Alberto embarked on his journey into the medical field 15
years ago and continues to serve. This experience has made
him understand the wide intricacies in the healthcare sector,
which differs for every region. He says, “My career in
companies such as Allergan and Johnson & Johnson in
different countries has allowed me to detect the needs of
doctors and patients in the digital world, and the need for
digitalization of the sector in many different countries.”
This experience has given him the opportunity to
understand the insights that are the basis of the Top Doctors
Company. He comprehended that, on the one hand, the
patient’s difficulty in finding the right doctor and, on the
other hand, the need for doctors and clinics to be able to
respond to the digital patient and how they can optimize
consultations and clinics to be the competitive post-COVID
world.
“In health”, he says, “Quality is always very important,
from the point of view of treatment and medical specialists,
but also in terms of costs. The concept of identifying the top
is the basis of our business.”
Ideating this into reality and reminiscing about the
inception of Top Doctors, he recalls, “Like all good
businesses, I was lucky to have support and collaboration,
coming from Castle and Connolly of America’s Top
Doctors, who trusted in the business model outside of the
US, and we have been able to create a business model with

The platform is free for patients, generating over 275m web
visits per year and contributes to solving poor access to
medical specialists. It is the only platform that allows
patients to make their own choice of doctor, a choice based
on quality information, showing the doctors most
recommended by other specialists and the real experiences
of other patients.
Success is built by the trust of patients on the platform and
this is driven by the leader’s strong commitment to the
long-term goals of the company. The vision of Top Doctors,
as Alberto elaborates, “is to be a unique platform providing
the most relevant and up-to-date information available to
patients worldwide, keeping them informed on the latest
medical developments, and crucially guiding them to select
the right medical specialist for their needs. From an
internal survey, we know that patients usually have to go to
on average 2.7 different doctors before finding the right
specialist for them.”
The foremost features of Top Doctors are that through Top
Doctors, 90% of users find the right doctor in the first
consultation. They also provide easy access to booking
face-to-face or online consultations with these specialists.
Alberto says, “We work every day to improve
communication between patients and leading health
professionals around the world.”

Top Doctors offers a number of solutions and services to
their member doctors and visitors to the website. These
include online reputation management, online booking
assistance, teleconsultation solutions, and other
technological solutions that help to improve the doctor’s

is incessantly incorporating novel solutions. Talking about
this, the founder says, “We have incorporated the latest
technological trends, including AI, in our symptom checker

their day-to-day work flow.”

Strategic planning specific to the industry helps a firm to
produce better results as compared to its competitors. This
company’s prime focus is only on secondary care and top
specialists, which is reflected in its business model. It is
based on hyper-specializing sales processes and structures
in hubs. Their investment in technological innovation is in

He says, “Once you have communicated and sold the

rigorous with employees, keeping promises, helping them to
grow, coaching them, and taking them out of their comfort

important sentences of wisdom as counsel to them, which

for leaders to not just be in the present but to be thoughtful
of their future endeavors to be competent and a trusted
brand in the market. Talking about the company’s goals in
the upcoming years, Alberto says, “The objective of Top

In an interview with Insights Care, Lindsey walks us
through her journey and her role in enabling customized
care—in a way that eliminates obstacles, reduces patient
anxiety, and meets patients' care expectations effortlessly.
Following are the excerpts from the Interview:
Kindly tell us the source of inspiration. What led you to
venture into healthcare?
We tend to interact with the healthcare system on occasions
when we feel vulnerable or anxious about the situation we
are faced with, whether that's personally or with a loved
one. During some of my interactions with healthcare
organizations, I often observed that there were areas that
could be improved, and thought I could best use my skills
to be part of that change.
I have always been drawn to healthcare organizations that
could make that difference and provide personalized care to
each patient. That’s one of the reasons I was attracted to
Cleveland Clinic London, which is always pushing the
boundaries of patient-centered care through research,
education and innovation.
Brief us about yourself and shed some light on your
journey towards becoming a part of Cleveland Clinic
London.
I am a passionate healthcare leader, having started my
career 19 years ago at Hammersmith Hospital, London.
Over the years, I have held many positions in both the NHS
and private sector, and have been lucky along the way to
have had mentors and managers who have inspired and
supported me. Most recently, I held the position of CEO of
King Edward VII's Hospital, in the Harley Street Medical
Area, and was proud to be the first female to hold the CEO
post.

