Describe and Model:
1. Journal: What do I want my life to look like after high school?
(10 minutes).
2. Discuss student responses to their journals. This will serve as
an introduction and lead us into completing the transition
activities
3. I will introduce each transition activity to the students in a
whole group setting. I will be reading aloud some of the
surveys to students who need it to be. Before beginning, I will
talk with students about how high school is the last time they
will be entitled to receive services so it important to have a plan
before you graduate from high school. Knowing what supports
or service agencies that are available for you to connect with are
important to know for making choices about where you want to
live and work and whether or not you want to continue your
education. In your IEP meetings the team will discuss and want
to know what your goals are for you after you leave high school
such as: where do you want to live, who are you going to live
with, are you going to rent or buy a home, what will you do for
work, how will you get around etc. these activities are helpful to
look at and refer back to in your IEP meetings so that everyone
can see your interests.
2. 4. Students will complete the transition surveys independently
but may ask questions if they need help. I will be scoring the
surveys that need to be.
5. Once students have finished their surveys they will complete
class work for other classes that needs to be done.

Practice
I will be leading a discussion with students about what they want to
do after high school. Students will have the opportunity to share
their plans and some goals they might have for themselves.

Practice
Students will complete transition assessments ontheir help serve as
a guide in planning and determining postsecondary goals.
LessonClosing
Students will turn in their transition activities and continue to work
on their work for other classes that needs to be done.
Formative Evaluation
Basedon the results of the students transition assessments students
will use those results to plan and determine postsecondary goals.
Students will also being using these results when they conduct their
career research.
