a health care provider, they are devoted to providing the most current up to date treatment and promote safety through the encouragement and participation of evidence based initiatives and quality improvement projects. After reviewing the textbook, and the essential public health services and selected nursing activities, I was amazed to learn about the roles and responsibilities of a public health nurse. There are many different benefits to having a public health nurse as part of staffing such as being able to monitor and identify different health issues and illnesses in the community, tracking and studying the public population and health conditions, providing insight from a community standpoint regarding acute health issues, and being able to provide numerous resources to staff and patients regarding health prevention and disease management topics. Public health nurses are professionals in research and they understand the importance of evidence based practice. They utilize evidence-based research to help protect the health in the community and to also strengthen the health of the public. According to Weum, Bragstad, and Glavin (2018),“public health nurses’ rank their sources of knowledge, it showed that the four most frequently used sources were national plans and guidelines, knowledge acquired while training as a public health nurse, personal experience, and guidelines and procedures issued by their local authorities” (p. 8). Public health nurses focus more on the prevention of diseases and incidents rather than on the care that is required after diseases and incidents occur (Anderson and McFarlane, 2015). According to Teodorowski, Cable, Kilburn, and Kennedy (2019), “community nurses are expected to deliver evidence-based practice, which is challenging given the diversity and breadth of the evidence base from which they can draw” (p. 370). Due to today’s increase in patient access to online health information, it is important for public health nurses to retrieve their information from evidenced-based sources such as the Centers of Disease Control and Prevention and World Health Organization websites.
