Levalbuterol Sulphate Inhaler) is used in the treatment of
chronic obstructive pulmonary disease (bronchitis and
emphysema) when there is evidence of spasm
(narrowing) of the airways (bronchi).
These two medicines work together to help open the
airways in the lungs. Ipratropium Bromide and Albuterol
Sulfate is used to help treat airway narrowing
(bronchospasm) that happens with chronic obstructive
pulmonary disease (COPD) in adult patients who need to
use more than one bronchodilator medicine.

Uses of Duolin Inhaler
Ipratropium Bromide and Levalbuterol Tartarate combination Inhaler is
used to help control the symptoms of lung diseases, such as asthma,
chronic bronchitis, and emphysema. It is also used to treat air flow
blockage and prevent the worsening of chronic obstructive pulmonary
disease (COPD) in patients who need another medicine.
Ipratropium and Levalbuterol belong to the family of medicines known as

Uses of Duolin Inhaler
Duolin Inhaler is a combination of Ipratropium Bromide and Levalbuterol
Tartarate.
Ipratropium bromide is an anticholinergic (parasympatholytic) agent,
which inhibits vagally-mediated reflexes by antagonizing the action of
acetylcholine, the transmitter agent released from the vagus nerve.
Levosalbutamol tartarate is a relatively selective beta2-adrenergic agonist,
whose activation leads to an increase in intracellular adenyl cyclase, the
enzyme which catalyses the conversion of adenosine triphosphate (ATP) to
cyclic-3', 5'- adenosine monophosphate (cAMP).

Duolin Inhaler Dosage
The dosage of Duolin Inhaler (Generic Ipratropium Bromide and
Levalbuterol Tartarate Inhaler) is given below:
The dosage of Duolin Inhaler is 2 inhalations, four times a day. Patients
may take additional inhalations as required; however, the total number
of inhalations should not exceed 12 in 24 hours. It is recommended to
“test-spray” three times before using for the first time and in cases where
the aerosol has not been used for more than 24 hours. Avoid spraying
into eyes.
Duolin Inhaler may be used with a Zerostat VT Spacer device in patients
who find it difficult to synchronize aerosol actuation with inspiration of
breath.

Duolin Inhaler Contraindications
Duolin Inhaler is contraindicated in patients with a
hypersensitivity to Ipratropium Bromide, Levalbuterol Tartarate
(Levosalbutamol Tartarate) or any other component of this
medication. Reactions have included urticaria, angioedema,
rash, bronchospasm, anaphylaxis, and oropharyngeal edema.
Ipratropium Bromide is contraindicated in patients who are
hypersensitive to Atropine or its derivatives.
Duolin Inhaler is contraindicated in patients with a history of
hypersensitivity to soya lecithin or related food products such as
soybean and peanut.

Storage Instructions
Duolin Inhaler (Generic Ipratropium Bromide

Warnings
Paradoxical Bronchospasm: Duolin Inhaler could produce paradoxical
bronchospasm that can be life-threatening. In case it occurs, this
medicine should be stopped immediately and alternative therapy
instituted.
Cardiovascular Effect: The Levalbuterol Tartarate (Levosalbutamol
Tartarate) contained in Duolin Inhaler, like other beta-adrenergic
agonists, can produce a clinically significant cardiovascular effect in some
patients, as measured by pulse rate, blood pressure and/or symptoms. In
case any of these symptoms occur, discontinuation of the medication may
be indicated.
Do Not Exceed Recommended Dose: Fatalities have been reported in
association with excessive use of inhaled sympathomimetic medicines, in
patients with asthma.

Warnings
Immediate Hypersensitivity Reactions: Immediate hypersensitivity
reactions may occur after administration of Ipratropium Bromide or

urticaria, angioedema, rash, bronchospasm, anaphylaxis, and
oropharyngeal edema. If such a reaction occurs, treatment with Duolin

Duolin Inhaler Side Effects
Additional side effects, reported in less than two percent of the patients in the
Duolin Inhaler (Ipratropium Bromide and Levalbuterol Tartarate Inhaler)
treatment group include edema, fatigue, hypertension, dizziness, nervousness,
paresthesia, tremor, dysphonia, insomnia, diarrhea, dry mouth, dyspepsia,
vomiting, arrhythmia, palpitation, tachycardia, arthralgia, angina, increased
sputum, taste perversion, and urinary tract infection/dysuria.
Allergic-type reactions such as skin reactions including rash, pruritus, and
urticaria (including giant urticaria), angioedema including that of tongue, lips
and face, laryngospasm and anaphylactic reaction have been reported with
Ipratropium Bromide And Albuterol Sulfate Inhaler, with positive rechallenge in
some cases. Many of these patients had a history of allergies to other medicines
and/or foods including soybean.

Overdose
The effects of overdosage are expected to be related primarily to
Levalbuterol Tartarate. Acute overdosage with Ipratropium Bromide by
inhalation is unlikely since Ipratropium Bromide is not well absorbed
systemically after aerosol or oral administration.
Symptoms of overdosage with albuterol may include seizures, anginal
pain, hypertension or hypotension, hypokalemia, tachycardia with rates
up to 200 beats per minute, metabolic acidosis, arrhythmias, nervousness,
headache, tremor, dry mouth, palpitation, nausea, dizziness, fatigue,
malaise, and sleeplessness and exaggeration of the pharmacologic effects
listed in side effects. As with all sympathomimetic aerosol medications,
cardiac arrest and even death may be associated with abuse of
Levosalbutamol.

Overdose
Treatment consists of discontinuation of Duolin Inhaler (Ipratropium
Bromide and Levalbuterol Tartarate Inhaler) together with appropriate
symptomatic therapy. The judicious use of a cardioselective beta-
receptor blocker may be considered, bearing in mind that such
medication can produce bronchospasm.
There is insufficient evidence to determine if dialysis is beneficial in
treating overdosage of Levosalbutamol.

Duolin Inhaler During Pregnancy
Ipratropium Bromide and Levalbuterol Tartarate Inhaler (Duolin Inhaler)
has been classified by the US FDA as Pregnancy Category C.
There are no adequate and well-controlled studies of Ipratropium Bromide and
Levalbuterol Tartarate Inhaler (Duolin Inhaler), Ipratropium Bromide Or
Albuterol Sulfate, in pregnant women. Animal reproduction studies have not
been conducted with this medicine. However, Albuterol Sulfate has been shown
to be teratogenic in mice and rabbits.
Duolin Inhaler (Ipratropium Bromide and Levalbuterol Tartarate Inhaler) can
be used during pregnancy only if the potential benefit justifies the potential risk
to the fetus.
Because of the potential for beta-agonist interference with uterine contractility,
use of Duolin Inhaler for the treatment of COPD during labor should be restricted
to those patients in whom the benefits clearly outweigh the risk.
