On the year under review, the Cooperative
Development Authority Dagupan Extension Office
utilized its manpower and resources to contribute to our
organization objectives “Growth and Viability of
Cooperative Enterprises Improved” with four major
programs namely: Cooperative Development Program
(CDP); Cooperative Regulatory Program (CRP); Support
to Operations (STO); and General Administration
Services (GAS).
This report shares some of the details of our
activities and highlights the accomplishments that give
us the opportunity to realize the breadth of our
commitment to our cooperators and our mandate to
promote the viability and growth of cooperatives as
instruments of equity, social justice, and economic
development in fulfilment of the mandate in section 15,
Article XII of the Constitution
A. Cooperative Development Program
The Cooperative Research, Information and Training Unit engaged themselves with
extensive activities on handholding of micro and small cooperatives, delivered various technical
assistance services, entered a partnership with stakeholders in the conduct of capability building
programs for cooperatives in line with the mandatory training for officers, and the conduct of
Pre-Registration Seminars. We also conducted the annual search of Gawad Parangal for
outstanding primary cooperatives, documented their success stories, co-implemented the
conduct of the Cooperative Month Celebration, distributed IEC materials, conducted cooperative
taxation education, and koop-balitaan, processed the accreditation renewal of both the ATPs
and CEAs and conducted GAD related activities. The overall performance of CRITS was “Very
Satisfactory”. The commitment to customer service runs deep within our entire organization,
thus, the feedback rating from clients was a “better” rating
In supporting the entrepreneurial aspirations of the cooperative, the Cooperative Project
Development Assistance Unit, implemented programs like Koop Kapatid Program, encouraged
cooperative members to participate in M2C, and assisted the cooperatives to be involved in
C2C and C2NC, monitored the contribution of entrepreneur members in job generation and
income increases. Support from the national government agencies such as DTI, DAR, DA,
NCIP, DOST as well as the LGUs helped in strengthening the position of cooperatives as
business enterprises. Additionally, the CPDAS also advocated in the cooperative business
process through the value chain, monitored its financial inclusion program, and enrolled
cooperatives to CBMIS. The overall performance of CPDAS was “Very Satisfactory”. From the
customer's feedback perspective, the rating received from clients was “better”.

B. Cooperative Regulatory Program
One thing that hasn’t wavered is our commitment to register cooperatives. The Registration
Unit performed well in registering new cooperatives, as well as the processing of amendments
in their ACBL, with registration documents acted upon within the prescribed period based on the
citizens charter, issued Certificate of Authority to cooperatives establishing branches, Letter of
Authority was issued for approved cooperative satellites and Certificate of Recognition to
Laboratory Cooperatives and updating of COOPRIS. Our clients rated the registration process
better.
The Regulatory Unit continued to monitor the health of cooperatives thru inspection. Reports
on inspection were evaluated to determine the gaps in their operations. Cooperatives with
complete mandatory reports were processed and evaluated. Compliant cooperatives were
issued their COCs while the non-compliant cooperatives were indorsed for appropriate action or
interventions. The RU is also responsible for the consolidation of ATIR. All in all the RU was
rated as better by clients.
On the enforcement activities, the Legal Unit acted on complaints within 15 days upon
receipt of reported violations or complaints. Non-compliant cooperatives were given SCOs,
rendered various legal opinions, conducted investigations, and well as facilitated the conduct of
mediation and conciliation. Moreover, the Legal Officer acted in the recommendation to dissolve
non-compliant cooperatives or based on the inspection findings recommendations of the CDS-
IIs. The overall performance of this unit was “Very Satisfactory”.
C. Support to Operation
We also continued to expand the footprint of CDA thru institutional strengthening by way of
improved developmental service delivery mechanisms and enhanced members and public
protection framework. Planning and meetings sparked a passion and provided perspective for
the direction of the Agency. Firstly, STO covered the Agency’s plan preparation,
implementation, and monitoring. Secondly, we continued the monitoring of the Philippine
Cooperative Medium Term Development Plan.

On the other hand, STO covered Quality Management Systems (QMS), thus the Key
Performance Indicators (KPIs) were religiously monitored and analyzed coupled with the regular
conduct of 5S. To update our QMS team, they also attended training relating to IQA. The office
was audited by both the IQA and the surveillance audit.
The MIS regularly maintained and updated the EO website, upgraded the internet
connectivity, every so often preventive maintenance on desktops and laptops such as the latest
OS were installed with the latest antivirus for personnel. Moreover, the MIS posted articles on
the website.
The General Administration Services was up to date in the budget preparation and
execution program with FA reports was prepared on time and in addition, the financial
performance of the Agency was assessed during the mid part and year-end of 2019.
Noteworthy to mention was the 2019 financial performance in which allocation and obligation
was 100%.
The Human Resource Development was active in facilitating the preparation of the
Individual Development Plan which became the basis for the Agency’s capacity interventions for
personnel and conduct of HR development activities. To have an appreciation of quality and fair
performance rating, an orientation on SPMS was conducted. The OPCR and IPCR were
regularly submitted and assessed by PMS. The HR performed the recruitment and placement
process in accordance with CSC rules and Agency regulations.
The General Services implemented the motor vehicle maintenance program, building, and
equipment maintenance program conducted physical inventories under the property
management program. The GS was also responsible for carrying out the disaster preparedness
activities in cooperation with the Bureau of Fire Protection.
The Records Officer performed the records and document management like sorting and
maintenance of records. Moreover, records were classified for retention and disposal. For the
cashiering services, revenue generated was remitted on time to the bank and recorded
appropriately.
A major development is the compliance to ISO 9001:2015 requirements to
consistently provide products and services that meet customer and applicable statutory
and regulatory requirements and to enhance customer satisfaction through the effective
application of the system, including processes for improvement of the system and the

assurance of conformity to customer and applicable statutory and regulatory
requirements.
This is in the heart of our business and is ingrained in our DNA. The CDA conducted
employee training on QMS/IQA. Our employees have an increased awareness of the
importance of a job well done, and a heightened level of pride and independence.
The Agency has engaged in extensive consultations with all its stakeholders including
partners from the National Government Agency. As a result, two recognition was given to CDA
namely: 1) NCIP - Invaluable contribution to the successful conduct of Regional Action Planning
Workshop 2) Regional Competitiveness Committee - Invaluable assistance and exceptional
support as Partner Agency.
The Villar SIPAG supports and recognizes organizations that help our less fortunate
countrymen break free from the clutches of poverty. This is a public service program of
Senator's Manuel and Cynthia Villar who have a passionate desire to search for those who have
an advocacy in improving the quality of lives of their fellow men.
There were two cooperatives from the 40 cooperatives recipients of Villar Sipag
Awardees for their contribution to poverty reduction. The awardees were: Albacopa Federation
of Cooperatives of Pangasinan and Suyo Multi Purpose Cooperative of Ilocos Sur.

The members of the REXECOM likewise contributed to facilitating the changes needed
and creating an effective working environment. They contributed to making management
decisions, monitor performance, and make sure everything is done in compliance with the rules
and regulations of the CDA and to cater to the needs of the organization.
We continue to grow and evolve. As of December 31, 2019, our workforce is 39 for Region
1. An increase of six personnel with two contracts of service (COS). To our stakeholders, thank
you for making our work possible and for partnering with us in implementing our programs and
projects. We humbly request the continued support and cooperation of our shareholders as we
move forward.

In closing, I would like to acknowledge the Board of Directors for their continued
stewardship and guidance throughout the past year. I would also like to thank our dedicated

Accomplishment Report from Cooperative
Research, Information and Training
This consolidated report discloses the annual performance review of CDA Dagupan
Extension various units including provincial reports. In addition, The Cooperative Research,
Information, and Training Unit have achieved significant milestones on its programs and
projects implemented.
1) Handholding and Technical Assistance Service
The total number of TAS rendered by the field CDS for the year 2019 is 1,211, where
various issues and concerns of the cooperative sector were provided by way of inquiry, training
assistance, mentoring, coaching, cliniquing, and consultancy. These TAS were rendered to

2) Capability Building Program for Cooperatives
The CDA assisted in the conduct of mandatory training for cooperatives such as
Fundamental of Cooperatives, Governance and Management, Enhancement Training for
Officers and staff, and conduct of Pre Registration Seminar

(143) PRS conducted for the year both in the
regional and field level taking into
consideration the center and most accessible
place for 6,433 prospective members. There
was a deluge of requests for PRS on account
of those wanting to organize transport co-ops.

Mandatory Training for Officers
Governance & Management are the two
training components identified by our agency
which co-op officers should undergo.
Fundamentals of Cooperatives and
Governance cum GAD Orientation were
conducted on April 10-12, 2020. To maximize
the free orientation seminar sponsored by the
LGU, the participants were also oriented on
the Gender and Development (GAD) in
Cooperatives as mandated by the agency.
CDS II Florentino B. Lorenzo discussed the
basic concepts and principles of GAD in the
context of oops’ governance.
The LGU of San Carlos City, Pangasinan headed by its City Cooperative Development Officer,
Arlaine M. De Vera requested for the conduct of the said seminar for the registered
cooperatives operating in San Carlos City. It was held in the DTI – Negosyo Center Building. 33
participants attended from the 16 coops who attended the activity on July 25 - 26, 2019.
The City of Candon, - Office for the Development of Cooperatives provides technical
assistance in organizing new cooperatives and registers them at the Cooperative Development
Authority (CDA). It is headed by Ms. Grace L. Ramos, City Cooperative Development Officer the
CDO conducted basic mandatory seminars for co-op officers should as required by the agency.
A total of 40 participants (16-Male; 24-Female) coming from 17 micro co-ops attended the
Fundamentals of Cooperatives and Governance and Management Seminar.on June 21, 2019).
The activities were conducted in recognition of the Memorandum of Agreement between CDA -
DEO and the LGU – Candon City.
The LGU of Batac City thru their City Agriculturist, Ms. Marylime T. Gappi requested for the
conduct of the said seminar for the registered micro cooperatives operating in Batac City. It was
attended by 38 participants coming from 7 micro cooperatives. Field CDS, Rosievic R. Medina
also pitched in as she oriented the participants on the reportorial requirements of the Agency. A

total of 71 participants (39-Male; 32-Female) coming from 18 micro co-ops attended the October
9 -10, 2019.
The City Government of Vigan has requested for the conduct of the two (2) day seminar
for Vigan-based micro cooperatives. The City Cooperative Development Office, headed by
Hermie T. Etrata initiated the activity in coordination with CDS II Raymund N. Pilorin. There
were 28 officers coming from 12 Vigan-based coops who attended the activity. CDS II Pilorin
also pitched in as discussant in the seminar and provided technical assistance incidental to the
said activity on October 22 - 23, 2019.
The LGU-Bayambang coordinated with the CDA thru District CDS II Sheryl Lou M. Fabia
for the conduct of the seminar for the cooperative supported by their local chief executive,
Mayor Cezar T. Quiambao whose membership were his classmates in high school. District CDS
II Sheryl Lou M. Fabia also assisted in the activity working in close coordination with the LGU –
Bayambang. 17 officers attended the seminar on the fundamentals of Cooperatives and
Governance on October 29-30, 2019 at Bayambang, Pangasinan.
Reportorial Requirements
It’s that time of the year to remind and inform registered cooperatives on their
submission of mandatory reports and secure the Certificate of Compliance (CoC) with
the agency with an end view of increasing the number of compliant cooperatives in the
country.

