I feel it would take a few weeks to get this plan implemented only because it would be a change for
hospital staff and we all know at time that changes take time to get them going and all staff
following new procedures. Staff would have to take the extra few minutes to explain why a
medication is being given. But, on the other hand they would only have to explain it once not every
single time a dose was given and the patient would have a record of the medication they are taking.
The personnel involved in my plan would be the hospital pharmacist or IT department to send down

My expectations from implementing this action would be that more patients are aware of why a
medication is being administered to them and what reactions could possibly happen. In addition I
feel that doing so would help element medication distribution errors. The printing of medications
being administered would have an increase on the budget due to the paper cost and ink as well as

Implementation of Physical Restraints in Behavioral Health
Introduction
In behavioral health nursing, using physical restraints is a very integral aspect to the overall health
and well–being of patients and staff. Although this is still a very prominent and sensitive subject
amongst healthcare professionals, I deemed it important that my facility implement and utilize
physical restraints on our behavioral health unit. Nurses need to be educated on the use of restraints,
which lead to my self–education on this topic. Nurses need to be aware of the benefits and the

In the first week, I took my idea to implement physical restraints on my unit to my Unit Nurse
Manager. She deemed my idea credible to the needs of our unit, and encouraged me to gain as much
research and information as I could that pertained to the subject. Along with my Unit Nurse
Manager, we set up a meeting with the RN Chief of Nursing, Unit Psychiatrist, and Unit Medical
Director to present my research finding on Week 3. After their approval, my Unit Medical Director,
RN Nurse Manager, Unit Nurse Manger, and I met with CEO later that same week. He approved our
budget, and agreed that the hospital will cover all fees associated with this project. On week 4, I met
with the Unit Nurse Manager and we unveiled the implementation plan to the remainder of the staff.
After revealing the plan to staff, they assisted me in devising a plan that would allow to us be
properly educated and trained on the use of physical restraints. Week 4 was the week when my plan
really started to come to life. We had a two and a half day training class on the proper use of
physical restraints in mental health facilities. After being trained and receiving our order of
restraints, we put the restraints on the beds and were ready use them only if the unfortunate situation
presented itself. Week 6 is where I ultimately reviewed the effectiveness of the restraints, and
documented my findings.

Abstract For most of the 20th century implementation of the most effective reading programs has
been the primary topic of discussion among educators, administration and policy makers alike. In
more recent years the implementation of a "balanced framework" of literacy has been the focus in
the classroom setting. In this reflection piece I will define balanced literacy, reflect upon a recently
implemented lesson plan based upon this framework as well as offer next steps moving forward in

Outline Of The Implementation Plan
Implementation Plan According to CDC, each year estimated about 250,000 cases of central line–
associated bloodstream infection on happens in United States. The average cost of treatment of this
CLABSIs are 25,000 dollar per episode. CLABSI is the third most common health care associated
infections per Health and Human Service. And also there is reported about 500 to 4000 patients
death related to CLABSI in US. CLABSI are preventable but there are need to be much education
and resources needed. The central line associated infections are classified as exit site infection or
tunnel infection. Nurses are the key health care personnel work with Central lines at the bedside.
Nurses play a key role in assessment and prevention and management of these lines to avoid
infection. Nurses working with lines must be educated on the importance of infection prevention
and held to a high standard on infection control. The most important step to avoid or decrease the
central line associated infection is to develop and implement the Central line bundle. Central bundle
is considered as the guidelines for central line insertion and maintenance. The guidelines consist of
maximal barrier precautions on insertion and dressing change. In order to be completely adhere to