What roles and activities do you cover at Cleveland
Clinic London daily?
As the Chief of Operations at Cleveland Clinic London, I
have a diverse portfolio, which includes overseeing the
utilization, productivity and efficiency of all patient
activities across the organization, as well as leading
operations. This includes patient support services, supply
chain management, protective services, and estates and
facilities, which are all critical functions in delivering care
to our patients.
I take great pride in ensuring our team has the ability to
focus on reaching as many patients as possible while
delivering outstanding, safe, quality care that's personalized
to every patient.
What in your opinion does the healthcare system in your
area need to focus on, to improve care for patients?
Cleveland Clinic is world renowned for its approach to
transparency and publishing patient outcomes, and we do
this to improve the quality of care we provide. In our
experience, it drives the best clinical outcomes and the
highest levels of the patient experience.
Through investment in technology in Cleveland Clinic
London, we have introduced a fully integrated Electronic
Patient Record. We strive to measure and enhance the
patient experience and provide more interactive, joined-up
care. Ultimately we want to make it more convenient for
patients to be able to inform and control their own care
decisions.
Technology is important, but we must never lose sight of
the ability to enhance the delivery of empathy in our
interactions. The core of healthcare is the expression of
empathy from one person to another, and this ‘patient first’
ethos is central to our model of care at Cleveland Clinic.
What role does compassion play when interacting with
and encouraging patients to live healthier lives?
Compassion is essential. We want our patients to see how
they are being prioritized with safe, high-quality care that is
tailored to their satisfaction and experience.
Cleveland Clinic London offers a ‘team of teams’ approach,
with a multi-disciplinary team working collaboratively to
ensure the best treatment and outcome for the patient. This
enables a seamless treatment journey that always puts the
patient’s well-being at the heart of decision-making.
Our patients find comprehensive, compassionate care with
consultants who have the specialized skills and knowledge
to provide the best possible care, no matter what the
patient’s needs. That care is then tailored to the patient,
providing them with a personalized, empathetic experience
that puts their needs and values first. In turn, that reduces
the anxiety and obstacles so often associated with the
patient experience, which will improve the healing process
for that patient.
What endeavors are you currently pursuing to improve
treatment and care measures at Cleveland Clinic
London?
Ensuring quality and safety will always be our key focus,
and this is embedded throughout the organization as we
deliver high-acuity care to our patients every day. Our
multidisciplinary approach brings different clinical
perspectives to care, so patients benefit from the expertise
of many specialists. Our best practices include tools,

What are some of the challenges you face when
conducting operations and how do you turn them into
opportunities for growth?
The greatest opportunities do not come without hurdles.
While we have an opportunity to scale high-quality
healthcare across larger populations, healthcare workers
across the world are experiencing increased burnout. Right
now, the biggest challenge in healthcare is the staffing and
workforce shortage.
Another challenge is making quality healthcare sustainable
with the ability to reach more people through major digital
and process transformations. In London, our team is also
working on the need to ensure that organizational activities
are directed towards our fundamental goal of reducing the
health system’s environmental footprint.
What advice would you like to give to budding
entrepreneurs and enthusiasts who desire to venture
into the healthcare sector?
The ultimate reason we work in healthcare is to care for
people, and for this reason, our outcomes should reflect
their goals and experiences so we can better serve them.
Healthcare leaders must concentrate equally on their staff as
they do their patients. Your team will do extraordinary
things for your patients if you empower and trust them by
delivering meaning and purpose to their work, opportunities
to learn and grow, and recognition.
How do you envision scaling your services and
operations in 2023 and beyond?
As part of one of the world’s most respected academic
medical centers, we offer access to a global network of
physicians and specialists, as well as a world-class patient
experience built around the best practices and core values
of Cleveland Clinic.
Through 2023, growth is a key strategic priority for
Cleveland Clinic London, so there is a focus on scaling our
services. We are also opening an additional outpatient
center in the City of London this year to complement our
presence in the Harley Street Medical Area and our main,
184-bed, hospital in Belgravia.
London is a thriving center for healthcare, medical tourism,
technological innovation and clinical R&D, so I'm excited
about what the future holds.