The participants were also advised on their compliance with R.A. 10708 otherwise known as
the Tax Incentives Management & Transparency Act (TIMTA) which requires their submission
of a tax incentive report to the CDA for onward submission to the BIR.
Aside from walking through the participants on how to undertake the online submission of
reports, the team also updated the participants regarding the Cooperative Business Matching
Information System (CBMIS) platform of the agency and the online payment of CDA
Registration fees through the Landbank Link. Bizportal.
Enhancement Training for Co-op Officers
To capacitate co-op members and officers we have coordinated the conduct of the
Entrepreneurship Trainings at San Fernando City, La Union on February 26, 2019. The district
activity was undertaken in partnership with the DTI La Union Provincial Office headed by
Provincial Director Merlie D. Membrere wherein their resource speakers talked about
Consumer Education & Awareness and Fair Trade Laws.

4) Accreditation Program for Training Providers, External Auditors and Med/Com
Anticipating the number of accreditations expiring in the year, the EO targeted 5
accreditation applications and/or renewals for ATPs and 7 for CEAs to be an endorsement to
the CO. There were 4 EAs who were able to secure their initial and/or renew their accreditation
and 3 Training Providers were able to renew for the year.

Tax Forum: A Dialogue for the Uniform Interpretations on the Cooperatives” Application for
Certificate of Tax Exemption, Payment of Documentary Stamp, and Local taxes
The activity was participated by five (5) representatives from the BIR Regional
Office, together with six (6) CDA personnel and three (3) representatives from the Local
Government Units of Alaminos City, Pangasinan, and 15 representatives from the cooperative
sector. Our host for the dialogue was the CCDC, and CCDO of Alaminos City on February 12,
2019. As a consensus, a position paper shall be submitted by the CDA - DEO to the BIR Central
Office concerning the issues and concerns raised and discussed during the dialogue. The forum
sought to address the perennial dilemma of cooperatives in their application for Certification of
Tax Exemption (CTE).

Pertinent tax laws and issuances were presented during the discussion. There was also some
jurisprudence presented as a basis for legal interpretation on the treatment of cooperatives’ tax
privileges.

Compliance of Cooperatives in the Submission of Annual Report Tax Incentive Report
according to the DOF-CDA-BIR Joint Administrative Order No. 1 Series of 2019
The Joint Administrative Order JAO) No. 1-2019 (Inclusion Law (TRAIN Law) and CDA
Memorandum Circular (MC) No. 2019-06, Series of 2019 requires all types and categories of
cooperatives registered with the CDA which were issued with the Certificate of Tax Exemption
(CTE) by the BIR to submit a complete Annual Tax Incentives Report of their income-based tax
incentives, value-added tax, and duty exemptions, under R.A. No. 9520 as mandated under
Section 4, paragraph (2) of R.A. No. 10708 and Section 3 of R.A. No. 10963, otherwise known
as the Tax Reform for Acceleration. The activity sought to inform and enable the cooperatives to
comply with the said directive. The activity was attended by representatives from the BIR –

6) Business Continuity Management
In 2016, the CDA entered into an agreement with the Humanitarian Leadership
Academy (HLA) Philippines Centre (now the Center for Humanitarian Learning and Innovation)
to develop and After the training, the trained CDA personnel are expected to roll out the BCM
for Cooperatives in their assigned areas as an agency deliverable to teach them as business
enterprises what to do before, during and after an identified hazard to be able to resume
providing goods/services to their members the soonest possible time.
The Ilocos Consolidated Cooperative Bank sponsored the 1st leg training on BCM on July
30-31, 2010. The activity was conducted in two (2) batches for the co-op - affiliates of the ICCB
in Ilocos Sur and Ilocos Norte. It aimed to teach the participants how to be better prepared and
more resilient to business disruptions brought about by natural and human-induced hazards by
coming up with a Business Continuity Plan (BCP), and incorporating their existing emergency
plans thereto. A total of 45 participants coming from 30 cooperative-affiliates participated in the

activity which was held in the ICCB Conference Room located at Quirino Boulevard, Bantay,
Ilocos Sur.
As the 2nd batch of the activity, there were a total of 24 participants coming from 18
cooperative-affiliates of the ICCB operating in the province of Ilocos Norte. They were also
oriented on how to prepare their own BCP based on the hazard which they have identified. The
activity was a discovery for both the participants and the facilitators on the level of preparedness
and contingencies which their cooperatives as business enterprises have already in place
before, during, and after the occurrence of the hazard. There was a lot of sharing of experiences
and ideas during the presentations of the different BCPs of the participants. However, a major
challenge in institutionalizing BCM in the agency is its budget constraints since legislative action
is required for budget allocation which consequently limits the ability of the agency to carry-out
BCM internally and rolling it out to the stakeholders. Thus, the partnership with the HLA was a
good initiative to increase the capacity of the CDA in cascading information to cooperatives that
are at risk of hazards that threaten their survival and sustainability.

7) Conduct of Campaign Activities thru Quad Media
The PIA – PANGASINAN is an avid supporter of the CDA in terms of information
dissemination and cooperative advocacy. The extension office is regularly invited in its radio
program, “Pantongtongan Tayo” (Pag-usapan Natin) aired over DZMQ Radyo Pilipinas –
Dagupan on October 1, 2019, with its anchor, Ms.Imeeh Casiño. It is an opportunity for our
agency programs and activities to be disseminated and promoted.
Mr. Enriquez announced the conduct of the Regional Cooperative Month Celebration on
October 18 - 19, 2019 in Alaminos City, Pangasinan in partnership with the RCDC – I and the
City Government of Alaminos as represented by Mr. Solomon B. Tablang,
City Cooperative Development Officer/RCDC – I Secretary. Ms. Casiño inquired on what the
CDA has been doing regarding the proliferation of “pseudo-cooperatives” used by unscrupulous
individuals to lure people in investing their hard-earned money. He explained the rationale of the
said activity why it is celebrated annually in our country as provided in Proclamation No. 493, S-
2003. He advised the public who are interested to join a cooperative to verify first with the CDA
its status whether if it is indeed registered or not operating or dissolved to avoid being swindled.

9) Monitoring of Cooperative Development Councils (CDCs). The first regular meeting was
attended by representatives from the different National Government Agencies and Local
Government Units namely DILG, BFAR, DTI, PGP-SP, NFA, DOLE, LBP, DAR, NEDA, CDA
Office of the Provincial Agriculture Office of La Union, PPO-Pangasinan, DENR-EMB and the
City Government of Alaminos City, Pangasinan. There were also 14 cooperatives which
attended the meeting from the 4 provinces of Region 1. The agenda of the meeting is as
follows:

8) Gender and Development Program (GAD)
In line with the Social Development Program of the La
Union MPC and support to the advocacy in
Mainstreaming Gender and Development (GAD) in Co-
operatives, the coop initiated the conduct of a FREE
one (1) day GAD Orientation Seminar for micro and
small coops The activity was accomplished in
coordination and assistance of the DTI – Provincial
Office headed by its Provincial Director, Merlie D.

Among the highlights of the meeting is the presentation of a Research
Entitled: Development of Performance Models and Capacity-building Framework for Micro and
Small Multi-Purpose Cooperatives in Ilocos Norte by Mr. Errol John Valdez, MMSU.
The 3rd regular RCDC meeting was held at Capitol Resort Hotel, Lingayen, Pangasinan
on September 19, 2019. The agenda was as follows: 2019 Regional Cooperative Month
Celebration (RCMC) Planning, Agricultural Programs for Cooperatives, Updates on CDA
programs, advocacy and activities: Republic Act No. 11364, otherwise known as the CDA
Charter of 2019, Status of the cooperatives’ compliance in the submission of Annual Tax
Incentive Report and the Result of the Regional Screening and Board of Judges Validation and
Deliberation in the CDA 2019 Gawad Parangal Awards and Recognition Program. The meeting
was conducted in cooperation with the Provincial Government of Pangasinan represented by
Mr. Ellsworth G. Gonzales, Provincial Population Officer/RCDC –Treasurer.
Ms. Renee Faye DG. Cariño discussed the procedures in TIMTA reporting, the filling-
out of forms to be submitted to CDA. The Chairperson commented that the council should help
the coops in the submission of the said report and it should be proactive. Some of the council
member-NGAs also presented their programs in which co-ops may participate
Reorganization of the Provincial Cooperative Development Council (PCDC) of Ilocos Sur
The collaborative goal of reorganizing the PCDC of Ilocos Sur has finally been realized last
Oct 21, 2019, Vigan City, Ilocos Sur thru the coordination and efforts of PCDS Raymund N.
Pillorin with the Provincial Government of Ilocos Sur represented by Ms. Lizel H. Alonzo,
Provincial Cooperative Officer. Following the CDA – Memorandum Circular, the following were
the elected officers of the council: Hon. Juan Carlo S. Medina, Chairperson; Dr. Antonio

10) The 9th National Tripartite Conference was conducted on September 11-13, 2019 at
KCC Convention and Events Center, Gen Santos City, South Cotabato. The National Tripartite
Conference is a regular activity held every two years headed by the LCDOP. The LGU of
General Santos City hosted the 9th National Tripartite Conference for Cooperative
Development. This year's theme "Kapayapaan, Kaunlaran at Kalikasan sa Diwa ng
Kooperatiba" aims to strengthen the cooperatives, the local government units, and the private

The CDA continued to coordinate with various LGU partners in preparation for the annual
cooperative month celebration. Among our hosts for these events were: Alaminos City (RCMC),
Province of Ilocos Norte (PCMC), and Candon City (CCMC).
The 2019 Regional CMC was held in partnership with the LGU of Alaminos City,
Pangasinan. As a keynote speaker, our very own Administrator Vidal D. Villanueva III was
invited to the celebration. It was a two (2) day event wherein the participants from the region
were given the option to have a tour of the Hundred Islands during the first day

11) Mini Planning Workshop held at CDA La Union Provincial Office on January 22, 2019
The activity sought to cascade to the field personnel this year’s Work & Financial Plan
(WFP) of the agency thereby facilitating the submission of their targets by the parameters of the
WFP.

