An Investigation At Winterbourne View Hospital
Over the last 15 years or so a number of cases concerning patient care and safety have come to light
prompting investigations and inquiries that have led to changes in the way care is delivered. These
include inquiries at Winterbourne View hospital, Mid–Staffordshire hospital(Mid staffs) and Harold
shipman to name a few. At Winterbourne View Hospital, an undercover investigation for the BBC
panorama revealed practices that amounted to criminal abuse by staff towards patients. A serious
case review set up by the government in collaboration with the CQC, Local NHS and Police found
that patients were subjected too many incidences of Physical restraint and poor quality care. It also

Francis published a number of reports between 2010 to 2013 which revealed failings by mid staffs
in providing basic care; from provision of food, drink, pain relief, wash facilities to unhygienic
wards and toilets. The francis report also estimated that between January 2005 and march 2009,
approximately 400–1200 deaths occurred as a result of poor care.
The inquiry laid blame on poor staffing, lack of compassion, poor management. Francis also noted a
lack of concern when issues had been raised stating "Staff who spoke out felt ignored and there is
strong evidence that many were deterred from doing so through fear and bullying". Again other
agencies were slow to react to warning signs that things were not right at Mid staffs.
One of the recommendations that emerged from the francis report was the expectation upon all NHS
staff of a "duty of Candour". It says Every healthcare organisation and everyone working for them
must be honest, open and truthful in all their dealings with patients and the public, and
organisational and personal interests must never be allowed to outweigh the duty to be honest, open
and truthful.

Centred has various definitions yet not one that's universally accepted. Debra De Silva in her review

There are many components of implementing person centred coordinated care but I'll highlight those
I feel are important. Firstly, evidence shows that there needs to be a fundamental understanding that
patient choice matters. According to Hartley and Benington (2010, pp. 2–5) there are greater
expectations from patients and communities for more personalised and flexible care. Research
shows that a patient's decision regarding treatment change when they're well informed and this can
only mean a better experience of care for a patient. For example, an international Cochrane Review
that included 11 trials involving major elective surgeries showed that demand declined by 20% after
patients became well informed (Mulley et al, 2012). This also leads to greater convenience for
patients and having the option to go where they like and when they like for care can only become a
reality when true integration and coordination is achieved within the healthcare system. This relies
on clinicians accepting that they don't know best when it comes to patient preferences and letting
them decide their own preferences for receiving care and the settings where to receive it. I was

A child resulting from a pregnancy arising from a doctor's lack of care would appear to have a

Although it is now with Chase Farm Hospital, it employs 1100 less staff with the same hospital bed

An Investigation At Winterbourne View Hospital Over the last 15 years or so a number of cases concerning patient care and safety have come to light prompting investigations and inquiries that have led to changes in the way care is delivered. These include inquiries at Winterbourne View hospital, Mid–Staffordshire hospital(Mid staffs) and Harold shipman to name a few. At Winterbourne View Hospital, an undercover investigation for the BBC panorama revealed practices that amounted to criminal abuse by staff towards patients. A serious case review set up by the government in collaboration with the CQC, Local NHS and Police found that patients were subjected too many incidences of Physical restraint and poor quality care. Francis published a number of reports between 2010 to 2013 which revealed failings by mid staffs in providing basic care; from provision of food, drink, pain relief, wash facilities to unhygienic wards and toilets. The francis report also estimated that between January 2005 and march 2009, approximately 400–1200 deaths occurred as a result of poor care. The inquiry laid blame on poor staffing, lack of compassion, poor management. Francis also noted a lack of concern when issues had been raised stating "Staff who spoke out felt ignored and there is strong evidence that many were deterred from doing so through fear and bullying". In 2013 the Francis report highlighted the need for student nurses to test and experience their values and levels of care and compassion, and whilst it is seen that the failures highlighted in the report go further than a lost compassion of nursing, this exercise is to look at the key concepts of care and the principles of the 6 Cs (Care, Compassion, Courage, Communication, Competence and Commitment) which have been identified as fundamental values, although not the only ones as nurses have other core values such as organisational, professional and NHS values to work to. I have selected one of the 6 Cs (communication) for a more detailed description from which I will then provide as requested in my brief an example specific to its application in a particular service. The area I have selected to apply this to, is Dementia. The nature of the interrelation of care and nursing has been recognised by theorists over several decades internationally, back in 2002 caring both as a natural attribute of being human and the core of nursing was identified by Sister Simone Roach of Canada. Sister Simone Roach was responsible for a similar set of the 6Cs to the ones we recognise today. Several reports of poor care arose in 2009 regarding Mid Staffordshire hospital which concluded that patients needed to be protected from harm, patients had been subject to unacceptable risk of harm and that patients had failed to be treated in the appropriate way.