centered system organized around what patients need. The

manage inventories and distribute medical supplies, but it is
also an area where providers may create significant cost-
cutting possibilities throughout their organization. Having
an effective supply chain is critical for organizations trying
to save costs and improve care. On average, medical
supplies now account for around a quarter of hospital
spending. Supply chain expenses are projected to rise as a
result of general inflation and increased prices for medical
equipment and goods. Three of the most typical healthcare
supply chain management concerns are fragmentation,
complexity and disruption. As such, providers must

mprovements that occurred recently in our world have
Ichanged or transformed us in every aspect of our lives.
They showed that the internet is such a precious
invention for us from our business life to our private life.
Even though this pandemic made us realize valuable things
in our lives, it also showed us our deficiencies. We started
to fulfill our needs through the internet during the
pandemic.
Its conditions affected our mental needs just like they
affected our physical health. Although we were able to
fulfill most of our needs through the internet, we could not
fulfill the ones of our children. The most significant one was
the situation I encountered when my 2-year-old daughter
refused to be breastfed. There was a lockdown, and we did
not know what to do. Hospitals were full of COVID cases;
this might have caused serious health problems for me and
my family.
After some research I had done, I talked to a breastfeeding
counselor on my phone. I did not know what to do because
my daughter was hungry. I transferred the money which the
counselor demanded to them. We met at our appointment
time via an app similar to Zoom. This meeting was really
beneficial to my daughter and her mother, but there was an
unsafe space. People did not know how to contact the
counselor or not trust them through only social media. God
only knows how many children were brought up with such
conflicts because of this trust issue.
This is how the idea of Kidolog crossed my mind. Even
though the reason why the process started was

in Ankara. After graduation, he started to work
with his family in their company and also began

to foreigners, he continued his serial
entrepreneurship. He established the Botanik
Café&Bistro brand and Lokkalata brand.
With the Lokkalata brand, by giving franchises
across Turkey, he was considered worthy of the
Franchise prize by UMAD. He still owns
businesses that are successful in the catering
sector in Ankara, Eskişehir, and Kırıkkale. Right
now, he is getting a master's degree at the
Information Systems Institution of Hacettepe
University. He is the co-founder and the CEO of
Kidolog, which parent counseling platform
launched in February 2021 for its user's service.
He is married with a daughter. He has a good
command of English.

We are improving our platform day by day. We launched
our app within one year after our establishment and started
to present some of our features only through the app. We
are providing many services for mothers, and we are
planning to make their lives easier with many features to be
presented sooner. They can track their children through the
app. Not only the duration and the amount of the baby's
feeding but is also important to be organized. Thanks to our
app, she can record how many times and also how many
mgs she breastfed him from which breast. There are also
many available services such as tracking the reading
duration and screen time.
Our platform drew many mothers’ attention since it was

