1. Abdominal Cerclage and
Isthmocoele Repair

2. Case History
● G1 - Had a cervical Incompetence ,
rescue/emergency cerclage done, patient admitted
in Hospital but developed Preterm labour so LSCS
done at 26 to 27 weeks , elsewhere. Baby couldn’t
be saved
● Presented with complaints of continous Spotting
and Discharge , USG and MRI showing Isthmocele.

3. ● As patient needed Isthmocele repair with Laparoscopy
● Abdominal cerclage option was explained to the couple
and proceeded

4. ● Hysteroscopy done first and defect visualised

5. ● Laparoscopy done and bladder adhesions dissected.
● Defect identified with the help of transillumination and
Sutured with V lock sutures after freshening the edges.
● Overlapping of edges done

6. Abdominal Cerclage
● Mersilene tape is used as in Transvaginal cerclage
● Internal os is identified with the uterine artery entering the
cervix.
● Cervical suturing done .
