Undertaking for ba test

1. UNDERTAKING
This is to confirm that I, ______________________________________(Name /employee
No./Designation) working at __________________________AFS(Name of AFS) am not
under the influence of alcohol and have not consumed alcohol/psychoactive substance
in last 12 hours from the time of reporting for duty.
I am also aware that in case of violation of my above undertaking, my license/approval will
be suspended for a period of three years
This undertaking is in line with directives issued by O/o DG ,DGCA ,New Delhi vide their
Order DGCA-15031//2020-DAS dated 29/03/2020

a) This undertaking is applicable to all personnel reporting to duty and who are
engaged in Refueling/Defueling operations and associated activities on the apron.
b) The signed-off undertakings should be filed in the BA Test folder in chronological
order for scrutiny along with the DGCA’s order issued in this context
