1. PREPARERS REQUISITION FOR KANSAS TAX FORMS
The Kansas Department of Revenue (KDOR) uses electronic scanning equipment to process tax returns. Even though many
individual taxpayers that filed a return last year will receive pre-printed forms this year, we understand your need for an
additional supply. In order to make our processing system work to the utmost efficiency, we are requesting that you use the
originals of the forms listed below. To keep costs at a minimum, one-instruction booklet each for Individual Income,
Homestead Claim, Corporate Income and KS-2848 will be included in your shipment.
If you requested forms on this requisition in the past, KDOR should have your order information in the database and will send
you the same quantity each year, automatically. If you have not ordered in the past, please complete this form and return it
to KDOR. We will ship your order and add the information to our database so your order will be shipped automatically in
upcoming tax years. This order form will not be mailed each year. If you have an order change the form can be downloaded
from the web and faxed into the Department with the updated amounts.
In an effort to keep our printing costs down, we have eliminated the printing and/or distribution of the following flat tax forms
or envelopes and are making them available only through the instruction booklets.