The Cooperative Project Development and Assistance Section (CPDAS) humbly report
the following accomplishments for the period January – November 2019 in
coordination/collaboration with the CDA-DEO personnel, particularly to the technical staff.
Handholding Program for Micro and Small Cooperatives - A total of twenty-nine (29) hand-
holding rendered to micro and small cooperatives through mentoring and coaching namely:
Orientation on Koop Kapatid program, enrollment to the Cooperative Business Matching
Information System, training assistance on bookkeeping, financial & records management,
identification of technical assistance & capacity building needed for the Koop Kapatid program,
preparation of mandatory reports, online submission of CAPR, requirements for the BMBE
Registration and Assistance to Registration Documents of Coops
Technical Assistance Services (TAS) – There were eight (8) cooperatives provided technical
assistance specifically on the Memorandum of Agreement for the Koop Kapatid Program &
implementation of activities and advisory on MC 2019-08.
Capability Building Program for Cooperatives
Koop Kapatid– Big Brother – Small Brother Concept. This concept has been around for
quite a while now. It was credited to a certain Ernest Coulter who founded the Big Brothers
Movement in America in 1904. He intended to recruit volunteers who will act as big brothers to
young kids and mentoring them as they grow into adulthood. The Co-operative movement had
likewise adopted this concept and it is being recommended to the co-operatives under the
principle of “Cooperation among Co-operatives”. Recognizing the soundness of this idea, other
organizations incorporated it in their respective programs (Bawang, 2018). Under this program,
the target is one per province and as of this date, we have accomplished the following: for
Training Needs Analysis, seven were conducted broken down into:

The cooperatives enrolled in the Koop Kapatid Program for the year 2018 was monitored,
to wit:

1. La Union –– St. Louis Credit Cooperative as the big brother endorsed to the La
Union Provincial Cooperative Union (LU PCU) an accredited training provider
of the Authority the small brother (Sitio Malanas Credit Cooperative) to attend
the training such as Strategic Planning (4 officers) and Financial Management
(4 officers). The training fee was funded by the big brother coop.
2. Ilocos Sur –
2.1 Big Brother – Sto. Domingo Development Cooperative and Small Brother is Sto.
Domingo Municipal Employees Savings and Credit Cooperative.
Only the part-time bookkeeper of the small brother cooperative was oriented
regarding the accounting system of the Big Brother Cooperative. After the orientation,
the accountant requested the books of account of the small brother for actual recording
in the books of account but unfortunately, according to the bookkeeper of the small
brother coop, their records were in the hands of the accountant of another cooperative
which they seek helped and until now the source documents were not yet returned to the
cooperative.
2.2 Big Brother – Fatima Vigan MPC and Small Brother is Pantay Onion Producers
Cooperative.
The officers of the Small Brother Cooperative were not able to undergo job training
on the possible technical assistance to be provided by the Big Brother Cooperative as
stated in the MOA (Bookkeeping; Records keeping, Fundamentals of Cooperative;
Cooperative Management and Governance and Rules Formulation) for the following
reasons: 1. Some officers were on training in Manila on Good Agricultural Practice and
others were busy on the farm for the production of white Onion. They promised that the
technical assistance to be provided by Big Brother Cooperative will be implemented this
year.
3. Pangasinan – Big brother – Calasiao Plant & Related Companies Employees MPC and
Small brother is Alyansa, Bayanihan at Kapatiran Central Pangasinan Agriculture
Cooperative. The identified training was Bookkeeping, Fundamental of Cooperatives,
Leadership & Values re-orientation, Strategic Planning & Rules Formulation.

The scheduled activities of the Big Brother and Small Brother program were not
implemented last year despite the willingness of the big brother cooperative to share
their experiences and learnings for the reason that the officers and staff of the small
brother coop cannot decide as to when the trainings are conducted because of their
schedules. However, the two cooperatives both agreed that they will implement the
identified activities this
4. Ilocos Norte – Big Brother is San Joaquin MPC and Small Brother is San Leandro MPC
The bookkeeper of the San Leandro is having her bookkeeping training at the San
Joaquin MPC since November 2018 (every Saturday).
For 2019, the following coops were also monitored:
1. Gledco MPC, big brother of Balatong MPC &BaggakTiDaya Development
Cooperative conducted a dialogue & discussed issues and concerns on capital build
up programs and lending policies & collection.
2. Metro Paypayad MPC, big brother to Darapidap Abadaya Fisherfolks Savings and
Credit Cooperative provided the following assistance such as review of policies,
cooperative forms, loan processes, conduct of board meeting & bookkeeping
process.
3. Free Farmers MPC, big brother of Cuancabal MPC, conducted the mandatory
trainings namely Fundamentals of Cooperatives & Cooperative Management &
Governance.
4. United Christians MPC, big brother provided assistance on bookkeeping to Lewesen
Mt. Range Agro Forest Soft Broom Producers Cooperative.
IV. Coop – Sustainable Enterprise Development (COOP-SEED)
The COOP-SEED Program was extended until the first Quarter of this year to achieve
some deliverables that were not accomplished last year. An assessment at the national level
was conducted on March 12-13, 2019 at Quezon City. Among the deliverables of the program
that was accomplished until the 1st
Quarter this year were the following:
1. Four (4) entrepreneur-members registered under BMBE (Garnaden MPC);
2. Reported the two coop beneficiaries in terms of contribution of entrepreneur member in
job generation and increase in income profile;
3. (1) M2C marketing transactions of the two coop beneficiaries were also documented &
reported;
4. (2) C2C marketing transactions were documented and reported;
5. (9) C2NC marketing transactions were documented and reported.
V. Strengthening partnership with NGAs, LGUs, academe and other stakeholders
1. Department of Trade & Industry (DTI) 30 cooperatives (P=17; LU=3; IS=3; IN=7) were
referred to avail the programs and services and 40 coops (P-30; LU=3; IS=4; IN=3) were

monitored which availed the programs of DTI such as training, financing forum, BMBE
registration, shared service facility, P3 Program, and trade fairs and Mentor Me on Wheels.
For the business matching trade fairs facilitated, 2 from Pangasinan were accomplished
(1 in Alaminos City and 1 in Umingan), 1 Regional (during the Regional Cooperative
Month Celebration), and 1 in the participation of coops to Rima Ti Arianna Trade Fair at
Robinson’s Mall, Manila.
2. National Commission on Indigenous People (NCIP) – fifteen (15) technical assistance
were provided to IP’s group (P=6; IS=6; IN=3). Technical assistance rendered were: the conduct of PRS,
orientation on Koop Kapatid program, the encoding of CAPR/AFS, Mandatory reports, preparation of
ACBL, and bookkeeping.
3. Local Government Units - a total of 52 activities coordinated/conducted such as
training, participation in various celebrations, and other coop activities. (P=29; LU=10; IS=7;
IN=6).
VI. Strengthening of Cooperatives Business Enterprise – an orientation on Financial
Inclusion was conducted as one of the topics during the Regional Cooperative Month
Celebration held at Alaminos City, Pangasinan on October 19, 2019, with a representative from
the Bangko Sentral ng Pilipinas as resource speaker.

the TESDA Office spearheaded the conduct of a Dialogue Meeting with the Municipal
Government of Salcedo and the target beneficiaries of PRLEC programs on September 24,

The enactment into law of the Credit Surety Fund Cooperative Act of 2015 on 6 February
2016, is an enhanced financial landscape for Filipino entrepreneurs. Specifically titled “An Act
Providing for the Creation and Organization of Credit Surety Fund Cooperatives to Manage and
Administer Credit Surety Funds to Enhance the Accessibility of Micro, Small and Medium
Enterprises, Cooperatives and Nongovernment Organizations to the Credit Facility of Banks and
for Other Purposes”, the new law institutionalizes the Credit Surety Fund (CSF) Program

Cooperative Regulatory Program
Registration Unit
One of the primary functions of the Cooperative Development Authority is the registration of
cooperatives including amendments thereto. In line with this function are the following:
I. Issuance of Cooperative Name Reservation Notice (CNRN)
• Submission of Cooperative Name Reservation Report Form (CNRRF) shall be submitted
either to CDA Central Office or Regional Offices nationwide through personal, postal mail,
courier, electronic mail, or online.

• The applicant has the option to choose from the following period to reserve their proposed
names with the corresponding reservation fees
a. Thirty (30) calendar days - Php 100.00
b. Sixty (60) calendar days - Php 200.00
c. Ninety (90) calendar days - Php 300.00
• Cooperative name shall be under the prescribed guidelines.
• All reserved names shall be valid per the period specified in the Cooperative Name
Reservation Notice.
II. Issuance of Certificate of Registration (COR)
The prospective cooperative acquires its juridical personality upon issuance of a Certificate
of Registration.
III. Issuance of Certificate of amendments
This is being issued in case of any amendments in the Articles of Cooperation and by laws
of the Cooperative
lV. Issuance of Certificate of Authority (COA)
A Certificate of Authority shall be issued by CDA to cooperatives applying for the
establishment of a branch office upon compliance with the requirements. A Branch Office refers
to a business office outside the principal office where cooperative activities and business
operations are undertaken as per the approved cooperative development plan.
V. Issuance of Letter of Authority (LOA)
A Letter of Authority shall be issued by CDA to cooperatives applying for the establishment
of a satellite office upon compliance with the requirements. Satellite Office refers to an office
established by a cooperative outside of its principal/main office but within its area of operation to
provide limited services to its members but which does not however maintain books of accounts
as it is done only by the principal/main office.
Vl. Issuance of Certificate of Recognition (CoR)
Certificate of Recognition shall be issued for the organization of laboratory cooperative for
minors (ages below 18 years old) by a Guardian Cooperative.
VlI. Issuance of Certificate of Merger or Consolidation
This refers to the process where two or more cooperatives become a single cooperative,
which, in the case of a merger, there is a surviving cooperative and consolidated cooperative in
the case of consolidation.

seminar was conducted on July 18 - 19, 2019
with approximately 30 participants coming
from different cooperatives in Pangasinan.

of LGU Dasol, another bookkeeping seminar
was conducted on August 15 - 16, 2019 with
approximately 30 farmer’s participants
coming from different cooperatives and
associations in Dasol, Pangasinan.

findings for the cooperatives in the
province of La Union and in partnership
with the Provincial Cooperative Union of
La Union (PCU-LU), an orientation was
conducted last November 26, 2019 at the
DMMMSU LA UNION MPC Bldg.,
located at Don Mariano Marcos State
University Ortega St., Catbangen, San
Fernando City, La Union. It was attended
by Twenty-Two (22) Cooperatives with
Eleven (11) male and Thirty (31) Female.
It was funded using the remitted CETF of
the cooperatives approximately Php
26,500.00.

Based on Consolidated 2019 Quarterly Reports (CDA-PL-WFPIS-FR-026), CDA-Dagupan
Extension Office Through field CDS II, inspected Eight Hundred Forty (833) Cooperatives for
the Calendar Year 2019 with a total findings of Seven Thousand Nine Hundred Twenty
One(7,921) wherein micro cooperative contributed the most number of findings. Inspections of

Highlight of 2019 Accomplishment Report
Province of La Union
As of 2019, the Province of La Union has a total number of
152 operating cooperatives. The following activities
highlighted the performance of CDA-La Union Provincial
Office for the year 2019 as follows:
⮚In partnership with the Department of Trade and Industry
(DTI)-La Union Provincial Office and Millionaires
Cooperatives in La Union, the province conducted a two (2)
days orientation seminar on the Reportorial Requirements,
Cooperative Updates, and Tax Incentives Management and
Transparency Act (TIMPTA) Law at the People’s Hall in San Fernando City, La
Union. Consumer Education and Awareness of Fair Trade Laws was also conducted by DTI in
that event. A total of 115 participants coming from various cooperatives in the Province
attended this 2- day’s seminar.
⮚ A Memorandum of Agreement (MOA) was being forged between the LUELCO
Employees MPC (as Big Brother) and Macabato MPC (as Small Brother) under the
Koop Kapatid Program of CDA.
⮚ 21 Pre-Registration seminars were conducted with a total of 901 participants. As a
result of this undertaking, there were 9 cooperatives registered.
⮚ One Satellite office of the La Union Multi-Purpose Cooperative was registered.
⮚ 149 operating cooperatives were inspected.
⮚ As part of the Cooperative Month Celebration, free mandatory seminars for Micro
Cooperatives on Governance and Management of Cooperatives and Fundamentals

of Cooperatives were conducted with 49 participants and 55 participants
respectively.
⮚ An orientation seminar on Gender and Development (GAD) Memorandum Circulars
and GAD tools was conducted in partnership with DTI-La Union Provincial Office (for
the seminar venue) with 35 cooperatives in attendance.
⮚ On Cooperative Meetings conducted:
o CCDC San Fernando City 4 meetings
o PCDC La Union 2 meetings
o MCODO 4 quarterly meetings for CDA updates
and issuances
⮚ The La Union Cooperative Month Celebration opening ceremony was held on
October 21, 2019, with Dir. Josefina B. Bitonio of CDA as the Keynote Speaker.
Activities during the opening ceremony include Mini Trade Fair, free testing on blood
pressure and blood sugar, free massage, and haircut. This was attended by around
250 participants. Other activities during the month-long celebration were livelihood
training on vinegar making, papaya pickles, and dishwashing liquid, and packaging
and labeling. Non-Mandatory Training on Credit Risk Management and Leadership
and Values Formation were also conducted. A total of 300 coops. Members were in
attendance during the culminating activity on November 11, 2019, with RD Bitonio as
the Guest Speaker. Awarding of cash and certificates to the winners of the Search
for Most Outstanding Cooperative in the Provincial level was made.
⮚ The Department of Trade and Industry-La Union Provincial Office had partnered with
CDA La Union Provincial Office for the conduct of Negosyo Serbisyo sa Barangay
(NSB) to the identified 4th to 6th class municipalities in the province of La Union. As
contained in the Guidelines, the CDA is part of the organizational support
infrastructure in providing technical expertise and may serve as resource persons
under the NSB program

