4. การตรวจหาเชื้อ HPV สายพันธุที่มีความเสี่ยงสูงมีความไวในการวินิจฉัยมะเร็งปากมดลูก รวมทั้ง
ความผิดปกติของปากมดลูกที่เกี่ยวของสูงกวาการตรวจดวยเซลลวิทยา (ตารางที่ 6)
บทบาทของการตรวจ HPV DNA ในการตรวจคัดกรองโรคมะเร็งปากมดลูก
ดังที่กลาวขางตน การติดเชื้อ HPV เปนสาเหตุสําคัญของการเกิดมะเร็งปากมดลูก(12,13)
การตรวจ
HPV สายพันธุที่มีความเสี่ยงสูงมีความไวในการตรวจพบความผิดปกติของปากมดลูกที่เปน CIN 2/3 สูงถึง
รอยละ 95 – 100 และมีคุณคาในการทํานายผลลบสําหรับ CIN 2/3 สูงถึงรอยละ 99 – 100 โดยเฉพาะเมื่อใช
รวมกับการตรวจทางเซลลวิทยา ถาการตรวจทั้ง 2 อยางใหผลลบจะมีไว และคุณคาในการทํานายผลลบสูง
ถึงรอยละ 100(20 - 23)
การตรวจ HPV สายพันธุที่มีความเสี่ยงสูงจึงไดรับความสนใจ และนํามาใชในการ
ตรวจคัดกรองโรคมะเร็งปากมดลูก
การตรวจคัดกรองโรคมะเร็งปากมดลูกโดยการตรวจ HPV DNA รวมกับการตรวจเซลลวิทยา
คาใชจายในการตรวจคัดกรองโรคมะเร็งปากมดลูกขึ้นอยูกับความถี่ในการตรวจ ดังนั้นถามี
แนวทางในการตรวจคัดกรองโรคมะเร็งปากมดลูกที่สามารถลดความถี่ในการตรวจลง แตยังคงไวซึ่ง
ประสิทธิภาพของการตรวจยอมจะชวยประหยัดคาใชจายลงไดอยางมหาศาล และเปนที่ทราบกันดีแลววา
สตรีคนหนึ่งอาจใชระยะเวลานานถึง 20 ปหลังการติดเชื้อ HPV ในการพัฒนาไปเปนมะเร็งปากมดลูก(24)
Van den Akker-van Marie และคณะ(25)
ไดทําการศึกษาในประเทศเนเธอรแลนด โดยเปรียบเทียบ
อุบัติการณการเกิด CIN 3 ที่ระยะเวลา 5 ปหลังการตรวจคัดกรองโรคมะเร็งปากมดลูกดวยเซลลวิทยาซึ่ง
ใหผลปกติ กับอุบัติการณการเกิด CIN 3 ที่ระยะเวลา 10 ปหลังการตรวจดวยเซลลวิทยาใหผลปกติ และการ
ตรวจ HPV DNA ใหผลลบ และสรุปวา การเพิ่มการตรวจ HPV DNA เขาไปในการตรวจคัดกรองโรคมะเร็ง
ปากมดลูกสามารถยืดระยะเวลาของการตรวจแตละครั้ง (กําหนดไว 5 ป) ออกไปไดอีก 2 – 5 ป กลาวคือ
สามารถตรวจคัดกรองโรคไดทุก 7 – 10 ป นอกจากนี้ Bulkmans และคณะ(26)
กําลังทําการศึกษาแบบ
randomized controlled trial ที่มีชื่อวา POBSCAM RCT ในสตรี 44,000 รายที่มาตรวจคัดกรองโรคมะเร็ง
ปากมดลูก และมีอายุระหวาง 30 – 60 ป โดยเปรียบเทียบระหวางการตรวจดวยเซลลวิทยาเพียงอยางเดียว
กับการตรวจดวยเซลลวิทยา รวมกับการตรวจ HPV DNA เพื่อตรวจหารอยโรคตั้งแต CIN 3 ที่ระยะเวลา 5 ป
