OHIO’S PREMIER ONLINE TRAINING HOME
HEALTH AIDE CERTIFICATION PROGRAM
What is Incots?
Incots, is an online program that offers home health
aide training in compliance with Medicare, Medicaid
and Passport requirements
What is offered in the training?
The Home Health Aide Training consists of 12
modules. Each module contains:
Reading assignments
Case Study
Vocabulary
Videos
Quiz
What’s after the online modules?
At the completion of the 12 modules, students are
given a program review followed by a comprehensive
final exam.
Students must pass the final exam with a score of 75%
or higher.
Skills Lab
After successful completion of the online class,
students are required to complete 16 hours of skills
training. This training is done face to face with Incots
Nursing Instructors
When is the Lab Offered?
Each student is required to chose a time slot that they
will attend for 4 days.
Times: 7 AM-11 AM
12 PM-4 PM
5 PM- 9 PM
What are the dates for Lab?
The dates for the lab begin on July 22, 2013
Students are able to select their own time slot, but the
specific dates for their 4 days of training are scheduled
by Incots staff.
What about test scores?
Students must pass the daily quizzes with a score of 75
or higher. Students who complete the case study are
given a starting score of 5, to be added to their quiz
score
What about test scores?
Students must pass the final exam with a score of 75 or
higher. Students who have completed all case studies
are given a starting score of 5, to be added to their final
exam
What about test scores?
Students are allowed one make-up attempt for all daily
quizzes, if they do not pass on the first attempt.
Students must pass the final exam on the first attempt,
no make-ups are given.
What about test scores?
The Skills Lab is on a Pass/Fail basis. Students must
attend all 16 hours required. There will be limited
make-up dates available, on a first-come, first-served
basis
What’s after the Lab?
Upon successful completion of the skills training,
students are awarded certification as a Home Health
Aide.
Certificate of Achievement
Presented to
_________________________________________
on the 4th day of August, 2013, for successfully completing
the 75 hour home health aide training course as required
by the Medicare/Medicaid and Passport programs.

What’s next?
Graduation Day
Students are awarded their certificate of completion at
a graduation ceremony that will take place on August
4th, followed by a City Wide Job Fair
How do I get started?
Southeast Outreach Ministry Program and Incots are
partnering to offer the online training program at no
cost to participating students*
How does that work?
Scholarships
Incots will be awarding scholarships to every
participant. The regular cost of the online program is
$300 and Incots will award $150 to all enrolled
students.
Community, Friends and Family
Southeast Community Outreach Ministry and Incots
have partnered together to establish a Community,
Friends and Family Matching Fund to cover the cost of
the remaining balance of $150.*
All students must pay a $25 non-refundable
enrollment/application processing fee. Please do not
pay and enroll, unless you are sure you want to
participate in the program
C.F.F. Matching Fund Donations
Donations will be accepts at www.incots.org
Donations must include your enrollment code
Donations not made via website must be mailed to :

How does the CFF Matching Fund
Work?
Enrollment
 Enrolled students are assigned an enrollment code and
are able to request donations from Community,
Friends and Family. These donations are made online,
using the student’s designated enrollment code.
How do I enroll?
 Complete the application
 Submit the non-refundable enrollment fee
 Obtain your enrollment code
 Start contacting your Community, Friends and Family!
Who can I ask to contribute?
Community members such as
Your church members
Your barber, hair stylist, or nail tech
Your grocer, your dry cleaner
Former teachers and coaches
Who can I ask to contribute?
Friends such as
Neighbors
Facebook friends
Co-workers
Who can I ask to contribute?
And of course…..
Your family members
All of these individuals would love to help you success
and gain a new career.
Registration has begun
Registration for the program is June 10,2013 through
June 25, 2013
Students whose accounts are not at $150 by July 1, 2013
will need to discuss other arrangements with an Incots
counselor*