Orientation of Mandatory Reports A Total of 27 Management and Entrepreneurial Training
were conducted to the 17 Barangays with a total of 1,867 participants. CDA was able to orient
the participants on the importance of cooperative and the benefits it could provide to small

farmers, fisher folks, and micro-entrepreneurs. Together with TESDA, CDA-La Union was
present to all the 27 training conducted in 2019.

CDA Ilocos Sur Accomplishment Report for the year 2019
A. Pre – Registration Seminar
In line with our work and financial plan we have conducted Pre – Registration Seminar
(PRS) from different municipalities and different sectors from Ilocos Sur. We have the transport
sector; DSWD Pantawawid and Sustainable Livelihood Program beneficiaries and the
Indigenous Peoples
Joint Pre –Registration Seminars conducted in January 2019 to prospective members from
DSWD Pantawid and Sustainable Livelihood program in partnership with DWSD Provincial link
held at ABC Hall, LGU –Caoayan, Ilocos Sur.

B. Capacity Building Program
To equip with knowledge and skills among the cooperative officers, for the 1st quarter
we were able to deliver training on Parliamentary Procedures and Orientation on Mandatory
Reports free of charge thru our linkages with our partners like Nueva Segovia Consortium of
Cooperatives, Ilocos Consolidated Cooperative Bank, and Local Government Unit of Candon by
sponsoring meals and snacks.

in cooperation with City Cooperative Office, LGU Vigan City on February 13, 2019.
b.2. Orientation on Mandatory Reports.
A joint project of the Sponsored by Ilocos Consolidated Cooperative Bank on February
11, 2019, at Ilocos Consolidated Cooperative Bank Branch, Bantay, Ilocos Sur participated by
member cooperatives. Held at Candon City ABC Hall on February 12, 2019, participated by
different cooperatives from Candon City.

Orientation on the Mandatory Reports for Cooperatives and on the availament of Tax Exemption
Privileges on February 27, 2019 @ NSCC Plaza, Caoayan, Ilocos Sur attended by different cooperatives
in Region 1 , with a total of 70 cooperatives composed of 16 males and 54 females.
C. Gender and Development Program
An orientation on MC 2013 -22 Guidelines on Mainstreaming
On Gender and Development (GAD) in Cooperatives.
The Nueva Segovia Consortium of Cooperatives is a GAD advocate cooperative in
Ilocos Sur. During March, they conducted training on GAD Mainstreaming as part of their
celebration of Women’s Month wherein participated by fifty (50) members –cooperatives from
Ilocos Sur, Ilocos Norte, Abra, Pangasinan. Held at NSCC Convention Center, Caoayan, Ilocos
Sur.
D. Youth Congress
The CDA recognizes the valuable contributions of Guardian Cooperatives in the socio-
economic development of minors through the organization of Laboratory Cooperative thus an
orientation is being conducted as per MC. No. 2015 -03 – Guidelines for the Creation,
Organization, Supervision, and Monitoring of Laboratory Cooperative.

E. Koop –Kapatid Program
It is the policy of the State to develop, advance and nurture the cooperative movement
by providing development interventions through partnership building and coordinative efforts
and there is a need to reinforce the strategy in the provision of technical assistance to the
cooperatives which were not able to comply with the mandatory reports required by the CDA

through strengthening partnership with successful cooperatives through the KOOP KAPATID
(Big Brother Small Brother) Program and application of the Internationally Accepted Principle of
Cooperation Among Cooperatives thus a Memorandum of Agreement (MOA) is being forged
between Free Farmers as Big Brother and Cuancabal Multi-Purpose Cooperative as small
brother both from Cabugao, Ilocos Sur.
F. Cooperative Development Council
After long years of inactive of the PCDC Ilocos Sur, the new administration under
the leadership of our young and multi-awarded Governor, Hon. Ryan Luis V. Singson, our

council was reorganized on October 24, 2019, as part of our Provincial Cooperative Month
Celebration.
Ilocos Norte Accomplishments
In Ilocos Norte, government and non-government organizations recognized the
importance of cooperatives as a tool in poverty alleviations and economic development and
doing all possible ways to help and reach out to cooperatives. In recognition of the importance
of cooperatives, the Provincial Government of Ilocos Norte hosted the Cooperative Month
Celebration last October 2019 at the Ilocos Norte Centennial Arena and was attended by
various cooperatives in the province. Activities were trade fairs which showcased the products

of cooperatives, simple program, and discussion of CDA updates by Atty. Philip Rivera and the
highlight is the awarding of most outstanding cooperatives and most outstanding cooperative
officer/coordinator. Cash awards and certificates of recognition were given to the awardees. The
Laoag City Government also in partnership with the Metro Laoag Federation of Cooperatives
hosted Cooperative Month Celebration in the City of Laoag at the covered court of Barangay
Balatong, Laoag City. Activities were trade fairs which showcased cooperative products,
presentation of CDA Updates by Regional Director Josefina Bitonio and CDA Accountant
Enjirico Mondejares, and the highlight which is the awarding of outstanding cooperatives.

Certificates of the awardees were awarded by CDA Director Josefina Bitonio, City Cooperative
Officer Ms. Estela Damo, and Metro Laoag Federation of Cooperatives represented by Ms.
Lolita Miguel.
Ilocos Norte joined the celebration of the International Day of Cooperatives last July 6,
2019, by conducting a Coastal Clean-up in coastal areas of Barangay Lapaz, Laoag City.
Cooperatives in Laoag City joined the activity. Said activity is led by the City Government of
Laoag and the Metro Laoag Federation of Cooperatives.
The government helps in strengthening these cooperatives because doing so would mean
helping the members uplift their living conditions. It is a necessary task that can revitalize
neighborhoods and actually help create wealth within communities that have been shut out of
the rapid economic growth of the area. Technical support services are also extended by the
CDA via a cost-effective support service system that makes use of existing support service
providers (e.g. financial institutions, government agencies, etc.) and many more services that
needed by the cooperatives in the management and operation of their cooperatives. CDA-
personnel in Ilocos Norte despite limited resources managed to extend support to cooperatives
in Ilocos Norte as evidenced by various activities undertaken.
The CDA, as mandated by Republic Act No. 6939, seeks to ensure the safe and sound
operation of cooperatives in order to develop the cooperative sector into a viable and
sustainable economic enterprise, thus the Koop Kapatid was designed. In Ilocos Norte last
2019, the Gledco MPC being Big Brother signed an MOA with its Small Brothers Balatong MPC
and Baggak Ti Daya Development Cooperative. The Gledco MPC provided training assistance
to its small brothers. The training provided is Governance and Management of Cooperatives,
Fundamentals of Cooperatives, and Bookkeeping. The Gledco MPC also extended its
assistance to its Small Brothers in the preparation and submission of mandatory reports.
Financial Consultancy Services also extended by the Gledco to Baggak Ti Daya
Development Cooperative. Gledco had a mentoring with Balatong MPC re cooperative policies
and operation and how to increase savings and capital build-up programs. Gledco MPC also
had a dialogue with the Baggak Ti Daya Development Cooperative re capital build-up programs
and lending policies and collection. The Gledco MPC continuously guiding the cooperatives in
their operation (consultancy)

The year 2019 is a great year as the different municipalities and the Provincial
Government of Ilocos Norte and the National Government Agencies showed their utmost
support to the cooperative sector. The CDA personnel in Ilocos Norte recommended to the
Provincial Government of Ilocos Norte 5 cooperatives (Garnaden MPC of Nueva Era, LBN MPC
of Vintar, Independent Church Aglipayan Association Inc Agriculture Cooperative of Batac City,
Sumlaat San Carlos MPC of Banna, Ilocos Norte and Camanga Upland Farmers MPC of
Badoc, Ilocos Norte) as a recipient of 200,000.00 worth of livelihood assistance (grant). The
Garnaden MPC and LBN MPC were the recipients of the 2018 COOP-SEED Program of the
CDA and DTI. Said grant was awarded last November 15, 2019, at the Provincial Agriculture
Office.
In addressing the needs of the cooperative sector, the government particularly the
Cooperative Development Authority guides the cooperatives in their operation, compliance to
government rules and regulations, and provide skills for them to be knowledgeable in the
operation of their cooperatives because having the proper knowledge will surely make the
cooperative grow.
The CDA conducted training on the Fundamentals of Cooperative, Orientation on the
Preparation on Mandatory Reports, Governance and Mgt of cooperatives, Gender and
Development, and Financial Literacy Seminar Sponsored by the City Govt of Batac on April 10-
12, 2019. 23 participants, 17 micro coops.
The CDA also conducted Orientation on the Preparation of Mandatory Reports in Ilocos
Norte Sponsored by ICCB and MLFC. There were 2 trainings conducted which benefitted 41
cooperatives and 101 officers and members were oriented. Bookkeeping Seminar was also
conducted in Laoag City last November 2019 and Governance and Management of

Cooperatives last Dec. 6, 2019 attended by 48 cooperative officers(19 cooperatives) sponsored
by City Government of Laoag.
The CDA ensures that the cooperative's reports are complete before submission to the
regional office. CDA Regional Director, Dr. Jo Bitonio extended her support to the cooperatives
in Ilocos Norte.
The CDA continuously guides the cooperatives. One assistance is assisting the
cooperatives in their registration in the CBMIS. In Ilocos Norte, there were 5 cooperatives
registered in the CBMIS for 2019. One is the Bacarra Garlic Producers Coop which the
Chairperson Mr. Cariño proudly announced that the CBMIS helped him gained his customers
from outside Ilocos Norte.CBMIS indeed paved the way to market their products since enrolled
last April 2019. Because of the coops enrolment to the CBMIS, the cooperative was able to
reach the customers even outside region 1 and resulted in a regular and successful business
transaction. The CDA ensures that the cooperative's reports are complete before submission to
the regional office. CDA Regional Director, Dr. Jo Bitonio extended her support to the
cooperatives in Ilocos Norte.
In Ilocos Norte, there were 22 newly registered cooperatives for 2019 (8 SLPAs assisted
by DSWD, 3 Agrarian Reform Cooperatives, 3 Indigenous Peoples group, 1 Fishermen Coop, 1
service coop, 2 LGU employees Coops and 4 farmer groups).