established. We are close to our 2 year anniversary, and we
have provided 50.000 families with our services. The
attention of parents makes us happy, and we get great
feedback from them. Our many users log into our system
with the other users’ references and get help. As our origin
is in Turkey, we first started to serve in Turkey, but it was
not enough. We are serving to parents who speak Turkish in
many countries from Germany to China. Right now, we are
still conducting our infrastructure works; after they are
done, we will start to serve in German and in English.
Shortly, we are going forward to be the best friend of
parents from all around the world.
breastfeeding, we tried to keep Kidolog as widespread as
possible we could; for bringing up a child can also come
with conflict that needs to be resolved eventually. So, we
were there for mothers with experts who are suitable for all
ages. With the midwife and doula support starting at the
birth process, they can initiate the labor easily with
pregnancy yoga and pilates.
Generally, we give so much support to the ones who are
going to be a mother for the very first time. They don't
know what to do right after the birth, and with the
professional support we gave to them, they can solve the
problems which may be encountered after the birth with
experts. Right after that, our breastfeeding experts will be
there for the family.
From Breastfeeding to Whole Process
Problems of mothers, who are bringing up their children
from the breastfeeding process to the age of 18, are being
resolved by different experts. We’ve also realized that
parents who are taking our support for their children also
need to be helped as well. The amount of demand rose for
this situation. We provided our support to parents as a
whole by including specialist clinic psychologists in our
system. Briefly, we are with them from their potty-training
problems in their infancy to the peer pressure they are
facing in their schools when they were children.

Investments and Rewards
We got seed investment from Mr. İbrahim Özer who owns
one of the most important investment web in Turkey right
after our establishment. It was quite valuable for a start-up
that was established with zero funds. 3 months after the first
investment, Mr. Özer and a couple of angel investors had a
meeting with us to invest in the second one. After our
meetings resulted as a success, we left the seed investment
stage very efficiently. We improved our Turkey operations
very significantly in this frame. We even achieved the
success of being in the top 20 of the best start-ups in
BigBang which is a competition that took place at Istanbul
Technical University, which is among the prestigious
universities in Turkey, only a year after our establishment.
In January 2023, we received an investment of 7.5 million
dollars valuation. With this investment, Kidolog, which was
established 2 years ago, has increased its value 50 times.
We won two financial rewards just as important as the
investments on the stage. We were in first place in the
development programs of very valuable banks. We were
selected among the 13 best enterprises by one of the
significant technology companies, Turk Telekom. We won
many ranks and rewards in 2 years. These are our pride and
joy and signs of how we can impact the future. We found
many opportunities to improve ourselves while improving
Kidolog. We took serious steps in the way of being
entrepreneurs while raising the awareness of the parents.
We celebrated this by being on the list of Fortune 40 under
40 very recently.
New projects
I have already mentioned the demand of parents. One of our
missions is to have conscious parents and happy
generations. We are conducting our work to make the world
become a better place to live. For this mission, we are
maintaining our support to the world with our projects that
are being conducted and will be conducted later on. We
sponsored a young women's football team for the purpose
of supporting and raising awareness for girls. We are
reimbursing their tuition.
We are making plans to create a more sustainable world. We
are carrying on our work for the purpose of green parenting
and breast milk to become widespread. We started an
operation in Europe, the United States of America and the
United Arab Emirates in English, German and Arabic
languages. We are counting the days to show ourselves in
the Global Market in many native languages.

to Resolve Chronic Constipation
f you spend hours on the toilet or go to the bathroom
Iless than three times a week—then you might be
suffering from constipation. Constipation is a
widespread problem that is affecting more than 250 million
people across Europe and the US, among which 70% do not
seek medical help. Rather, they resort to the regular use of
laxatives which end up causing harmful side effects such as
intestinal paralysis, lazy bowels, or cathartic colon.
Observing the current practises of bowel management left
Dr. Markus Wilhelms, CEO, in awe. These laxative
solutions were not only harmful for chronic patients but
also weren't effective in meeting their care needs. Detecting
such clinical flaws, he and his team came up with
MOWOOT—a medical device inspired in abdominal
massage—a research-proven, non-laxative solution against
chronic constipation. Unlike having to visit a massage
professional every month, this device allows the patient to
seek daily relief, in sessions of 10 to 20 minutes. During the
treatment, the device stimulates the colon and facilitates
intestinal transit, thus proving to be a reliable solution
against constipation.
Such an innovation reflects Dr. Wilhelms’ curiosity and
leadership in addressing the global health concern. While
the world has been exploring shortcuts and unnatural ways
to seek relief from a poor bowel movement, Dr. Wilhelms
has taken measures to deliver a scientific-proven solution

