Professor of Restorative Dentistry

4. Content and Learning Outcomes
Content:
• Diagnosis of the patient to detect his caries risk status
• ADA Caries Risk Assessment Forms
• Customize management of the patient based on his caries risk level
Learning Outcome
• Identify the caries risk status of each patient based on specific criteria.
• Formulate caries management protocol each patient individually according
to his caries risk status
Prof. MA Fareed PhD 4
5. Caries Risk Assessment – Introduction
• Dental clinicians treat the entire patient and not just individual teeth
and caries lesions
• It is very important to detect caries lesions to provide prevention and
treatment (restoration), clinical examination process for diagnosis and
detection of caries lesions is important
• Caries detection and caries diagnosis are not exclusively a tooth-
centered process
• Management of the caries is the ability to individualize caries
diagnosis and treatment or for each patient
Prof. MA Fareed PhD 5
6. Caries Risk Assessment – Importance
• Formulate a caries risk assessment profile based on individual
patient’s current risk factors risk indicator
• Caries risk assessment is necessary to identify interventions to lower
the patient’s caries risk and activity
• High-risk categories à caries lesions over the last 2 to 3 years
• Caries management by risk assessment (CAMBRA) à provides
individualized evaluation of a patient’s pathologic factors and
protective factors and risk for developing future disease
• International Caries Classification and Management System (ICCMS)
Prof. MA Fareed PhD 6
7. Caries Risk Assessment – Variable Factors
• Social, Economic, and education Status
• Diet Analysis
• Salivary Analysis
• Dental Clinical Analysis (Dental Exam)
• Bacterial Biofilm Analysis
• Caries risk assessments for adults and adolescents older than 6 years
• Caries risk assessments for children under 6 years of age
Prof. MA Fareed PhD 7
8. Caries risk assessment form
Initial caries diagnosis process
Three sections
1. Contributing Conditions
Fluoride, surgary diet, dental visits
2. General Health Conditions
Sepcial care, chemo/radiotherapy, eating
disorders, salivary flow, medications,
3. Clinical Conditions
Caries, restorations, extraction, plaque,
open contact, ortho, xerostomia
Prof. MA Fareed PhD 8
11. ADA Risk Assessment Form (Age > 6)
Low risk ≤ 3, Medium Risk 4-8, High Risk ≥ 9
Prof. MA Fareed PhD 11
12. Low risk ≤ 3, Medium Risk 4-8, High Risk ≥ 9
Prof. MA Fareed PhD 12
13. Caries Risk Assessment - Sequence
• Interview (dental and medical history) of the patient
• Clinical examination for diagnosis and detection of caries lesions
• Caries risk assessment identifies the risk (causative, susceptibility)
• Risk factors can be modified and can predict the caries outcome
• Without caries risk/susceptibility, caries management at the
individual patient level will never be successful in the long term
Prof. MA Fareed PhD 13
14. High risk-Low risk
• Caries risk include age,
gender, fluoride
exposure, home care,
smoking habits, alcohol
intake, medications,
dietary habits, economic
and educational status,
and general health.
• Increased smoking,
alcohol consumption, use
of medications, and
sucrose intake result in
increased risk for caries
development
Prof. MA Fareed PhD 14
16. Caries Risk Assessment – Conclusions
• Risk assessment helps to identify the etiologic causes of the disease
for a specific patient at a specific point in time
• Useful in the prevention of caries lesions
• Help to determine treatment protocols for patient follow-up visits
• Selection of restorative material used and cavity preparation design
• Useful as a baseline for future reassessments of caries management
• Uncovering risk factors that are present before disease manifestation
• Prevention and management of caries because of the multifactorial