  • 6. 6 On the year under review, the Cooperative Development Authority Dagupan Extension Office utilized its manpower and resources to contribute to our organization objectives “Growth and Viability of Cooperative Enterprises Improved” with four major programs namely: Cooperative Development Program (CDP); Cooperative Regulatory Program (CRP); Support to Operations (STO); and General Administration Services (GAS). This report shares some of the details of our activities and highlights the accomplishments that give us the opportunity to realize the breadth of our commitment to our cooperators and our mandate to promote the viability and growth of cooperatives as instruments of equity, social justice, and economic development in fulfilment of the mandate in section 15, Article XII of the Constitution A. Cooperative Development Program The Cooperative Research, Information and Training Unit engaged themselves with extensive activities on handholding of micro and small cooperatives, delivered various technical assistance services, entered a partnership with stakeholders in the conduct of capability building programs for cooperatives in line with the mandatory training for officers, and the conduct of Pre-Registration Seminars. We also conducted the annual search of Gawad Parangal for outstanding primary cooperatives, documented their success stories, co-implemented the conduct of the Cooperative Month Celebration, distributed IEC materials, conducted cooperative taxation education, and koop-balitaan, processed the accreditation renewal of both the ATPs and CEAs and conducted GAD related activities. The overall performance of CRITS was “Very Satisfactory”. The commitment to customer service runs deep within our entire organization, thus, the feedback rating from clients was a “better” rating In supporting the entrepreneurial aspirations of the cooperative, the Cooperative Project Development Assistance Unit, implemented programs like Koop Kapatid Program, encouraged cooperative members to participate in M2C, and assisted the cooperatives to be involved in C2C and C2NC, monitored the contribution of entrepreneur members in job generation and income increases. Support from the national government agencies such as DTI, DAR, DA, NCIP, DOST as well as the LGUs helped in strengthening the position of cooperatives as business enterprises. Additionally, the CPDAS also advocated in the cooperative business process through the value chain, monitored its financial inclusion program, and enrolled cooperatives to CBMIS. The overall performance of CPDAS was “Very Satisfactory”. From the customer's feedback perspective, the rating received from clients was “better”.
  • 7. 7 CDA Resource Persons in Action in Various Capability Building Conducted by CDA-DEO B. Cooperative Regulatory Program One thing that hasn’t wavered is our commitment to register cooperatives. The Registration Unit performed well in registering new cooperatives, as well as the processing of amendments in their ACBL, with registration documents acted upon within the prescribed period based on the citizens charter, issued Certificate of Authority to cooperatives establishing branches, Letter of Authority was issued for approved cooperative satellites and Certificate of Recognition to Laboratory Cooperatives and updating of COOPRIS. Our clients rated the registration process better. The Regulatory Unit continued to monitor the health of cooperatives thru inspection. Reports on inspection were evaluated to determine the gaps in their operations. Cooperatives with complete mandatory reports were processed and evaluated. Compliant cooperatives were issued their COCs while the non-compliant cooperatives were indorsed for appropriate action or interventions. The RU is also responsible for the consolidation of ATIR. All in all the RU was rated as better by clients. On the enforcement activities, the Legal Unit acted on complaints within 15 days upon receipt of reported violations or complaints. Non-compliant cooperatives were given SCOs, rendered various legal opinions, conducted investigations, and well as facilitated the conduct of mediation and conciliation. Moreover, the Legal Officer acted in the recommendation to dissolve non-compliant cooperatives or based on the inspection findings recommendations of the CDS- IIs. The overall performance of this unit was “Very Satisfactory”. C. Support to Operation We also continued to expand the footprint of CDA thru institutional strengthening by way of improved developmental service delivery mechanisms and enhanced members and public protection framework. Planning and meetings sparked a passion and provided perspective for the direction of the Agency. Firstly, STO covered the Agency’s plan preparation, implementation, and monitoring. Secondly, we continued the monitoring of the Philippine Cooperative Medium Term Development Plan.
  • 8. 8 On the other hand, STO covered Quality Management Systems (QMS), thus the Key Performance Indicators (KPIs) were religiously monitored and analyzed coupled with the regular conduct of 5S. To update our QMS team, they also attended training relating to IQA. The office was audited by both the IQA and the surveillance audit. The MIS regularly maintained and updated the EO website, upgraded the internet connectivity, every so often preventive maintenance on desktops and laptops such as the latest OS were installed with the latest antivirus for personnel. Moreover, the MIS posted articles on the website. The General Administration Services was up to date in the budget preparation and execution program with FA reports was prepared on time and in addition, the financial performance of the Agency was assessed during the mid part and year-end of 2019. Noteworthy to mention was the 2019 financial performance in which allocation and obligation was 100%. The Human Resource Development was active in facilitating the preparation of the Individual Development Plan which became the basis for the Agency’s capacity interventions for personnel and conduct of HR development activities. To have an appreciation of quality and fair performance rating, an orientation on SPMS was conducted. The OPCR and IPCR were regularly submitted and assessed by PMS. The HR performed the recruitment and placement process in accordance with CSC rules and Agency regulations. The General Services implemented the motor vehicle maintenance program, building, and equipment maintenance program conducted physical inventories under the property management program. The GS was also responsible for carrying out the disaster preparedness activities in cooperation with the Bureau of Fire Protection. The Records Officer performed the records and document management like sorting and maintenance of records. Moreover, records were classified for retention and disposal. For the cashiering services, revenue generated was remitted on time to the bank and recorded appropriately. A major development is the compliance to ISO 9001:2015 requirements to consistently provide products and services that meet customer and applicable statutory and regulatory requirements and to enhance customer satisfaction through the effective application of the system, including processes for improvement of the system and the
  • 9. 9 assurance of conformity to customer and applicable statutory and regulatory requirements. This is in the heart of our business and is ingrained in our DNA. The CDA conducted employee training on QMS/IQA. Our employees have an increased awareness of the importance of a job well done, and a heightened level of pride and independence. The Agency has engaged in extensive consultations with all its stakeholders including partners from the National Government Agency. As a result, two recognition was given to CDA namely: 1) NCIP - Invaluable contribution to the successful conduct of Regional Action Planning Workshop 2) Regional Competitiveness Committee - Invaluable assistance and exceptional support as Partner Agency. The Villar SIPAG supports and recognizes organizations that help our less fortunate countrymen break free from the clutches of poverty. This is a public service program of Senator's Manuel and Cynthia Villar who have a passionate desire to search for those who have an advocacy in improving the quality of lives of their fellow men. There were two cooperatives from the 40 cooperatives recipients of Villar Sipag Awardees for their contribution to poverty reduction. The awardees were: Albacopa Federation of Cooperatives of Pangasinan and Suyo Multi Purpose Cooperative of Ilocos Sur.
  • 10. 10 Suyo MPC Albacopa Federation of Cooperatives The members of the REXECOM likewise contributed to facilitating the changes needed and creating an effective working environment. They contributed to making management decisions, monitor performance, and make sure everything is done in compliance with the rules and regulations of the CDA and to cater to the needs of the organization. We continue to grow and evolve. As of December 31, 2019, our workforce is 39 for Region 1. An increase of six personnel with two contracts of service (COS). To our stakeholders, thank you for making our work possible and for partnering with us in implementing our programs and projects. We humbly request the continued support and cooperation of our shareholders as we move forward.

BITONIO Director II COOPERATIVE DEVELOPMENT PROGRAM Accomplishment Report from Cooperative Research, Information and Training This consolidated report discloses the annual performance review of CDA Dagupan Extension various units including provincial reports. In addition, The Cooperative Research, Information, and Training Unit have achieved significant milestones on its programs and projects implemented. 1) Handholding and Technical Assistance Service The total number of TAS rendered by the field CDS for the year 2019 is 1,211, where various issues and concerns of the cooperative sector were provided by way of inquiry, training assistance, mentoring, coaching, cliniquing, and consultancy. These TAS were rendered to medium and large coops. These TAS were rendered to medium and large coops. 2) Capability Building Program for Cooperatives The CDA assisted in the conduct of mandatory training for cooperatives such as Fundamental of Cooperatives, Governance and Management, Enhancement Training for Officers and staff, and conduct of Pre Registration Seminar Pre-Registration Seminar (PRS) For Prospective Cooperatives There were one hundred forty-three (143) PRS conducted for the year both in the regional and field level taking into consideration the center and most accessible place for 6,433 prospective members. There was a deluge of requests for PRS on account of those wanting to organize transport co-ops.
  • 12. 12 Mandatory Training for Officers Governance & Management are the two training components identified by our agency which co-op officers should undergo. Fundamentals of Cooperatives and Governance cum GAD Orientation were conducted on April 10-12, 2020. To maximize the free orientation seminar sponsored by the LGU, the participants were also oriented on the Gender and Development (GAD) in Cooperatives as mandated by the agency. CDS II Florentino B. Lorenzo discussed the basic concepts and principles of GAD in the context of oops’ governance. The LGU of San Carlos City, Pangasinan headed by its City Cooperative Development Officer, Arlaine M. De Vera requested for the conduct of the said seminar for the registered cooperatives operating in San Carlos City. It was held in the DTI – Negosyo Center Building. 33 participants attended from the 16 coops who attended the activity on July 25 - 26, 2019. The City of Candon, - Office for the Development of Cooperatives provides technical assistance in organizing new cooperatives and registers them at the Cooperative Development Authority (CDA). It is headed by Ms. Grace L. Ramos, City Cooperative Development Officer the CDO conducted basic mandatory seminars for co-op officers should as required by the agency. A total of 40 participants (16-Male; 24-Female) coming from 17 micro co-ops attended the Fundamentals of Cooperatives and Governance and Management Seminar.on June 21, 2019). The activities were conducted in recognition of the Memorandum of Agreement between CDA - DEO and the LGU – Candon City. The LGU of Batac City thru their City Agriculturist, Ms. Marylime T. Gappi requested for the conduct of the said seminar for the registered micro cooperatives operating in Batac City. It was attended by 38 participants coming from 7 micro cooperatives. Field CDS, Rosievic R. Medina also pitched in as she oriented the participants on the reportorial requirements of the Agency. A
  • 13. 13 total of 71 participants (39-Male; 32-Female) coming from 18 micro co-ops attended the October 9 -10, 2019. The City Government of Vigan has requested for the conduct of the two (2) day seminar for Vigan-based micro cooperatives. The City Cooperative Development Office, headed by Hermie T. Etrata initiated the activity in coordination with CDS II Raymund N. Pilorin. There were 28 officers coming from 12 Vigan-based coops who attended the activity. CDS II Pilorin also pitched in as discussant in the seminar and provided technical assistance incidental to the said activity on October 22 - 23, 2019. The LGU-Bayambang coordinated with the CDA thru District CDS II Sheryl Lou M. Fabia for the conduct of the seminar for the cooperative supported by their local chief executive, Mayor Cezar T. Quiambao whose membership were his classmates in high school. District CDS II Sheryl Lou M. Fabia also assisted in the activity working in close coordination with the LGU – Bayambang. 17 officers attended the seminar on the fundamentals of Cooperatives and Governance on October 29-30, 2019 at Bayambang, Pangasinan. Reportorial Requirements It’s that time of the year to remind and inform registered cooperatives on their submission of mandatory reports and secure the Certificate of Compliance (CoC) with the agency with an end view of increasing the number of compliant cooperatives in the country.