Dr. Wilhelms’ Source of Inspiration
Dr. Wilhelms is a molecular biologist by education with
20+ years of experience in the field of gut health. He has
been working with innovative methodologies in healthcare
for the past 10 years. He is also a business coach for the
European Commission where he enjoys working with early-
stage companies as a mentor and lecturer in selected
accelerator programs for medical device development in his
spare time.
The founding team of MOWOOT—Dr.Wilhelms, Dr.
Immaculada Herrero, Marc Benet, and Dr. Angel Calzada
met in 2014. “After thoroughly analysing the unmet need
that we had detected in the management of bowel
dysfunctions of chronic patients with hundreds of
stakeholder interviews, we knew that this is something that
needs to be solved and that we were the right team to do it,”
reflects Dr.Wilhelms.
Thus, in 2014, MOWOOT's founding team performed a
two-month clinical immersion in a neuro-rehabilitation
hospital within the framework of the ‘Design Health
Barcelona’ program, a European version of the ‘Biodesign’
program for needs-driven innovation in healthcare, created
by the Stanford University. The goal was to observe and
detect unmet clinical needs.

But what was most striking for Dr.Wilhelms himself and
the team were the shortcomings of the current standard of
care in bowel management, especially for neurological
patients (spinal cord injury, multiple sclerosis, and related).
Existing solutions had low efficacy, several side effects and
other major inconveniences for chronic patients and did not
seem aligned with their needs.
“We saw that even though the patients were suffering, many
were reluctant to discuss this topic openly, due to its private
nature and the taboo that is still associated with defecation,”
says Dr.Wilhelms. “Maybe that was part of the reason for
this unmet need. Somebody had to do something about it,
and we were ready to take the challenge!”
Alignment of Internal Operations with External Needs
of the Market
Being the CEO of the company, Dr.Wilhelms is consistently
involved in activities like fundraising and usual paperwork.
But his main contribution within the company goes with
developing business strategy, aligning products and
markets, and ensuring that each team member understands
what is expected from him or her. The team member has a
well-defined role that impacts the success of what the
company is doing. Thus, Dr.Wilhelms believes, “They must
know the depth of why a particular role is important to the
company, followed by the goal of improving the quality of
life for patients suffering from bowel dysfunction. This
needs to be aligned internally and coordinated with the
market externally to make sure our innovation reaches the
patients that need it.”
MOWOOT—an Alternative to Enemas and Laxatives
MOWOOT is the first ‘Intermittent Colonic Exoperistalsis’
(ICE) device on the market, a non-drug and non-invasive
solution without the drawbacks and side effects of the
current standard of care. This device applies external
pressure alongside the large intestine to substitute reduced
or missing colonic contractions in severely constipated
patients. ICE works in line with the physiological functions
of the body and is not trying to work around them, as
laxatives, enemas and other approaches do. As a result, ICE
does not have the side effects of the other approaches and
many patients describe the treatment even as pleasant. Such
a device can simply be worn over clothes, sitting or lying
down comfortably, and positive results have been shown
with as little as ten minutes of daily use.
In patients with permanent severe neurological pathologies
that lead to neurogenic bowel dysfunction, ICE treatment
manages the problem, but lifelong adherence is necessary.
“We have indications that patients with less severe chronic
idiopathic constipation can ‘retrain’ their colonic motility
with our device, analogue to how an exoskeleton can retrain
gait in movement-impaired patients. But this needs to be
evaluated in further studies,” states Dr. Wilhelms.
R&D Operations
To ensure that the company delivers the best possible value
to different stakeholders and patient types, Dr. Wilhelms
believes that it is essential to keep one’s eye open to the
product and market needs that are continuously evolving.
“While we initially focused on entering the market with a
simple product that addresses the core of the unmet need,
we are now ‘fine-tuning’ our value proposition,” he says.
For example, the current R&D projects include improving
the eco-sustainability of the company’s product with the use
of novel materials, which is not only good for the
environment but will also be crucial in the future to secure
public healthcare reimbursement in line with the EU
sustainability goals.
In addition, Dr. Markus and his team are also working on
the development of an AI platform that is especially
interesting in the context of new devices that the team is
developing for complex adjacent clinical indications like

