2. GOOD DOCUMENTATION BEST PRACTICES:
Follow Data Entry Guideline
Do not enter anything illegible or unclear
Only enter what you see or what client tells you
Use highlighter to flag any questions
Error CORRECTION (Handwritten Notes)
Cross out the error with one line, leaving the
original entry readable.
Write the correct data next to or above the original
data, now crossed-out and circled.
Initial, date and, indicate the reason for the
correction, if necessary.
Never use correction fluid or tape (e.g., white out)
Never destroy original documents (even if they
require error correction!)
Do not alter past-dated notes or back-date e.g., by
writing alongside or adding to prior entries.
If you identify a problem with documentation you
cannot resolve on your own notify a supervisor.