โดยมีเปาหมายที่จะขยายเวลาการตรวจคัดกรองโรคมะเร็งปากมดลูกออกไปใหมากกวา 5 ป ผลที่ไดจาก
การศึกษานี้ อาจทําใหเกิดความเปลี่ยนแปลงในแนวทางการตรวจคัดกรองมะเร็งปากมดลูก โดยเพิ่มระยะหาง
ของการตรวจแตละครั้งออกไป
ในประเทศสหราชอาณาจักร มีการศึกษาการตรวจคัดกรองโรคมะเร็งปากมดลูกแบบ randomized
controlled trial โดยใชการตรวจ HPV DNA รวมกับการตรวจดวยเซลลวิทยา ไดแก The HART study
(HPV in Addition to Routine Testing) ซึ่งทําการศึกษาการตรวจคัดกรองโรคมะเร็งปากมดลูกในสตรี
11,085 รายในสหราชอาณาจักร การศึกษานี้ ยืนยันวาการตรวจ HPN DNA มีความไวมากกวา conventional
Pap smear ในการตรวจหารอยโรคขั้นสูง (รอยละ 97.1 เทียบกับ รอยละ 76.6) แตความจําเพาะของการตรวจ

HPV DNA ต่ํากวา conventional Pap smear (รอยละ 93.3 เทียบกับ รอยละ95.8) และ คุณคาในการทํานาย
ผลบวกก็ต่ํากวา conventional Pap smear (รอยละ 12.8 เทียบกับ รอยละ15.8) ถาใชการตรวจ HPV DNA
รวมกับการตรวจดวยเซลลวิทยาจะมีความไวรอยละ 100 ในการตรวจหารอยโรคขั้นสูง และ แนะนําใหทํา
การตรวจคัดกรองโรคมะเร็งปากมดลูกทุก 5 ป(27)
ในป ค.ศ. 2003 องคการอาหารและยา ประเทศสหรัฐอเมริกา ไดรับรองการใชการตรวจ HPV
DNA รวมกับการตรวจดวยเซลลวิทยา ในการตรวจคัดกรองโรคมะเร็งปากมดลูกในสตรีที่มีอายุตั้งแต 30 ป
ขึ้นไป ถึงแมวาการใชการตรวจ HPV DNA รวมกับการตรวจดวยเซลลวิทยาจะเปนความกาวหนาของการ
ตรวจคัดกรองโรคมะเร็งปากมดลูก แตขอมูลที่มีอยูในปจจุบันยังไมเพียงพอที่จะสรุปวาการตรวจคัดกรอง
โรคดวยวิธีดังกลาวกอใหเกิดผลดี และลดคาใชจายอยางชัดเจน รวมทั้งเปนที่ยอมรับของสตรีทั่วไปเมื่อเทียบ
กับการตรวจดวยเซลลวิทยา นอกจากนี้ ชวงระยะหางที่เหมาะสมของการตรวจคัดกรองมะเร็งปากมดลูกยัง
ไมมีการกําหนดแนนอนซึ่งมีตั้งแตทุก 5 ป จนถึง 7-10 ปดังกลาวแลวขางตน ทําใหแนวทางการตรวจคัด
กรองโรคมะเร็งปากมดลูกโดยการตรวจ HPV DNA รวมกับการตรวจดวยเซลลวิทยาขององคกรตางๆ ใน
ประเทศสหรัฐอเมริกา และหลายประเทศในทวีปยุโรปยังมีความแตกตางกัน
ในป ค.ศ. 2004 องคกรที่ดูแลดานสุขภาพในประเทศสหรัฐอเมริกา(28)
จึงไดจัดใหมีการประชุมเพื่อ
วางแนวทางในการตรวจคัดกรองโรคมะเร็งปากมดลูก โดยใชการตรวจ HPV DNA รวมกับการตรวจดวย