Aside from walking through the participants on how to undertake the online submission of reports, the team also updated the participants regarding the Cooperative Business Matching Information System (CBMIS) platform of the agency and the online payment of CDA Registration fees through the Landbank Link. Bizportal. Enhancement Training for Co-op Officers To capacitate co-op members and officers we have coordinated the conduct of the Entrepreneurship Trainings at San Fernando City, La Union on February 26, 2019. The district activity was undertaken in partnership with the DTI La Union Provincial Office headed by Provincial Director Merlie D. Membrere wherein their resource speakers talked about Consumer Education & Awareness and Fair Trade Laws.

Another Entrepreneurship Training was conducted on August 23, 2019, with the additional topic “HOW TO START YOUR BUSINESS” The cooperatives were given slots by the DTI to join other micro and small business entrepreneurs in the said training. 33 participants were coming from the cooperatives
  • 17. 17 4) Accreditation Program for Training Providers, External Auditors and Med/Com Anticipating the number of accreditations expiring in the year, the EO targeted 5 accreditation applications and/or renewals for ATPs and 7 for CEAs to be an endorsement to the CO. There were 4 EAs who were able to secure their initial and/or renew their accreditation and 3 Training Providers were able to renew for the year. 5) Information and Communication Program Development and Distribution of IEC Materials.   • 18. 18 Tax Forum: A Dialogue for the Uniform Interpretations on the Cooperatives” Application for Certificate of Tax Exemption, Payment of Documentary Stamp, and Local taxes The activity was participated by five (5) representatives from the BIR Regional Office, together with six (6) CDA personnel and three (3) representatives from the Local Government Units of Alaminos City, Pangasinan, and 15 representatives from the cooperative sector. Our host for the dialogue was the CCDC, and CCDO of Alaminos City on February 12, 2019. As a consensus, a position paper shall be submitted by the CDA - DEO to the BIR Central Office concerning the issues and concerns raised and discussed during the dialogue. The forum sought to address the perennial dilemma of cooperatives in their application for Certification of Tax Exemption (CTE). Payment of Documentary Stamp Tax thru a dialogue with the BIR Regional Office. Pertinent tax laws and issuances were presented during the discussion. There was also some jurisprudence presented as a basis for legal interpretation on the treatment of cooperatives’ tax privileges. Dialogue with BIR Compliance of Cooperatives in the Submission of Annual Report Tax Incentive Report according to the DOF-CDA-BIR Joint Administrative Order No. 1 Series of 2019 The Joint Administrative Order JAO) No. 1-2019 (Inclusion Law (TRAIN Law) and CDA Memorandum Circular (MC) No. 2019-06, Series of 2019 requires all types and categories of cooperatives registered with the CDA which were issued with the Certificate of Tax Exemption (CTE) by the BIR to submit a complete Annual Tax Incentives Report of their income-based tax incentives, value-added tax, and duty exemptions, under R.A. No. 9520 as mandated under Section 4, paragraph (2) of R.A. No. 10708 and Section 3 of R.A. No. 10963, otherwise known as the Tax Reform for Acceleration. The activity sought to inform and enable the cooperatives to comply with the said directive. The activity was attended by representatives from the BIR – Last July 6, 2019, Binmaley, Pangasinan is where we conducted coastal clean up to celebrate the worldwide coop celebration of the 25th United Nations International Day of Cooperatives and the 96th International Cooperative day with the theme: “COOPS 4 DECENT

As the 2nd batch of the activity, there were a total of 24 participants coming from 18 cooperative-affiliates of the ICCB operating in the province of Ilocos Norte. They were also oriented on how to prepare their own BCP based on the hazard which they have identified. The activity was a discovery for both the participants and the facilitators on the level of preparedness and contingencies which their cooperatives as business enterprises have already in place before, during, and after the occurrence of the hazard. There was a lot of sharing of experiences and ideas during the presentations of the different BCPs of the participants. However, a major challenge in institutionalizing BCM in the agency is its budget constraints since legislative action is required for budget allocation which consequently limits the ability of the agency to carry-out BCM internally and rolling it out to the stakeholders. Thus, the partnership with the HLA was a good initiative to increase the capacity of the CDA in cascading information to cooperatives that are at risk of hazards that threaten their survival and sustainability. . 7) Conduct of Campaign Activities thru Quad Media The PIA – PANGASINAN is an avid supporter of the CDA in terms of information dissemination and cooperative advocacy. The extension office is regularly invited in its radio program, “Pantongtongan Tayo” (Pag-usapan Natin) aired over DZMQ Radyo Pilipinas – Dagupan on October 1, 2019, with its anchor, Ms.Imeeh Casiño. It is an opportunity for our agency programs and activities to be disseminated and promoted. Mr. Enriquez announced the conduct of the Regional Cooperative Month Celebration on October 18 - 19, 2019 in Alaminos City, Pangasinan in partnership with the RCDC – I and the City Government of Alaminos as represented by Mr. Solomon B. Tablang, City Cooperative Development Officer/RCDC – I Secretary. Ms. Casiño inquired on what the CDA has been doing regarding the proliferation of “pseudo-cooperatives” used by unscrupulous individuals to lure people in investing their hard-earned money. He explained the rationale of the said activity why it is celebrated annually in our country as provided in Proclamation No. 493, S- 2003. He advised the public who are interested to join a cooperative to verify first with the CDA its status whether if it is indeed registered or not operating or dissolved to avoid being swindled.
  • 21. 21 9) Monitoring of Cooperative Development Councils (CDCs). The first regular meeting was attended by representatives from the different National Government Agencies and Local Government Units namely DILG, BFAR, DTI, PGP-SP, NFA, DOLE, LBP, DAR, NEDA, CDA Office of the Provincial Agriculture Office of La Union, PPO-Pangasinan, DENR-EMB and the City Government of Alaminos City, Pangasinan. There were also 14 cooperatives which attended the meeting from the 4 provinces of Region 1. The agenda of the meeting is as follows: a. Accomplishment Report of RCDC I b. Concept Paper in the Conduct of Regional Laboratory Co-op Congress c. Online Payment of CDA registration Fees (LINK.BIZ PORTAL) d. DILG-CDA Joint Memorandum Circular 2019-01Updates on Tax Exemption of Co-ops from the payment of Taxes, Fees and Charges e. Mandatory Reports of Co-ops 8) Gender and Development Program (GAD) In line with the Social Development Program of the La Union MPC and support to the advocacy in Mainstreaming Gender and Development (GAD) in Co- operatives, the coop initiated the conduct of a FREE one (1) day GAD Orientation Seminar for micro and small coops The activity was accomplished in coordination and assistance of the DTI – Provincial Office headed by its Provincial Director, Merlie D. Membrere by providing the venue. 29 participants were coming from various cooperatives operating in the province of La Union which was conducted last November 20, 2019. The training was facilitated by CDS II Florentino B. Lorenzana.
  • 22. 22 The 2nd regular RCDC meeting was conducted on July 11, 2019, Center for Culture and the Arts Candon City, Ilocos Sur The agenda of the meeting are as follows: presentation of R. A. 11032 (Ease of Doing Business Law); presentation of the Philippine Cooperative Development Plan (2018 – 2022); presentation of the CDA 2019 Gawad Parangal Awards and Recognition Program; presentation of CDA Updates and the Credit Surety Fund Cooperatives law. Among the highlights of the meeting is the presentation of a Research Entitled: Development of Performance Models and Capacity-building Framework for Micro and Small Multi-Purpose Cooperatives in Ilocos Norte by Mr. Errol John Valdez, MMSU. The 3rd regular RCDC meeting was held at Capitol Resort Hotel, Lingayen, Pangasinan on September 19, 2019. The agenda was as follows: 2019 Regional Cooperative Month Celebration (RCMC) Planning, Agricultural Programs for Cooperatives, Updates on CDA programs, advocacy and activities: Republic Act No. 11364, otherwise known as the CDA Charter of 2019, Status of the cooperatives’ compliance in the submission of Annual Tax Incentive Report and the Result of the Regional Screening and Board of Judges Validation and Deliberation in the CDA 2019 Gawad Parangal Awards and Recognition Program. The meeting was conducted in cooperation with the Provincial Government of Pangasinan represented by Mr. Ellsworth G. Gonzales, Provincial Population Officer/RCDC –Treasurer. Ms. Renee Faye DG. Cariño discussed the procedures in TIMTA reporting, the filling- out of forms to be submitted to CDA. The Chairperson commented that the council should help the coops in the submission of the said report and it should be proactive. Some of the council member-NGAs also presented their programs in which co-ops may participate Reorganization of the Provincial Cooperative Development Council (PCDC) of Ilocos Sur The collaborative goal of reorganizing the PCDC of Ilocos Sur has finally been realized last Oct 21, 2019, Vigan City, Ilocos Sur thru the coordination and efforts of PCDS Raymund N. The National Tripartite Conference is a regular activity held every two years headed by the LCDOP. The LGU of General Santos City hosted the 9th National Tripartite Conference for Cooperative Development. This year's theme "Kapayapaan, Kaunlaran at Kalikasan sa Diwa ng Kooperatiba" aims to strengthen the cooperatives, the local government units, and the private More than 2,000 delegates attended the said conference that also included a Trade Exhibit showcasing local, regional, and national cooperative products. As the first activity of the council, a Planning Meeting was conducted on the possible activities which the province may consider on the occasion of the country’s Cooperative Month Celebration last October as decreed under Proclamation No. 493, s. 2003Regional Cooperative Month Celebration 10) Regional Cooperative Month Celebration Local Government Units Partnership ( Alaminos City, Candon City and Laoag City)

