In end-of-life care….
• Should spiritual and religious needs be
considered routinely?
• Who should assess these?

• Who should offer spiritual care?

• There is a growing body of evidence that
indicates that a focus on religion and spirituality,
broadly defined, can have a positive impact on
the ways in which patient's perceive and
experience illness including end of life issues, and
can be beneficial in terms of mental and physical
health. [222; 166; 116].

Spiritual care
• that care which recognises and responds to the
needs of the human spirit when faced with
trauma, ill health or sadness and can include the
need for meaning, for self worth, to express
oneself, for faith support, perhaps for rites or
prayer or sacrament, or simply for a sensitive
listener.
• Spiritual care begins with encouraging human
contact in compassionate relationship, and
moves in whatever direction need requires [175]

Clarity
• Religion: the group (transpersonal) experience of
spirituality, extending over generations,
accompanied/reinforced by a distinct and
coherent story and ritual action
• Spirituality: the personal experience of the
religious narrative and ritual action, as part of, or
in contrast to the distinctive and coherent group
experience
• BOTH comprising “elements of meaning,
purpose, and connection to a higher power or
something greater than self” [201;60]

Spiritual pain and distress
• loneliness of dying
• struggle to accept the dying process.
• identity and personal resources are changed or
diminished
• life has not gone as the person wished, too late to
do anything about that.
• and not being forgiven by God; not being
reconciled with others; dying while cut off from
God or some higher power; not being forgiven by
others for a past offence

Reasons given for not considering faith
• patient and/or family reluctance;
• not knowing what to ask or how to approach the subject
• no available assessment tool (I would add ‘needing a tool’)
• feeling embarrassed or awkward
• feeling reluctant to bring up a personal subject
• expectation that the chaplain or social worker would do it
• stating that they themselves did not believe in God
• the patient being unable to communicate
• the patient being short‐term.
• Avoiding harm or offence to religious/cultural sensitivities, or
• aiming for a ‘good death’?

• In Alistair’s notes it is recorded that he is a
Catholic, but he had angrily declined to see
the priest when in hospital.
• However, one day, a few weeks before his
death, he draws a picture of a man struggling
to climb the steps of a church whose door was
closed with a large key protruding.
• Alistair presents this picture to you, watching
for your reaction

  • 3. In end-of-life care…. • Should spiritual and religious needs be considered routinely? • Who should assess these? • How? • Who should offer spiritual care? • How? 3
  • 4. • There is a growing body of evidence that indicates that a focus on religion and spirituality, broadly defined, can have a positive impact on the ways in which patient's perceive and experience illness including end of life issues, and can be beneficial in terms of mental and physical health. Spiritual care • that care which recognises and responds to the needs of the human spirit when faced with trauma, ill health or sadness and can include the need for meaning, for self worth, to express oneself, for faith support, perhaps for rites or prayer or sacrament, or simply for a sensitive listener. • Spiritual care begins with encouraging human contact in compassionate relationship, and moves in whatever direction need requires [175] 5

Clarity • Religion: the group (transpersonal) experience of spirituality, extending over generations, accompanied/reinforced by a distinct and coherent story and ritual action • Spirituality: the personal experience of the religious narrative and ritual action, as part of, or in contrast to the distinctive and coherent group experience • BOTH comprising “elements of meaning, purpose, and connection to a higher power or something greater than self” [201;60] 7

Spiritual pain and distress • loneliness of dying • struggle to accept the dying process. • identity and personal resources are changed or diminished • life has not gone as the person wished, too late to do anything about that. • and not being forgiven by God; not being reconciled with others; dying while cut off from God or some higher power; not being forgiven by others for a past offence 8

Reasons given for not considering faith • patient and/or family reluctance; • not knowing what to ask or how to approach the subject • no available assessment tool (I would add ‘needing a tool’) • feeling embarrassed or awkward • feeling reluctant to bring up a personal subject • expectation that the chaplain or social worker would do it • stating that they themselves did not believe in God • the patient being unable to communicate • the patient being short‐term. • Avoiding harm or offence to religious/cultural sensitivities, or • aiming for a ‘good death’? 9

What’s your interpretation • In Alistair’s notes it is recorded that he is a Catholic, but he had angrily declined to see the priest when in hospital. • However, one day, a few weeks before his death, he draws a picture of a man struggling to climb the steps of a church whose door was closed with a large key protruding. • Alistair presents this picture to you, watching for your reaction 17