เซลลวิทยา (conventional Pap smear หรือ liquid – based cytology) และสรุปวา ควรใชการตรวจ HPV DNA
รวมกับการตรวจเซลลวิทยา ในสตรีที่มีอายุตั้งแต 30 ปขึ้นไป เนื่องจาก
1. การติดเชื้อ HPV ในสตรีที่มีอายุนอยสวนใหญสามารถหายไดเองเมื่อเวลาผานไป (transient
infection)
2. จํานวนการมีเพศสัมพันธกับคูนอนใหมมักลดลงเมื่อมีอายุมากขึ้น ดังนั้นความชุกของการติดเชื้อ
HPV ซึ่งพบสูงสุดเมื่ออายุประมาณ 20 ปจะลดลงเมื่ออายุมากขึ้น
3. อุบัติการณของ CIN 2/3 เพิ่มขึ้นและสูงสุดในชวงอายุประมาณ 30 ป และมะเร็งปากมดลูกพบได
นอยมากในสตรีอายุนอยกวา 25 ป
4. ความจําเพาะของการตรวจ HPV DNA คอนขางต่ําในสตรีที่อายุนอยกวา 30 ป (รอยละ 80) เมื่อ
เทียบกับสตรีที่มีอายุมากกวา 30 ป (รอยละ 94)
ถาทั้งผลการตรวจ HPV DNA และ เซลลวิทยาใหผลลบจะมีคุณคาในการทํานายผลลบสูงมากเกือบ
รอยละ 100 การตรวจคัดกรองโรคครั้งตอไปควรทําทุก 3 ป (แผนผังที่ 1) โดยตรวจเซลลวิทยาเพียงอยาง
เดียว หรือ ตรวจ HPV DNA รวมกับเซลลวิทยา โอกาสที่จะมีพยาธิสภาพตั้งแต CIN 2 ขึ้นไปพบเพียง 1 ใน
1,000 โอกาสที่จะเปนตั้งแต CIN 2 ขึ้นไปใน 3 ปขางหนามีนอยกวา 2 ใน 1,000 และโอกาสที่จะเปนมะเร็ง
ปากมดลูกก็ยิ่งนอยมาก
ปจจุบัน เริ่มมีความเปลี่ยนแปลงในแนวทางการตรวจคัดกรองโรคมะเร็งปากมดลูก โดยมีการนําการ
ตรวจ HPV DNA มาใชรวมกับการตรวจดวยเซลลวิทยามากขึ้น เพื่อเพิ่มความไว และลดผลลบลวงของการ

ตรวจลง อยางไรก็ตาม แพทยผูดูแลควรมีความรู และทราบถึงขอจํากัดของการตรวจ HPV DNA
(ความจําเพาะที่ต่ํา โดยเฉพาะอยางยิ่ง การนํามาใชตรวจในสตรีที่มีอายุนอย เนื่องจากสตรีกลุมนี้มีอุบัติการณ
การติดเชื้อ HPV สูงอยูแลว แตโอกาสเกิดความผิดปกติของปากมดลูกที่เปนรอยโรคขั้นสูง และมะเร็งปาก
มดลูกมีนอยมาก) และสามารถใหคําแนะนําแกสตรีที่มาขอรับการตรวจคัดกรองโรคมะเร็งปากมดลูก
ตลอดจนเลือกใชในกลุมประชากรที่เหมาะสม เนื่องจาก การใชการตรวจ HPV DNA อยางพร่ําเพรื่อ
นอกจากจะสรางความกังวลใจแกสตรีที่ไดรับการตรวจแลว ยังทําใหเกิดการสงตรวจอื่นๆ ที่ไมจําเปน
รวมถึงการรักษาที่มากเกินไปดวย ซึ่งสงผลใหเกิดความสิ้นเปลือง และอาจเกิดขอแทรกซอนจากการตรวจ
และการรักษาที่ไมจําเปนดวย
