I. PURPOSE
To establish guidelines and procedures for the initiation, proper use and completion of the
OC Animal Care Home Quarantine Fee Notice.
II. POLICY
Owners/Custodians of animals who meet OC Animal Care’s requirements and agree to
abide by the conditions and restrictions related to the quarantine of an animal at the
residence of the animal owner/custodian are required by law to pay the current Home
Quarantine Fee.
Officers are expected to make all efforts to collect the Home Quarantine Fee when
imposing quarantine. However, in instances where the animal owner/custodian is unable
to pay the fee at the imposition of quarantine, officers will initiate the Home Quarantine
Fee Notice as a last resort.

A. Use of the Notice

Associate the following numbers with the corresponding lettered sections on
Attachment 1, Home Quarantine Fee Notice.
A. B#: The Rabies Control Specialist will enter the “B#” or Bite number
generated by Chameleon once a Bite window is created.
B. Activity#: Enter the Activity Number.
C. Date: Enter the date the Fee Notice is initiated (e.g., 09/12/13 or
September 12, 2013)
D. Name: Enter the first and last name of the owner/custodian of the animal
being quarantined, (e.g., Martin Hansen).
E. Address: Enter the street address of the owner/custodian of the animal
being quarantined (e.g., 6589 Elm Street).
F. City/State/Zip: Enter the City, State and Zip Code of the owner/custodian
of the animal being quarantined (e.g., Brea, CA 92622).
G. Quarantine Period: Enter the dates, of the quarantine period, beginning
with the date of the bite and ending at the last date of the quarantine period
(e.g., 09/12/13 – 09/22/13).
H. Date of Bite: Enter the date of bite/scratch, e.g., 09/12/13 or September
12, 2013.
I. Description of Animal: Enter the complete description of the animal under
quarantine, including the animal’s name, (e.g., Female, Boxer, fawn, 2yrs,
“Daisy”).
J. Enter signature and badge number of the officer issuing the fee notice.
B. Distribution of the Notice
1. The owner/custodian receives the original or top copy of the Notice along
with a pre-addressed white envelope.
2. Advise the owner/custodian that the fee must be paid within the prescribed
time or formal collection proceedings will begin.
3. Enter the word “Billed” in QT fee section of the Animal Bite/Scratch
Report next to RCPT# and check the “No” box.
4. Attach the remaining copies of the Notice to the Animal Bite/ Scratch
Report and place in the designated Bite Report slot located in the Squad
Room. Upon receiving the Home Quarantine Fee Notice, the Staff
Specialist overseeing the Rabies Desk will forward all billing paperwork
to the Accounting Department for processing.

C. Notice Review (Watch Commander/designee)
1. Verify the Notice is completed correctly.
2. Enter initials and badge number next to the signature of the Officer
initiating the Notice.
3. The Notice will remain attached to the Bite Report. The Rabies Control
Specialist will then create a Bite window in Chameleon and route the
paperwork to the OC Animal Care Accounting Department.
