Americare is an array of holistic services that encompasses health promotion and teaching, curative intervention, rehabilitation
support and maintenance, social adaptation and integration and support for the patient and family and appointed caregivers.
Duties and Responsibilities:
• Completes initial, comprehensive and ongoing assessments of patient and family to determine homecare needs. Provides a
complete physical assessment and history of current and previous illness(es)
• Conducts home visits as assigned, assessing the patient’s condition, progress toward goals, providing patient care, teaching
and other skilled interventions as ordered.
• Develops a care plan in collaboration with Case Manager, which establishes goals based on nursing diagnosis and
incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family in the planning
process.
• Completes, maintains and submits accurate and relevant hospice clinical notes regarding patient’s condition and care given.
Records pain/symptoms management changes /outcomes as appropriate
• Teaches the patient and family/ caregiver self-care techniques as appropriate. Provides medication, diet and other instructions
as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and
families/caregivers. Works in concert with the home health interdisciplinary team.
• Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition;
obtains/receives physician’s orders as required.

assists in implementing prior to discharge of the patient.
2. Clarin Main Health, TB-DOTS and Birthing Center is a PhilHealth accredited health facility owned and
managed by the Local Government Unit of Clarin, Bohol, Philippines located just beside Clarin Community Hospital.
It serves the constituents of the town but also accepts people from neighboring town who are seeking health needs. It
has a functional Birthing Facility, Out-patient Department, Diagnostic Laboratory and TB-DOTS Center equipped with
well trained, highly skilled staffs.
Duties and Responsibilities:
• Identifies the needs and concerns of individuals, groups, families, and the community
• Interprets and implements nursing plans and programs. Assists other health team members in implementing health
programs in the setting
• Acts as resource speaker on health and health related services. Advocates health programs in the community

3. Clarin Community Hospital is government hospital located at Pob. Norte, Clarin, Bohol, Philippines which
