Positive Change Program
Positive Change is a Health and Wellbeing group
program for adults who want to improve their health. It
assists people to eat well and be active, manage stress
and make long-term behaviour changes to support a
healthy lifestyle. It is usually conducted over nine or ten
weeks, including seven two-hour workshops and two to
three individual appointments. It does not provide a 'diet'
to follow but will help you learn to:
yy prepare and enjoy healthy food
yy discover enjoyable ways to be physically active
yy overcome problems and set goals
yy cope with stress
yy feel better about yourself
yy develop a support network to help you sustain
your new habits.
A journey to a healthier lifestyle
Your Path, Our Path is a group-based healthy
lifestyle program for Aboriginal and /or Torres Strait
Islander people. This flexible program encourages
participants to identify their interests, needs and
concerns and structuring the program around these.
Your Path, Our Path is a tailored program that
includes up to 12 workshops depending on
the groups’ needs. The program encourages
participants to seek a healthy lifestyle through good
nutrition and physical activity and teaches them life
skills to be able to do this in their daily lives.
Topics can include, but are not limited to:
yy Why do people get too heavy?
yy Understanding a symptom cycle
yy What to eat to stay healthy
yy Tools for better health
yy Introduction to exercise and National Guidelines
yy Problem solving techniques and goal setting
yy Self Esteem and Behaviors Changes
yy Dealing with difficult emotions
yy Healthy cooking
yy Understanding grief and loss
yy Communicating effectively with partners, carers,
and health professionals

Diabetes Self-Management Program
This program is for those who are diagnosed with
Type 2 Diabetes. This holistic six week program will
help you to better understand what diabetes is and
how to take care of your health. We will cover many
topics over the six weeks which include:
yy healthy eating
yy preventing complications, low blood sugar
yy physical activity
yy stress management
yy relaxation techniques
yy monitoring blood sugar
yy medications
yy sick days
yy skin and foot care.
Chronic Pain Self-Management
Program
This program is for those who experience chronic
pain. (Pain is defined as chronic or long-term when
it lasts for longer than three to six months or beyond
the normal healing time). During the six week program
you will learn techniques and skills to reduce the
impact of your condition on your day-to-day life.
We will discuss the below topics and much more:
Health and Wellbeing Programs

yy debunking myths
yy fatigue and sleep
yy physical activity
yy healthy eating
yy medications

Better Health for Life Program
This program suits those living with a long-term health
condition. This six week program will support you to
better manage your symptoms and achieve quality of
life. During the program you will learn to:
yy better manage difficult emotions
yy communicate effectively with partners, carers,
and health professionals
yy manage your symptoms
yy get started with appropriate levels of exercise
and healthy food
yy set goals and problem
solve
