2. • A cause-and-effect diagram, also known as a Fishbone diagram or Ishikawa diagram created by
Kaoru Ishikawa in 1968 is one of the seven tools of quality
• It is a graphical description of the elements of a specify quality problem and the relationship
between those elements
• All causes are then examined to find a way to change them with a solution that is within our
control, prevent reoccurrence and meets goals and objective
• The “effect” box at the starting/end of the diagram is the quality problem that needs correction
• A centre line connects the effect box to the major categories of possible problem causes,
displayed as branches off of the centre line
• Causes can be derived from brainstorming session. There are levelled as categories for example
6M’s (Manpower, machine, material, method, measurement, mother nature) for
manufacturing, 7P’s for marketing etc.
• Advantages :-
• In depth analysis of problem
• Easy to implement
• Gives a better understanding of problem
• Shows weak area in solution
• Disadvantages :-
• It doesn’t solve any problem
3. Lazy

No time importance
Effect Causes
Late
arrival of
students
4. 1. Late Wake up
2. Habitual Lateness
3. Lack of Strictness
1. Late Wakeup :- One of the main root cause of late arrival of the students to the classes is late
wake up in the morning because of waking up late in the morning we don’t get enough time to
do the daily regular activities before heading to the school/colleges sometimes we miss the
school bus or some times we have to skip the breakfast.
Why why why logic…..
a)Sleeping late night
b)Irregular time of sleeping
c)Laziness
2. Habitual Lateness :- Habitual lateness is a cause because of considering lack of importance of
time. One of the reason of learning from friends or family members habitually arrive late for
social gathering and usually infuriate us because of their thoughtfulness. There is a tendency of
“Chalta Hai” in every case so every body is careless about timing of classes.
3. Lack of Strictness :- Lack of strictness is also an important cause of getting late in the class
because of it students get carried away and don’t care about the discipline and punctuation for
the classes. The school not having clear policies and strictness regarding attendance make
student more lazy.
5. 1. Teacher should make clear to students on the very first day of the classes about the policies
of the lateness and its consequences
2. Teacher should explain the students the importance of social and professional value of being
on time
3. Teacher should give the consequences of being late like along with their attendance policy
(two times late entry counts as one absence)
4. Teacher should be sure to arrive the class and get the class started on time and one more
important thing should dismiss the class on time, too
5. Teacher should make the beginning of the class more meaningful
6. Student should have a proper time management so that they can utilize their time properly
and can do every work at time
7. Students should sleep early so that they can wake up early in the morning. They should also
have a regular time of sleeping
8. The management should not keep longer classes and they should provide small breaks
between different classes
9. The system should have strict policy for late comers and students having short attendance
10. Family members should encourage the student to be on time
6. • Cause and effect diagram is one of the important tool of the quality control. It doesn’t solve
the problem instead it will tell the reason or causes for particular problem
• Other than industries Ishikawa diagram can also be applied in our daily life to find the
reasons behind the problem
• It helps in easy understanding of the processes
• It will tell where the process improvement is required
• We learned how to draw the cause and effect diagram and use it to solve the various types of
complex problems
• It also helps in solving day to day problems
• This diagram help in visually display the many potential causes for a problem or effect