“Regarding bowel management, we believe that solutions should focus more on
working in line with the patient's physiology, instead of bypassing them. Apart from
being a natural and safer approach, this prepares the body for a smooth transition
from treatment to functioning autonomously and lowers the risk of dependency in
those patients that might be able to regain a healthy gut function.
Regarding the healthcare system in general, it is obvious that bringing a MedTech
innovation to the market is a complex and resource-intensive endeavour. You not only
need to deliver the expected clinical outcomes for the patients, but also make sure
your technology fits into the healthcare ecosystem of prescribers, payors, regulators,
etc. Especially reimbursement requirements are still largely different in each EU
country and sometimes hard to predict due to a certain lack of consistency.
While regulations and certain obstacles are necessary to make sure only safe and
effective innovations enter the healthcare system, a unified European system would
enable healthcare innovations to be adopted into clinical practices more efficiently,
which in turn would guarantee equal patient access throughout Europe. The relevant
stakeholders should collaborate to ensure that funding and reimbursement processes
are as efficient and timely as possible.
With the current obstacles of a country-by-country approach in the EU, combined
with the new MDR regulatory standards that are stricter than the FDA (USA), Europe is
losing its attractiveness for innovative medical technologies. Ultimately, European
patients might pay the price by missing out on innovative solutions.”

“Make sure you understand the needs of ALL stakeholders
(beyond the patient, such as prescribers, payors, regulators,
sales channels, etc) as soon as possible. What type of
evidence or incentives do these different stakeholders need
to adopt your innovation? We all start with the patient
outcomes in mind, but you will only be able to reach this
goal if you have a good value proposition for all the
stakeholders and fit into the system.”
Wrapping Up
To summarize, the company is currently working on
securing public healthcare reimbursement for its first
product which focuses on chronic constipation in additional
EU markets. For this, the company has several clinical
studies ongoing or in preparation that will enable it to enter
a broader market, geographically speaking but also
clinically.
In parallel, Dr. Markus and his team are developing new
devices for other bowel dysfunctions, such as IBS, which is
a huge market by itself. Furthermore, the addition of a
digital platform to their technology and the potential of big
data that they can harvest for pathologies that are still
poorly understood but affect over a quarter of the general
population is huge.
“As we received the FDA breakthrough device designation
for our technology last year, we will naturally also start to
work on the market entry in the USA. And once again, all
this starts with a thorough analysis of the requirements and
needs of the different stakeholders,” concludes Dr. Markus.

harvest could be huge, but at this stage, I cannot go into
more detail,” says Dr. Markus.
Endeavors to Improve Care Measures for Constipation
With the development of the company’s device and the
‘Intermittent Colonic Exoperistalsis’ technology, the
MOWOOT team has completed a full circle of innovation:
from detecting an unmet need in the standard of care,
developing a solution to fulfil this need, and now forming
part of a new and improved standard of care. MOWOOT
device has already entered several clinical guidelines in
Germany, the UK and on the EU level and is reimbursed by
private and public healthcare in some of these markets. Dr.
Markus and the team are currently working on expanding
their success story to new markets and clinical indications.

“Currently, our main goal in the different EU markets is to
get reimbursement codes from the public healthcare system.

by-country approach,” highlights Dr. Markus.

