2. Introduction
a) The internship was started on 1ST June, 2022 in Kitgum General
Hospital.
• Importance of the Internship
• The importance of the internship include the following;
• To learner
• Equip the learners with knowledge on practical basis
• Learners develop additional skills in handling various conditions in
clients
• It helps learners to improve on ward arrangement especially in
maintaining well set emergency trolley and health team in handling
emergencies.
• It helps learners identify their gaps in handling cases and come up
with solutions.
3. • It promotes quick recovery to clients since clients receive maximum
medical care.
• It improves physical, mental, social and spiritual wellbeing of clients
during hospitalization.
• Counseling interaction with clients improve nurse patient relationship
since we give them time.
• In our internship we help clients allay fear and anxiety.
• b) Internship site
• My chosen internship site is Kitgum General Hospital. Kitgum General
Hospital is a district hospital located in central division, was built in
1938.
4. • Care Unit, Pediatric Unit, Surgical ward, Medical ward, Theatre,
Laboratory, X-ray and Ultra sound, Record department, Mental and
Administrative department.
• Kitgum General Hospital offers full services to clients except some few
cases who need specialists with use of special machines which are not
in place who are referred. On average, it serves 60,087 patients in
outpatients and 15115 patients in inpatients annually

5. • SERVICES OFFERED IN KITGUM GENERAL HOSPITAL
• Services offered are; mental care, dental deliveries care, eye care,
General outpatient services, youth services, emergency care,
antenatal care, care, Postnatal care, family planning, screening of
cancer of the cervix and breast , immunization( young child ,
vaccinating girls or mothers in ages of reproduction), Admission:
surgical medical, tuberculosis, pediatric and NICU resuscitation, post
abortion care, orthopedic, physiotherapy and rehabilitative care, care
for HIV positive clients, ( STDs/STIs), counseling and guidance, minor
and major surgeries, health education and support to clients, other
departmental different services and referral of some cases.
• Generally, Kitgum General Hospital offers health promotive,

• Gynecological History:
• She said she had her menac in S.2 at 15 years old. Her LNMP
11/5/2022 EDD 18/2/2023.
• She is a Gr1 Para O.
• Presenting complain
• Persistent vomiting with generalized body weakness, no any oral
intake is retained in the stomach, dizziness and collapse.
• Past obstetric history: Nothing of concern
7. • Past medical history: She said, she had been healthy
• Family history: She reported that her mother is a case of
hypertension and on treatment.
• They are born 5: 3 girls, 2 boys. All are alive, both of her parents are
still alive as per her report
• Medical diagnosis: Hyperemesis Gravidarium
8. • Goal:
• To resuscitate the patient to resume normal state within 24hours to
three days
• Nursing Assessment
• Persistent vomiting
9. • Not able to retain any oral intake
• No energy/malaise
• Dizziness
• Temp. 36.0 degree Celsius, pulse 98b/m, BP 90/60 mmHg
• Flappy skin
• Nursing Diagnosis
• Low body glucose level related to poor intake, persistent vomiting as
evidenced by dizziness, weakness and collapse.
• Fluid volume deficit related to poor intake and lack of retention as
evidenced by flappy skin, dry mouth, and little concentrated urine.
