Seroflo (Fluticasone Propionate and Salmeterol Xinafoate
Powder for Inhalation) a combination product containing a
corticosteroid and a long-acting beta2-adrenergic agonist, is used
in the regular treatment of asthma in patients aged 4 years and
older, where use of a combination medication (long-acting beta2-
agonist and inhaled corticosteroid) has been found to be
appropriate, as well as a maintenance treatment of airflow
obstruction and reducing exacerbations in patients with severe
chronic obstructive pulmonary disease (COPD).
This medicine should not be used for the relief of acute
bronchospasm.

Active Ingredients
The active ingredients present in Seroflo Rotacaps are Fluticasone
Propionate and Salmeterol .
Each SEROFLO 100 Rotacap contains: Salmeterol (As Salmeterol
Xinafoate IP) 50 mcg and Fluticasone Propionate IP 100 mcg
Each SEROFLO 250 Rotacap contains: Salmeterol (As Salmeterol
Xinafoate IP) 50 mcg and Fluticasone Propionate IP 250 mcg and
each SEROFLO 500 Rotacap contains: Salmeterol (As Salmeterol
Xinafoate IP) 50 mcg and Fluticasone Propionate IP 500 mcg.

Seroflo Rotacaps are manufactured by Cipla Limited, India and are

Fluticasone Propionate and Salmeterol 50 mcg) and Seroflo 500

Uses of Seroflo Rotacaps
Seroflo (Fluticasone Propionate and Salmeterol Xinafoate
Inhalation Powder) is a combination of two medicines that are used
to help control the symptoms of asthma and improve breathing.
It is used when a patient's asthma has not been controlled
sufficiently on other asthma medicines, or when a patient's
condition is so severe that more than one medicine is needed every
day.
This medicine will not relieve an asthma attack that has already
started.

Seroflo Rotacaps Dosage
The starting dosage is based on severity of the condition under
treatment. The dosage of Seroflo Rotacaps (Generic Fluticasone
Propionate and Salmeterol Xinafoate Inhalation Powder) is given
below:
Asthma Treatment In Adults and Adolescents (12 years and older):
SEROFLO-250 Rotacaps: One rotacap twice daily or SEROFLO-500 Rotacaps
One rotacap twice daily depending on severity.
COPD Treatment In Adults:
SEROFLO-250 Rotacaps: One rotacap twice daily or SEROFLO-500
Rotacaps: One rotacap twice daily depending on severity.

Contraindications
Seroflo Rotacaps are contraindicated in persons with a
hypersensitivity (allergy) to Fluticasone Propionate, Salmeterol
Xinafoate or any of the other ingredients of this medicine.
Generic Fluticasone Propionate and Salmeterol Xinafoate
Inhalation Powder is not indicated for patients whose asthma
can be managed by inhaled corticosteroids with occasional use of
inhaled short-acting beta2-agonists. This medicine should not be
used for the relief of acute bronchospasm.

Storage Instructions
Seroflo Rotacaps (Generic Fluticasone Propionate and
Salmeterol Xinafoate Inhalation Powder) have to be
stored at controlled room temperature i.e. from 20°C to
25°C (68°F to 77°F) with excursions permitted between
15° to 30°C (59° to 86°F).
Keep this as well as all other medicines away from
children and pets.

Warnings
Long-acting beta2-adrenergic agonists, such as Salmeterol, one of the active
ingredients in Seroflo Rotacaps, may increase the risk of asthma-related
death.
When treating patients with asthma, only prescribe Seroflo Rotacaps for
patients not adequately controlled on other asthma-controller medications
or whose disease severity clearly requires initiation of treatment with 2
maintenance therapies.
Do not start treatment in acutely deteriorating asthma or to treat acute
symptoms.
Do not use in combination with additional long-acting beta2-agonist
because of risk of overdose.
Do not use Seroflo Rotacaps for patients whose asthma is adequately
controlled on low- or medium-dose inhaled corticosteroids

