重炭酸療法
 低灌流由来の乳酸アシドーシスではあるが
,Ph≧7.15の患者では血行動態改善目的,ま
たは昇圧薬の需要を減らす目的で重炭酸塩を
使用する事に反対する(弱い推奨,中等度の質
のエビデンス).

乳酸アシドーシスに重炭酸を使用しても血行動態は改善しない.
A prospective,controlled clinical study, Vacouver, Canada
 目的: 乳酸アシドーシスの患者に,重炭酸を使
用するとアシドーシスが改善するかを判定する.
 方法: 前向き,無作為,盲検,横断試験. 3次
病院のICUで代謝性アシドーシス(HCO3
-
17mmol/L,BE<-10)で高い乳酸値(平均
7.8mmol/L) の患者14名が対象. 全員
PACが留置され,13名はCA投与中.
NaHCO3か同モルのNaClを無作為の順番に
投与.

 結果:
NaHCO3 2mmol/kg/15min.
・pH 7.22→7.36, P<0.001
・HCO3
- 12→18mmol/L, P<0.001
・PaCO2 35→40mmHg, P<0.001
・PCWP 15-17mmHg, P<0.001
・Ca++ 0.95→0.87, P<0.001
・CO +18%, P<0.01
NaCl 2mmol/kg/15min.
・PCWP 14→17mmHg P<0.001
・CO +16%, P<0.01

A prospective, controlled clinical study, Lille, France
 目的: 乳酸アシドーシスの患者に重炭酸を使
用すると,血行動態変数と組織酸素化が改善す
るかを判定する.
 方法: 前向き,盲検,無作為,横断試験. 重炭
酸Na(1mmol/kg)またはNaCl(同量)を順
番に1時間投与されるが順序は無作為.
 対象: 乳酸>2.45mmol/L,
creatinine<2.3mg/dLの患者10名