Among our hosts for these events were: Alaminos City (RCMC), Province of Ilocos Norte (PCMC), and Candon City (CCMC). The 2019 Regional CMC was held in partnership with the LGU of Alaminos City, Pangasinan. As a keynote speaker, our very own Administrator Vidal D. Villanueva III was invited to the celebration. It was a two (2) day event wherein the participants from the region were given the option to have a tour of the Hundred Islands during the first day There were a total of 550 participants. Among the highlights of the activity was the recognition of the Gawad Parangal (GP) regional awardees; presentation of the CDA updates and issuances together with the other NGAs with cooperative development programs and best practices of successful coops in the region 11) Mini Planning Workshop held at CDA La Union Provincial Office on January 22, 2019 The activity sought to cascade to the field personnel this year’s Work & Financial Plan (WFP) of the agency thereby facilitating the submission of their targets by the parameters of the WFP.   • 25. 25 Cooperative Project Development Assistance Section The Cooperative Project Development and Assistance Section (CPDAS) humbly report the following accomplishments for the period January – November 2019 in coordination/collaboration with the CDA-DEO personnel, particularly to the technical staff. Handholding Program for Micro and Small Cooperatives - A total of twenty-nine (29) hand- holding rendered to micro and small cooperatives through mentoring and coaching namely: Orientation on Koop Kapatid program, enrollment to the Cooperative Business Matching Information System, training assistance on bookkeeping, financial & records management, identification of technical assistance & capacity building needed for the Koop Kapatid program, preparation of mandatory reports, online submission of CAPR, requirements for the BMBE Registration and Assistance to Registration Documents of Coops Technical Assistance Services (TAS) – There were eight (8) cooperatives provided technical assistance specifically on the Memorandum of Agreement for the Koop Kapatid Program & implementation of activities and advisory on MC 2019-08. Capability Building Program for Cooperatives Koop Kapatid– Big Brother – Small Brother Concept. This concept has been around for quite a while now. It was credited to a certain Ernest Coulter who founded the Big Brothers Movement in America in 1904. He intended to recruit volunteers who will act as big brothers to young kids and mentoring them as they grow into adulthood. The Co-operative movement had likewise adopted this concept and it is being recommended to the co-operatives under the principle of “Cooperation among Co-operatives”. Recognizing the soundness of this idea, other organizations incorporated it in their respective programs (Bawang, 2018). Under this program, the target is one per province and as of this date, we have accomplished the following: for Training Needs Analysis, seven were conducted broken down into: (2) - La Union which is Tubao Womens MPC & Macabato Baro a Namnama MPC (2) - Ilocos Norte - Balatong MPC and Affiliates of Metro Laoag Federation of Cooperatives (2) - Ilocos Sur - Cuancabal MPC and Darapidap Abadaya Fisherfolks Savings & Credit Coop. La Union –– St. Louis Credit Cooperative as the big brother endorsed to the La Union Provincial Cooperative Union (LU PCU) an accredited training provider of the Authority the small brother (Sitio Malanas Credit Cooperative) to attend the training such as Strategic Planning (4 officers) and Financial Management (4 officers). The training fee was funded by the big brother coop. 2. Ilocos Sur – 2.1 Big Brother – Sto. Domingo Development Cooperative and Small Brother is Sto. Domingo Municipal Employees Savings and Credit Cooperative. Only the part-time bookkeeper of the small brother cooperative was oriented regarding the accounting system of the Big Brother Cooperative. After the orientation, the accountant requested the books of account of the small brother for actual recording in the books of account but unfortunately, according to the bookkeeper of the small brother coop, their records were in the hands of the accountant of another cooperative which they seek helped and until now the source documents were not yet returned to the cooperative. 2.2 Big Brother – Fatima Vigan MPC and Small Brother is Pantay Onion Producers Cooperative. The officers of the Small Brother Cooperative were not able to undergo job training on the possible technical assistance to be provided by the Big Brother Cooperative as stated in the MOA (Bookkeeping; Records keeping, Fundamentals of Cooperative; Cooperative Management and Governance and Rules Formulation) for the following reasons: 1. Some officers were on training in Manila on Good Agricultural Practice and others were busy on the farm for the production of white Onion. They promised that the technical assistance to be provided by Big Brother Cooperative will be implemented this year. 3. Pangasinan – Big brother – Calasiao Plant & Related Companies Employees MPC and Small brother is Alyansa, Bayanihan at Kapatiran Central Pangasinan Agriculture Cooperative. The identified training was Bookkeeping, Fundamental of Cooperatives, Leadership & Values re-orientation, Strategic Planning & Rules Formulation.
  • 28. 28 The scheduled activities of the Big Brother and Small Brother program were not implemented last year despite the willingness of the big brother cooperative to share their experiences and learnings for the reason that the officers and staff of the small brother coop cannot decide as to when the trainings are conducted because of their schedules. However, the two cooperatives both agreed that they will implement the identified activities this 4. Ilocos Norte – Big Brother is San Joaquin MPC and Small Brother is San Leandro MPC The bookkeeper of the San Leandro is having her bookkeeping training at the San Joaquin MPC since November 2018 (every Saturday). For 2019, the following coops were also monitored: 1. Gledco MPC, big brother of Balatong MPC &BaggakTiDaya Development Cooperative conducted a dialogue & discussed issues and concerns on capital build up programs and lending policies & collection. 2. Metro Paypayad MPC, big brother to Darapidap Abadaya Fisherfolks Savings and Credit Cooperative provided the following assistance such as review of policies, cooperative forms, loan processes, conduct of board meeting & bookkeeping process. 3. Free Farmers MPC, big brother of Cuancabal MPC, conducted the mandatory trainings namely Fundamentals of Cooperatives & Cooperative Management & Governance. 4. United Christians MPC, big brother provided assistance on bookkeeping to Lewesen Mt. Range Agro Forest Soft Broom Producers Cooperative. IV. Coop – Sustainable Enterprise Development (COOP-SEED) The COOP-SEED Program was extended until the first Quarter of this year to achieve some deliverables that were not accomplished last year. An assessment at the national level was conducted on March 12-13, 2019 at Quezon City. Among the deliverables of the program that was accomplished until the 1st Quarter this year were the following: 1. Four (4) entrepreneur-members registered under BMBE (Garnaden MPC); 2. Reported the two coop beneficiaries in terms of contribution of entrepreneur member in job generation and increase in income profile; 3. (1) M2C marketing transactions of the two coop beneficiaries were also documented & reported; 4. (2) C2C marketing transactions were documented and reported; 5. (9) C2NC marketing transactions were documented and reported. V. Strengthening partnership with NGAs, LGUs, academe and other stakeholders 1. Department of Trade & Industry (DTI) 30 cooperatives (P=17; LU=3; IS=3; IN=7) were referred to avail the programs and services and 40 coops (P-30; LU=3; IS=4; IN=3) were
  • 29. 29 monitored which availed the programs of DTI such as training, financing forum, BMBE registration, shared service facility, P3 Program, and trade fairs and Mentor Me on Wheels. For the business matching trade fairs facilitated, 2 from Pangasinan were accomplished (1 in Alaminos City and 1 in Umingan), 1 Regional (during the Regional Cooperative Month Celebration), and 1 in the participation of coops to Rima Ti Arianna Trade Fair at Robinson’s Mall, Manila. 2. National Commission on Indigenous People (NCIP) – fifteen (15) technical assistance were provided to IP’s group (P=6; IS=6; IN=3). Technical assistance rendered were: the conduct of PRS, orientation on Koop Kapatid program, the encoding of CAPR/AFS, Mandatory reports, preparation of ACBL, and bookkeeping. 3. Local Government Units - a total of 52 activities coordinated/conducted such as training, participation in various celebrations, and other coop activities. (P=29; LU=10; IS=7; IN=6). VI. Strengthening of Cooperatives Business Enterprise – an orientation on Financial Inclusion was conducted as one of the topics during the Regional Cooperative Month Celebration held at Alaminos City, Pangasinan on October 19, 2019, with a representative from the Bangko Sentral ng Pilipinas as resource speaker.

Specifically titled “An Act Providing for the Creation and Organization of Credit Surety Fund Cooperatives to Manage and Administer Credit Surety Funds to Enhance the Accessibility of Micro, Small and Medium Enterprises, Cooperatives and Nongovernment Organizations to the Credit Facility of Banks and for Other Purposes”, the new law institutionalizes the Credit Surety Fund (CSF) Program

ARCIPE Sr. CDS Cooperative Regulatory Program Registration Unit One of the primary functions of the Cooperative Development Authority is the registration of cooperatives including amendments thereto. In line with this function are the following: I. Issuance of Cooperative Name Reservation Notice (CNRN) • Submission of Cooperative Name Reservation Report Form (CNRRF) shall be submitted either to CDA Central Office or Regional Offices nationwide through personal, postal mail, courier, electronic mail, or online.
  • 41. 41 • The applicant has the option to choose from the following period to reserve their proposed names with the corresponding reservation fees a. Thirty (30) calendar days - Php 100.00 b. Sixty (60) calendar days - Php 200.00 c. Ninety (90) calendar days - Php 300.00 • Cooperative name shall be under the prescribed guidelines. • All reserved names shall be valid per the period specified in the Cooperative Name Reservation Notice. II. Issuance of Certificate of Registration (COR) The prospective cooperative acquires its juridical personality upon issuance of a Certificate of Registration. III. Issuance of Certificate of amendments This is being issued in case of any amendments in the Articles of Cooperation and by laws of the Cooperative lV. Issuance of Certificate of Authority (COA) A Certificate of Authority shall be issued by CDA to cooperatives applying for the establishment of a branch office upon compliance with the requirements. A Branch Office refers to a business office outside the principal office where cooperative activities and business operations are undertaken as per the approved cooperative development plan. V. Issuance of Letter of Authority (LOA) A Letter of Authority shall be issued by CDA to cooperatives applying for the establishment of a satellite office upon compliance with the requirements. Satellite Office refers to an office established by a cooperative outside of its principal/main office but within its area of operation to provide limited services to its members but which does not however maintain books of accounts as it is done only by the principal/main office. Vl. Issuance of Certificate of Recognition (CoR) Certificate of Recognition shall be issued for the organization of laboratory cooperative for minors (ages below 18 years old) by a Guardian Cooperative. VlI. Issuance of Certificate of Merger or Consolidation This refers to the process where two or more cooperatives become a single cooperative, which, in the case of a merger, there is a surviving cooperative and consolidated cooperative in the case of consolidation.

The law provides its powers, functions, and responsibilities; the governing body; budget and funds; and rulemaking power, among others. Special Order to conduct an inspection is based on the submitted individual inspection plan of CDS IIs and Team Inspectorate. Large cooperatives are being inspected by the Team Inspectorate. This activity is conducted to be able to determine the proper technical assistance or intervention that could be extended to cooperatives. Out of Eight Hundred Nineteen (819) cooperative targeted for inspection for the current year, Eight Hundred Thirty-Three (833) is accomplished which is equivalent to One Hundred Two Percent (102%). . No. of cooperatives examined with inspection report transmitted to cooperative The examination of cooperatives conducted for the year 2019 is Six (6) which is One hundred percent (100%) of the target. Five (5) were provided with technical assistance while the other . No. of inspection reports evaluated. All types of cooperatives inspected with reports submitted are for evaluation so that the findings would be analyzed per category. Eight Hundred Thirty-Three (833) has been received from the inspectors and the percentage of accomplishment on the inspection report evaluated is One Hundred one percent (101%) of the received reports. No. of non-compliant cooperatives recommended / endorsed for appropriate action or intervention Cooperatives issued with show-cause status for the year 2018 which have already complied with the mandatory requirements have been recommended and endorsed to the Legal Section for the lifting of the Show Cause Order. Based on the findings of CDA field personnel, inspected cooperatives that are non-

Three Hundred Three (303) cooperatives have been endorsed for appropriate legal action or intervention. The accomplishment is 303%. No. of cooperatives issued notice of non- compliance on mandatory reports Another way to increase collection of the required reports is to constantly remind those cooperatives which are neglectful with their obligation to the Authority particularly on the submission of the required mandatory reports. In connection to this, Three Hundred Twelve (312) notices of non-compliance to mandatory reports have been sent to cooperatives. The accomplishment is Two Hundred Eight Percent (208%) of the total target which is One Hundred Fifty (150). No. of cooperatives issued with Certificate of Compliance The cooperatives which have complied in the submission of mandatory reports with complete attachments and including the newly registered which have secured the Certificate of Compliance is totalled to Eight Hundred Seventy-One (871) which is One Hundred Twenty Three Percent (123%) of the target for the year. No. of cooperatives complying with Joint Administrative Order on RA No. 10708 or (TIMTA Law) Out of Seven Hundred Fifty-Nine (759) cooperatives targeted only five Hundred Eighty-Six (586) cooperatives have submitted reports and complied with the Joint Administrative Order on RA No. 10708 or TIMTA Law.   • 48. 48 Bookkeeping Seminar In partnership with the Local Government Unit (LGU) Bayambang, a bookkeeping seminar was conducted on July 18 - 19, 2019 with approximately 30 participants coming from different cooperatives in Pangasinan. In partnership with Department of Agriculture of LGU Dasol, another bookkeeping seminar was conducted on August 15 - 16, 2019 with approximately 30 farmer’s participants coming from different cooperatives and associations in Dasol, Pangasinan. As one of the CDA programs, a bookkeeping seminar was conducted on October 3, 2019 at Knowledge CDA, 3 rd Floor, Siapno Building, Perez Boulevard, Dagupan City with approximately 50 participants coming from different cooperatives in Region I. Internal Control Seminar In partnership with National Tobacco Association, an internal control seminar was conducted on October 16-18, 2019 at Arte Luna, Paoay, Ilocos Norte with almost 40 tobacco famers participants coming from different cooperatives and association in Region I. Inspection Findings and CDA Updates On how to comply with the inspection findings for the cooperatives in the province of La Union and in partnership with the Provincial Cooperative Union of La Union (PCU-LU), an orientation was conducted last November 26, 2019 at the DMMMSU LA UNION MPC Bldg., located at Don Mariano Marcos State University Ortega St., Catbangen, San Fernando City, La Union. It was attended by Twenty-Two (22) Cooperatives with Eleven (11) male and Thirty (31) Female. It was funded using the remitted CETF of the cooperatives approximately Php 26,500.00.