They showed that the internet is such a precious invention for us from our business life to our private life. Even though this pandemic made us realize valuable things in our lives, it also showed us our deficiencies. We started to fulfill our needs through the internet during the pandemic. Its conditions affected our mental needs just like they affected our physical health. Although we were able to fulfill most of our needs through the internet, we could not fulfill the ones of our children. The most significant one was the situation I encountered when my 2-year-old daughter refused to be breastfed. There was a lockdown, and we did not know what to do. Hospitals were full of COVID cases; this might have caused serious health problems for me and my family. After some research I had done, I talked to a breastfeeding counselor on my phone. I did not know what to do because my daughter was hungry. I transferred the money which the counselor demanded to them. We met at our appointment time via an app similar to Zoom. This meeting was really beneficial to my daughter and her mother, but there was an unsafe space. People did not know how to contact the counselor or not trust them through only social media. God only knows how many children were brought up with such conflicts because of this trust issue. This is how the idea of Kidolog crossed my mind. Even though the reason why the process started was Born on May 10, 1991, in Kırıkkale. He completed primary, secondary and high school education successfully. In 2013, he got his bachelor's degree from the department of management in Ankara. After graduation, he started to work with his family in their company and also began to be interested in technological developments and programming. He established his first technological gym in Izmir. After selling this enterprise, which got attention with its success, to foreigners, he continued his serial entrepreneurship. He established the Botanik Café&Bistro brand and Lokkalata brand. With the Lokkalata brand, by giving franchises across Turkey, he was considered worthy of the Franchise prize by UMAD. He still owns businesses that are successful in the catering sector in Ankara, Eskişehir, and Kırıkkale. Right now, he is getting a master's degree at the Information Systems Institution of Hacettepe University. He is the co-founder and the CEO of Kidolog, which parent counseling platform launched in February 2021 for its user's service. He is married with a daughter. We are serving to parents who speak Turkish in many countries from Germany to China. Right now, we are still conducting our infrastructure works; after they are done, we will start to serve in German and in English. Shortly, we are going forward to be the best friend of parents from all around the world. breastfeeding, we tried to keep Kidolog as widespread as possible we could; for bringing up a child can also come with conflict that needs to be resolved eventually. So, we were there for mothers with experts who are suitable for all ages. With the midwife and doula support starting at the birth process, they can initiate the labor easily with pregnancy yoga and pilates. Generally, we give so much support to the ones who are going to be a mother for the very first time. They don't know what to do right after the birth, and with the professional support we gave to them, they can solve the problems which may be encountered after the birth with experts. Right after that, our breastfeeding experts will be there for the family. From Breastfeeding to Whole Process Problems of mothers, who are bringing up their children from the breastfeeding process to the age of 18, are being resolved by different experts. We’ve also realized that parents who are taking our support for their children also need to be helped as well. The amount of demand rose for this situation. We provided our support to parents as a whole by including specialist clinic psychologists in our system. It was quite valuable for a start-up that was established with zero funds. 3 months after the first investment, Mr. Özer and a couple of angel investors had a meeting with us to invest in the second one. After our meetings resulted as a success, we left the seed investment stage very efficiently. We improved our Turkey operations very significantly in this frame. We even achieved the success of being in the top 20 of the best start-ups in BigBang which is a competition that took place at Istanbul Technical University, which is among the prestigious universities in Turkey, only a year after our establishment. In January 2023, we received an investment of 7.5 million dollars valuation. With this investment, Kidolog, which was established 2 years ago, has increased its value 50 times. We won two financial rewards just as important as the investments on the stage. We were in first place in the development programs of very valuable banks. We were selected among the 13 best enterprises by one of the significant technology companies, Turk Telekom. We won many ranks and rewards in 2 years. These are our pride and joy and signs of how we can impact the future. We found many opportunities to improve ourselves while improving Kidolog. We took serious steps in the way of being entrepreneurs while raising the awareness of the parents. We celebrated this by being on the list of Fortune 40 under 40 very recently. New projects I have already mentioned the demand of parents. One of our missions is to have conscious parents and happy generations. We are conducting our work to make the world become a better place to live. For this mission, we are maintaining our support to the world with our projects that are being conducted and will be conducted later on. We sponsored a young women's football team for the purpose of supporting