Warnings
Candida albicans infection of the mouth and throat may occur. Monitor patients
periodically for signs of adverse effects on the oral cavity. Doctors must advise
patients to rinse the mouth following inhalation.
Pneumonia: Increased risk in patients with COPD. Monitor patients for signs and
symptoms of pneumonia.
Immunosuppression: Potential worsening of infections (e.g., existing tuberculosis,
fungal, bacterial, viral, or parasitic infection; or ocular herpes simplex). Use with
caution in patients with these infections. More serious or even fatal course of
chickenpox or measles can occur in susceptible patients.
There exists a risk of impaired adrenal function when transferring from oral
steroids. Taper patients slowly from systemic corticosteroids if transferring to
Seroflo Rotacaps.
Hypercorticism and adrenal suppression: There is a possibility to occur with very
high dosages or at the regular dosage in susceptible individuals. In case such
changes occur, discontinue Seroflo Rotacaps slowly.

Warnings
Paradoxical bronchospasm: Discontinue Seroflo Rotacaps and institute
alternative therapy if paradoxical bronchospasm occurs.
Patients with cardiovascular or central nervous system disorders: Use with
caution because of beta-adrenergic stimulation.
Decreases in bone mineral density: Assess bone mineral density initially and
periodically thereafter.
Effects on growth: Monitor growth of pediatric patients.
Glaucoma and cataracts: Close monitoring is warranted.
Metabolic effects: Be alert to eosinophilic conditions, hypokalemia, and
hyperglycemia. .
Coexisting conditions: Use with caution in patients with convulsive disorders,
thyrotoxicosis, diabetes mellitus, and ketoacidosis.

Seroflo Rotacaps Side Effects
Most common side effects of Fluticasone Propionate and
Salmeterol Xinafoate Inhalation Powder (Seroflo Rotacaps) with
an incidence rate of 3% or more observed during treatment of:
Asthma: upper respiratory tract infection or inflammation,
pharyngitis, dysphonia, oral candidiasis, bronchitis, cough,
headaches, nausea and vomiting.
COPD: pneumonia, oral candidiasis, throat irritation, dysphonia,
viral respiratory infections, headaches, musculoskeletal pain.

Overdose
The overdosage symptoms of Seroflo Rotacaps (Fluticasone Propionate and
Salmeterol Powder for Inhalation) are given below:
Fluticasone Propionate: Signs/symptoms of hypercorticism are likely to be
observed in case of Chronic overdosage with Fluticasone Propionate
Salmeterol: The observed signs and symptoms with overdosage of Salmeterol are
those of excessive beta-adrenergic stimulation and/or occurrence or exaggeration
of any of the following: seizures, angina, hypertension or hypotension,
tachycardia with rates up to 200 beats/min, arrhythmias, nervousness, headache,
tremor, muscle cramps, dry mouth, palpitation, nausea, dizziness, fatigue,
malaise, and insomnia.
Overdosage with Salmeterol can lead to clinically significant prolongation of the
QTc interval, which can produce ventricular arrhythmias. Other symptoms of
overdosage may include hypokalemia and hyperglycemia.

Overdose
Treatment consists of discontinuation of this medicine together with
appropriate symptomatic therapy. The judicious use of a
cardioselective beta-receptor blocker may be considered, bearing in
mind that such medication can produce bronchospasm. There is
insufficient evidence to determine if dialysis is beneficial for
overdosage of Salmeterol. Cardiac monitoring is recommended in cases
of overdosage.

Seroflo Rotacaps During Pregnancy
Fluticasone and Salmeterol Powder for Inhalation (Seroflo Rotacaps) is
classified by the US FDA as Pregnancy Category C. There are no
adequate and well-controlled studies with Seroflo Rotacaps (Fluticasone
and Salmeterol Powder for Inhalation) in pregnant women.
Fluticasone propionate alone was teratogenic in mice, rats, and rabbits, and
Salmeterol alone was teratogenic in rabbits and not in rats. From the
reproduction toxicity studies in mice and rats, no evidence of enhanced
toxicity was seen using combinations of Fluticasone Propionate and
Salmeterol when compared with toxicity data from the components
administered separately. Seroflo Rotacaps should be used during pregnancy
only if the potential benefit justifies the potential risk to the fetus.
