• This is the 2nd presentation of the complete DigiCare Learning Package, which
can also be used as an independent presentation. (All Learning Packages in
the slide 3)
• We recommend to present this 2nd learning package by utilizing active
teaching methods, e.g., interactive lecture, flipped learning, individual and
small group discussions (See the ideas we’ve included in the slide notes).
• To get the most out of this package, please read the ideas and suggestions for
further reading material (See the slide notes).

Digital care
DigiCare Model enables future healthcare professionals to utilize existing
infrastructure (environment, technology, digitalization, support, culture, customs,
economy, privacy and policy) to integrate digital coaching into healthcare.
Digital coaching emphasizes communication and patient initiation & feedback to
support patients with non-communicable diseases, and their families, to achieve
self-management competency, self-care motivation and health literacy towards
positive health and quality of life.
The DigiCare Model is enveloped by sustainable and quality ethics, including
individuals’ rights and responsibilities, equity in digital healthcare and
governance of healthcare data.

DigiCare Model enables future healthcare professionals to utilize existing infrastructure (environment, technology, digitalization, support, culture, customs, economy, privacy and policy) to integrate digital coaching into healthcare. Digital coaching emphasizes communication and patient initiation & feedback to support patients with non-communicable diseases, and their families, to achieve self-management competency, self-care motivation and health literacy towards positive health and quality of life. The DigiCare Model is enveloped by sustainable and quality ethics, including individuals’ rights and responsibilities, equity in digital healthcare and governance of healthcare data. PERSON (with Non- Communicable Disease, NCD) 7/31/2023 11 • The core and foundation of the DigiCare Model is the relationship between a person with NCD and a Health Care Professional (HCP) • The Model offers benefits for persons with NCDs by improving • Health literacy and understanding of positive health (salutogenesis) • Motivation to health behaviour changes • Self-care and self-management • Digital literacy • Competence to use digital tools to support self-management • Communication among relevant stakeholders (e.g., HCPs, family, community)

PERSON (as Health Care Professional, HCP) 7/31/2023 12 • Refers to HCPs, e.g., nurse, doctors, medical practitioners, healthcare students. • The DigiCare model aims to support HCPs’ competence to empower persons with NCD • DigiCare Model supports the development of competence in: • Professional communication • Coaching and the use of coaching models • Self-management motivation and support for persons with NCD • Digital healthcare • Understanding the benefits of digital well-being and health care tools in health care delivery • Introducing digital tools to persons with NCD for them to independently self-manage and monitor health and well-being

  1. Ideas for the teacher/presenter: Please, note that the first 5 slides are for the teachers’ information prior to using this Learning Package and implementing this into the course or curriculum. The DigiCare consortium recommends to use interactive lectures, flipped learning, individual and small group discussions, world café and other active teaching methods while presenting this presentation. In case of using this during the lectures, interrupt your lecture and encourage students to individual, pair or small group discussions related to topics presented. More ideas can be found in the slide notes.

  3. Ideas for the teacher/presenter: Stop your presentation here for a while.. Ask students to form pairs or small groups. Instruct them to discuss about the digitalization and health, healthcare. Encourage them to brainstorm and think “outside of the box”. Why, what, between who etc. is digitalized healthcare important or is it? Ask student to find materials about the implementation of the digital healthcare in their country, community, family. You can ask students to work in pairs or groups and collect their thoughts, evidence-based information etc. into the Flinga, Padlet or any other virtual bulletin board. Present your work. (by their own language) Some discussion points_ Several questions might raise from students’ own experiences, encourage them to share their experiences. Can you think about the link between 4 layers of the model? Why is a person considered as the CENTER of the TIP in the reverted pyramid? How do you think the model functions in your country?
