4. Form 3(Pension Fund)
 Consolidated Return of Employees who are entitled and require to become members of
the Pension Fund on the date the Pension comes into force (within 15 days of Coverage)

5. Form F1 (Insurance Fund)
 Consolidated Return of Employees who are entitled and require to become members of
the Insurance Fund on the date the Pension comes into force (within 15 days of
Coverage)

6. Form F4 (Pension Fund)
 Consolidated Return of Employees who are entitled and require to become members of
the Pension Fund on the date the Pension comes into force (within 15 days of Close of
Every month)

7. Form F5 (Pension Fund)
 Return of Members leaving service during the month(within 15 days of Close of Every
month)

8. Form F2 (Insurance Fund)
 Return of Employees entitled for membership of Insurance Fund (within 15 days of Close
of Every month)

9. Form F3 (Insurance Fund)
 Return of Members of Insurance fund leaving service during the month(within 15 days
of Close of Every month)

10. Form F7 (Pension Fund)
 Contribution Card for members for the Year: This form Showing month wise recoveries
towards Provident Fund in respect of a member for one financial year. To be furnished by
the employer before 30th April of the following years.

11. Form F8 (Pension Fund)
 Consolidated Annual Contribution Statement: This form provides annual contributions of
each member of the establishment. A Vital form for compiling the annual Provident Fund
statement of a subscriber to be submitted by 30th April of the following year.

12. Form F4 (Insurance Fund)
 Consolidated Annual Contribution Statement: This form provides annual contributions of
each member of the establishment. A Vital form for compiling the annual Insurance Fund
statement of a subscriber.

13. Form F9
 Declaration by a person taking up employment in an establishment in which the
Employee’s pension scheme is in force.

14. Un-Exempted Establishment

15. Form 1(Revised)
 This is given by Provident Fund members and utilized for Provident Fund dues to the nominees. It also
contains the details of the Family members

16. Form 9 (Revised)
 Return of employees who are entitled and required to become members of the
employee’s Provident Fund & Pension fund (within 15 days of coverage)

17. Form 5A
 Return of ownership of the establishment- To be forwarded immediately after coverage
& whenever there is a change in the ownership, it has to be intimated with in 15 days of
change.
 Return of ownership to be sent to the Regional Commissioner (in Duplicate) with in 15
days of Coverage.

18. Form 12A
 The details of the contributions recovered from the members & paid along with details of
employer’s contribution & administrative charges - This is to be submitted monthly by
25th of following month.
 Consolidated Statement Dues and Remittance by 25th of the following month to which
the Dues relate.

19. Form 5
 The details of the employees enrolled newly to the Provident Fund- To be submitted
along with Form-12A every month within 15 days of the following month.
 Return of Employees Qualifying for membership to the Employee’s Provident Fund for
the first time during every month.(Within 15 days of the following month)

20. Form 10
 The details of the employees leaving service during the month- To be submitted along
with form-12A
 Return of members Leaving service during the Month.

21. Form 3A
 Member’s Annual Contribution Card: Form showing the month wise recoveries towards
E.P.F & Pension Fund in respect of a member for one financial year. To be furnished by
Employer before 30th of April of Following year.
 In this form we need to fill both employee as well as employers contribution month wise
starting from 1st of April (xxxx) to 31st March of (yyyy)
 The details of wages & contributions in respect of each member, to be prepared financial
year wise- To be submitted to the Provident Fund office by 30th of April every year.

22. Form 6A
 Yearly consolidated statement of contributions- To be forwarded yearly along with form-
3A. It should be ensured that all the form-3A are entered in form-6A, irrespective of
whether the form-3A was forwarded for the broken period and the total dues as per the
form-12A for the whole year agrees with the total of form-6A within 30th April.
 This Provides annual contributions of each member of the establishment
 It is a vital form for compiling the annual provident fund statement of a subscriber & To
be submitted within 30th of April

23. Form 11
 TRANSFER CLAIM FORM
 Mainly used for PF transfer from previous Employer
