2. CASE: GULLIAN BARRE SYNDROME
Name: Aniket Pawashe
Age: 35yr.
Gender: Male
Chief Complaint :Weakness in both hand and legs, Difficulty in doing daily
activities , Difficulty in transfers.
HOPI: Patient had history of dengue following which he developed
1) Sudden weakness in all four limbs causing difficulty in all activities requiring assistance in basic
activities.
2) Weakness was progressive affecting right lower limb and left upper limb more .
DESCRIPTION
3. EXAMINATION AND FINDINGS
Range of Motion: passively full in both upper and lower limbs.
Manual muscle testing :over all muscle power reduced (2/5).
Balance- fair static balance (standing).
Gait-reduced arm swing, reduced swing phase and heel strike.
Sensory examination: all sensations intact.
4. REHABILITATION
Proprioceptive neuromuscular facilitation for initiation of pelvis and upper and
lower limb movements given
Range of motion exercises done with assistance.
Weight bearing and shifting activities given.
Core activation exercise done over static and dynamic surface.
Gait training done using HOCOMO LOKOMAT to train and achieve all phases
of gait.
Balance training done using Therabalance with virtual feedback .
5. REHABILITATION
Progress:
Patient currently able to get up from sitting by himself .
Full range of motion achieved on right upper limb and 110 degree range achieved
on left upper limb.
Gait pattern improved with initiation of heel strike Standing balance is good (static).
Plan:
To achieve dynamic balance and achieve normal Gait pattern.
