  6. 6. What are Stem Cells? <ul><li>Unspecialized cells that can become various somatic or body cells, e.g., eye cells, skin cells, liver cells, neurons, etc. </li></ul><ul><li>Found in high number in your bone marrow </li></ul>

  8. 9. <ul><li>These are stem cells they give rise to blood cell types such as platelets, disease-fighting macrophages & a host of others. When they leave the marrow and engraft in other tissues they tend to become cells that support, heal & restore. </li></ul>
  9. 10. <ul><li>Injuries, infections & other damage in organs & tissues releases a chemical signal that summons stem cells from the bone marrow to repair these. </li></ul>

Adults over age 40 tend to have a lot of senescent or less robust stem cells. Can they be revitalized? Mesenchymal Stem Cells (MSC) <ul><li>MSCs produce bone cells, fat cells, tendons, ligaments and muscle cells. </li></ul><ul><li>They also produce the extracellular matrix that supports neurons and other cells including those in the eye. </li></ul><ul><li>They have the inherent flexibility or plasticity to become other cells types including retinal type cells. </li></ul>

  14. 15. Treating Macular Degeneration with stem cell-rich bone marrow <ul><li>Both hematopoietic and mesenchymal stem cells are extracted from the bone marrow and are then immediately given back to the patient, either by intravenous (iv) means or retrobulbar injection or both. </li></ul>

Bone Marrow Harvesting <ul><li>A local anesthetic is used to deaden the harvesting area. </li></ul><ul><li>Most of the time bone marrow is removed from the tibia are large bone in the lower leg. The procedures takes only a few hours. </li></ul><ul><li>Patients report feeling little if any pain or discomfort. </li></ul>
  18. 19. Two primary ways stem cell-rich bone marrow is given for ARMD <ul><li>By IV: The bone marrow is given intravenously. </li></ul><ul><li>Retrobulbar injection: Cells are injected in tissues behind the eye. </li></ul>

Umbilical Cord Blood Stem Cells <ul><li>Umbilical cord blood contains many of the same kinds of stem cells as bone marrow. </li></ul><ul><li>Many doctors & researchers believe that umbilical cord stem cells reverses retinal pigment epithelial dysfunction which is one of the main causes of age-related macular degeneration. Cord Blood Stem Cells cont. <ul><li>In some instances Dr. Steenblock may refer a ARMD patient to be treated with cord blood stem cells outside the US (It is illegal for doctors to use cord blood stem cells to treat CP in the US.) Usually this is because both bone marrow & cord blood stem cells are needed to achieve the greatest degree of healing & restoration possible. </li></ul>
  22. 23. Hyperbaric Oxygen Therapy <ul><li>When indicated Dr. Steenblock prescribes a course of HBOT. This is oxygen given under pressure in a HBO chamber. This treatment gets oxygen into parts of the body including the brain which reverses ischemia (blood flow is compromised) and hypoxia (lack of oxygen). </li></ul>

Hyperbaric Oxygen Therapy continued <ul><li>HBOT constricts blood vessels and disrupts the vicious cycle where a lack of oxygen leads to tissue swelling, which then leads to further oxygen deficiency, etc. It also speeds up elimination of accumulated waste deposits in the retina called drusen, a player in ARMD. </li></ul>Dr. Steenblock ’s 4-chamber HBOT Room

HBO and Stem Cells <ul><li>Hyperbaric oxygen releases stem cells from the bone marrow into the circulation. </li></ul><ul><li>Hyperbaric oxygen treatments enhance the growth of the stem cells and their ability to migrate into damaged tissues. </li></ul><ul><li>HBOT is typically given daily before and after stem cell treatments. </li></ul>

Intravenous (IV) Treatments <ul><li>In some instances Dr. Steenblock determines that certain growth factors, amino acids, vitamins, minerals or other compounds will help encourage rapid healing of ARMD either before or following a stem cell treatment. Sometimes heavy metals need to be removed using a chelating drug. These are given by IV drip as this gets them into the circulation so they can go to work right away. </li></ul>Customized IV cocktails

Enhanced External Counterpulsation (EECP) <ul><li>EECP is simple in concept but profound in its effects: A computer driven machine inflates large cuffs around the patient ’s legs & waist whenever the heart relaxes. This pushes blood back through arteries opening them up. This increases blood & oxygen flow throughout the body including the eyes. Lack of both characterizes ARMD. </li></ul>
  27. 28. Periodic Acceleration Therapy <ul><li>In PAT patients lying prone on a motorized platform get a moderate cardiovascular work-out from a gentle back-and-forth motion. This creates a very mild stress on blood vessels that gets them busy producing Nitric Oxide (NO), a chemical that increases blood flow to organs and tissues body wide. It also mobilizes stem cells from bone marrow. ARMD benefits greatly from the influx of blood, oxygen & stem cells into their eyes. </li></ul>