and raising awareness for girls. We are reimbursing their tuition. We are making plans to create a more sustainable world. We are carrying on our work for the purpose of green parenting and breast milk to become widespread. We started an operation in Europe, the United States of America and the United Arab Emirates in English, German and Arabic languages. But what was most striking for Dr.Wilhelms himself and the team were the shortcomings of the current standard of care in bowel management, especially for neurological patients (spinal cord injury, multiple sclerosis, and related). Existing solutions had low efficacy, several side effects and other major inconveniences for chronic patients and did not seem aligned with their needs. “We saw that even though the patients were suffering, many were reluctant to discuss this topic openly, due to its private nature and the taboo that is still associated with defecation,” says Dr.Wilhelms. “Maybe that was part of the reason for this unmet need. Somebody had to do something about it, and we were ready to take the challenge!” Alignment of Internal Operations with External Needs of the Market Being the CEO of the company, Dr.Wilhelms is consistently involved in activities like fundraising and usual paperwork. But his main contribution within the company goes with developing business strategy, aligning products and markets, and ensuring that each team member understands what is expected from him or her. This needs to be aligned internally and coordinated with the market externally to make sure our innovation reaches the patients that need it.” MOWOOT—an Alternative to Enemas and Laxatives MOWOOT is the first ‘Intermittent Colonic Exoperistalsis’ (ICE) device on the market, a non-drug and non-invasive solution without the drawbacks and side effects of the current standard of care. This device applies external pressure alongside the large intestine to substitute reduced or missing colonic contractions in severely constipated patients. ICE works in line with the physiological functions of the body and is not trying to work around them, as laxatives, enemas and other approaches do. As a result, ICE does not have the side effects of the other approaches and many patients describe the treatment even as pleasant. Such a device can simply be worn over clothes, sitting or lying down comfortably, and positive results have been shown with as little as ten minutes of daily use. In patients with permanent severe neurological pathologies that lead to neurogenic bowel dysfunction, ICE treatment manages the problem, but lifelong adherence is necessary. “We have indications that patients with less severe chronic idiopathic constipation can ‘retrain’ their colonic motility with our device, analogue to how an exoskeleton can retrain gait in movement-impaired patients. But this needs to be evaluated in further studies,” states Dr. Wilhelms. Apart from being a natural and safer approach, this prepares the body for a smooth transition from treatment to functioning autonomously and lowers the risk of dependency in those patients that might be able to regain a healthy gut function. Regarding the healthcare system in general, it is obvious that bringing a MedTech innovation to the market is a complex and resource-intensive endeavour. You not only need to deliver the expected clinical outcomes for the patients, but also make sure your technology fits into the healthcare ecosystem of prescribers, payors, regulators, etc. Especially reimbursement requirements are still largely different in each EU country and sometimes hard to predict due to a certain lack of consistency. While regulations and certain obstacles are necessary to make sure only safe and effective innovations enter the healthcare system, a unified European system would enable healthcare innovations to be adopted into clinical practices more efficiently, which in turn would guarantee equal patient access throughout Europe. The relevant stakeholders should collaborate to ensure that funding and reimbursement processes are as efficient and timely as possible. With the current obstacles of a country-by-country approach in the EU, combined with the new MDR regulatory standards that are stricter than the FDA (USA), Europe is losing its attractiveness for innovative medical technologies. Dr. Markus advises, “Make sure you understand the needs of ALL stakeholders (beyond the patient, such as prescribers, payors, regulators, sales channels, etc) as soon as possible. What type of evidence or incentives do these different stakeholders need to adopt your innovation? We all start with the patient outcomes in mind, but you will only be able to reach this goal if you have a good value proposition for all the stakeholders and fit into the system.” Wrapping Up To summarize, the company is currently working on securing public healthcare reimbursement for its first product which focuses on chronic constipation in additional EU markets. For this, the company has several clinical studies ongoing or in preparation that will enable it to enter a broader market, geographically speaking but also clinically. In parallel, Dr. Markus and his team are developing new devices for other bowel dysfunctions, such as IBS, which is a huge market by itself. Furthermore, the addition of a digital platform to their technology and the potential of big data that they can harvest for pathologies that are still poorly understood but affect over a quarter of the general population is huge. “As we received the FDA breakthrough device designation for our technology last year, we will naturally also start to work on the market entry in the USA. And once again, all this starts with a thorough analysis of the requirements and needs of the different stakeholders,” concludes Dr. Markus.