RUEDAS PCDS PANGASINAN Highlight of 2019 Accomplishment Report Province of La Union As of 2019, the Province of La Union has a total number of 152 operating cooperatives. The following activities highlighted the performance of CDA-La Union Provincial Office for the year 2019 as follows: ⮚In partnership with the Department of Trade and Industry (DTI)-La Union Provincial Office and Millionaires Cooperatives in La Union, the province conducted a two (2) days orientation seminar on the Reportorial Requirements, Cooperative Updates, and Tax Incentives Management and Transparency Act (TIMPTA) Law at the People’s Hall in San Fernando City, La Union. Consumer Education and Awareness of Fair Trade Laws was also conducted by DTI in that event. A total of 115 participants coming from various cooperatives in the Province attended this 2- day’s seminar. ⮚ A Memorandum of Agreement (MOA) was being forged between the LUELCO Employees MPC (as Big Brother) and Macabato MPC (as Small Brother) under the Koop Kapatid Program of CDA. ⮚ 21 Pre-Registration seminars were conducted with a total of 901 participants. As a result of this undertaking, there were 9 cooperatives registered. ⮚ One Satellite office of the La Union Multi-Purpose Cooperative was registered. ⮚ 149 operating cooperatives were inspected. ⮚ As part of the Cooperative Month Celebration, free mandatory seminars for Micro Cooperatives on Governance and Management of Cooperatives and Fundamentals
  • 66. 66 of Cooperatives were conducted with 49 participants and 55 participants respectively. ⮚ An orientation seminar on Gender and Development (GAD) Memorandum Circulars and GAD tools was conducted in partnership with DTI-La Union Provincial Office (for the seminar venue) with 35 cooperatives in attendance. ⮚ On Cooperative Meetings conducted: o CCDC San Fernando City 4 meetings o PCDC La Union 2 meetings o MCODO 4 quarterly meetings for CDA updates and issuances ⮚ The La Union Cooperative Month Celebration opening ceremony was held on October 21, 2019, with Dir. Josefina B. Bitonio of CDA as the Keynote Speaker. Activities during the opening ceremony include Mini Trade Fair, free testing on blood pressure and blood sugar, free massage, and haircut. This was attended by around 250 participants. Other activities during the month-long celebration were livelihood training on vinegar making, papaya pickles, and dishwashing liquid, and packaging and labeling. Non-Mandatory Training on Credit Risk Management and Leadership and Values Formation were also conducted. A total of 300 coops. Members were in attendance during the culminating activity on November 11, 2019, with RD Bitonio as the Guest Speaker. Awarding of cash and certificates to the winners of the Search for Most Outstanding Cooperative in the Provincial level was made. ⮚ The Department of Trade and Industry-La Union Provincial Office had partnered with CDA La Union Provincial Office for the conduct of Negosyo Serbisyo sa Barangay (NSB) to the identified 4th to 6th class municipalities in the province of La Union. We have the transport sector; DSWD Pantawawid and Sustainable Livelihood Program beneficiaries and the Indigenous Peoples Joint Pre –Registration Seminars conducted in January 2019 to prospective members from DSWD Pantawid and Sustainable Livelihood program in partnership with DWSD Provincial link held at ABC Hall, LGU –Caoayan, Ilocos Sur.

C. Gender and Development Program An orientation on MC 2013 -22 Guidelines on Mainstreaming On Gender and Development (GAD) in Cooperatives. The Nueva Segovia Consortium of Cooperatives is a GAD advocate cooperative in Ilocos Sur. During March, they conducted training on GAD Mainstreaming as part of their celebration of Women’s Month wherein participated by fifty (50) members –cooperatives from Ilocos Sur, Ilocos Norte, Abra, Pangasinan. Youth Congress The CDA recognizes the valuable contributions of Guardian Cooperatives in the socio- economic development of minors through the organization of Laboratory Cooperative thus an orientation is being conducted as per MC. No. 2015 -03 – Guidelines for the Creation, Organization, Supervision, and Monitoring of Laboratory Cooperative.

Koop –Kapatid Program It is the policy of the State to develop, advance and nurture the cooperative movement by providing development interventions through partnership building and coordinative efforts and there is a need to reinforce the strategy in the provision of technical assistance to the cooperatives which were not able to comply with the mandatory reports required by the CDA Photos after MOA signing through strengthening partnership with successful cooperatives through the KOOP KAPATID (Big Brother Small Brother) Program and application of the Internationally Accepted Principle of Cooperation Among Cooperatives thus a Memorandum of Agreement (MOA) is being forged between Free Farmers as Big Brother and Cuancabal Multi-Purpose Cooperative as small brother both from Cabugao, Ilocos Sur. Ilocos Norte Accomplishments In Ilocos Norte, government and non-government organizations recognized the importance of cooperatives as a tool in poverty alleviations and economic development and doing all possible ways to help and reach out to cooperatives. In recognition of the importance of cooperatives, the Provincial Government of Ilocos Norte hosted the Cooperative Month Celebration last October 2019 at the Ilocos Norte Centennial Arena and was attended by various cooperatives in the province. Activities were trade fairs which showcased cooperative products, presentation of CDA Updates by Regional Director Josefina Bitonio and CDA Accountant Enjirico Mondejares, and the highlight which is the awarding of outstanding cooperatives.   • 73. 73 Certificates of the awardees were awarded by CDA Director Josefina Bitonio, City Cooperative Officer Ms. Estela Damo, and Metro Laoag Federation of Cooperatives represented by Ms. Lolita Miguel. Ilocos Norte joined the celebration of the International Day of Cooperatives last July 6, 2019, by conducting a Coastal Clean-up in coastal areas of Barangay Lapaz, Laoag City. Cooperatives in Laoag City joined the activity. Said activity is led by the City Government of Laoag and the Metro Laoag Federation of Cooperatives. The government helps in strengthening these cooperatives because doing so would mean helping the members uplift their living conditions. It is a necessary task that can revitalize neighborhoods and actually help create wealth within communities that have been shut out of the rapid economic growth of the area. Technical support services are also extended by the CDA via a cost-effective support service system that makes use of existing support service providers (e.g. financial institutions, government agencies, etc.) and many more services that needed by the cooperatives in the management and operation of their cooperatives. CDA- personnel in Ilocos Norte despite limited resources managed to extend support to cooperatives in Ilocos Norte as evidenced by various activities undertaken. The CDA, as mandated by Republic Act No. 6939, seeks to ensure the safe and sound operation of cooperatives in order to develop the cooperative sector into a viable and sustainable economic enterprise, thus the Koop Kapatid was designed. In Ilocos Norte last 2019, the Gledco MPC being Big Brother signed an MOA with its Small Brothers Balatong MPC and Baggak Ti Daya Development Cooperative. The Gledco MPC provided training assistance to its small brothers. The training provided is Governance and Management of Cooperatives, Fundamentals of Cooperatives, and Bookkeeping. The Gledco MPC also extended its assistance to its Small Brothers in the preparation and submission of mandatory reports. Financial Consultancy Services also extended by the Gledco to Baggak Ti Daya Development Cooperative. Gledco had a mentoring with Balatong MPC re cooperative policies and operation and how to increase savings and capital build-up programs. Gledco MPC also had a dialogue with the Baggak Ti Daya Development Cooperative re capital build-up programs and lending policies and collection.   • 74. 74 The year 2019 is a great year as the different municipalities and the Provincial Government of Ilocos Norte and the National Government Agencies showed their utmost support to the cooperative sector. The CDA personnel in Ilocos Norte recommended to the Provincial Government of Ilocos Norte 5 cooperatives (Garnaden MPC of Nueva Era, LBN MPC of Vintar, Independent Church Aglipayan Association Inc Agriculture Cooperative of Batac City, Sumlaat San Carlos MPC of Banna, Ilocos Norte and Camanga Upland Farmers MPC of Badoc, Ilocos Norte) as a recipient of 200,000.00 worth of livelihood assistance (grant). The Garnaden MPC and LBN MPC were the recipients of the 2018 COOP-SEED Program of the CDA and DTI. Said grant was awarded last November 15, 2019, at the Provincial Agriculture Office. In addressing the needs of the cooperative sector, the government particularly the Cooperative Development Authority guides the cooperatives in their operation, compliance to government rules and regulations, and provide skills for them to be knowledgeable in the operation of their cooperatives because having the proper knowledge will surely make the cooperative grow. The CDA conducted training on the Fundamentals of Cooperative, Orientation on the Preparation on Mandatory Reports, Governance and Mgt of cooperatives, Gender and Development, and Financial Literacy Seminar Sponsored by the City Govt of Batac on April 10- 12, 2019. 23 participants, 17 micro coops. The CDA also conducted Orientation on the Preparation of Mandatory Reports in Ilocos Norte Sponsored by ICCB and MLFC. There were 2 trainings conducted which benefitted 41 cooperatives and 101 officers and members were oriented. The CDA ensures that the cooperative's reports are complete before submission to the regional office. CDA Regional Director, Dr. Jo Bitonio extended her support to the cooperatives in Ilocos Norte. The CDA continuously guides the cooperatives. One assistance is assisting the cooperatives in their registration in the CBMIS. In Ilocos Norte, there were 5 cooperatives registered in the CBMIS for 2019. One is the Bacarra Garlic Producers Coop which the Chairperson Mr. Cariño proudly announced that the CBMIS helped him gained his customers from outside Ilocos Norte.CBMIS indeed paved the way to market their products since enrolled last April 2019. Because of the coops enrolment to the CBMIS, the cooperative was able to reach the customers even outside region 1 and resulted in a regular and successful business transaction. The CDA ensures that the cooperative's reports are complete before submission to the regional office. CDA Regional Director, Dr. Jo Bitonio extended her support to the cooperatives in Ilocos Norte. In Ilocos Norte, there were 22 newly registered cooperatives for 2019 (8 SLPAs assisted by DSWD, 3 Agrarian Reform Cooperatives, 3 Indigenous Peoples group, 1 Fishermen Coop, 1 service coop, 2 LGU employees Coops and 4 farmer groups).
