In Part 3 of the Course Project, you consider how the evidence you gathered during Part 2 can be translated into nursing practice.
Now that you have located available research on your PICOT question, you will examine what the research indicates about nursing practices. Connecting research evidence and findings to actual decisions and tasks that nurses complete in their daily practice is essentially what evidence-based practice is all about. This final component of the Course Project asks you to translate the evidence and data from your literature review into authentic practices that can be adopted to improve health care outcomes. In addition, you will also consider possible methods and strategies for disseminating evidence-based practices to your colleagues and to the broader health care field.
To prepare:
Consider Parts 1 and 2 of your Course Project. How does the research address your PICOT question?
With your PICOT question in mind, identify at least one nursing practice that is supported by the evidence in two or more of the articles from your literature review. Consider what the evidence indicates about how this practice contributes to better outcomes.
Explore possible consequences of failing to adopt the evidence-based practice that you identified.
Consider how you would disseminate information about this evidence-based practice throughout your organization or practice setting. How would you communicate the importance of the practice?
To complete:
In a 3- to 4-page paper:
Restate your PICOT question and its significance to nursing practice.
Summarize the findings from the articles you selected for your literature review. Describe at least one nursing practice that is supported by the evidence in the articles. Justify your response with specific references to at least 2 of the articles.
Explain how the evidence-based practice that you identified contributes to better outcomes. In addition, identify potential negative outcomes that could result from failing to use the evidence-based practice.
Outline the strategy for disseminating the evidence-based practice that you identified throughout your practice setting. Explain how you would communicate the importance of the practice to your colleagues. Describe how you would move from disseminating the information to implementing the evidence-based practice within your organization. How would you address concerns and opposition to the change in practice? It should be combined with the other two components of the Course Project and turned in as your Portfolio Assignment for this course.

Focused SOAP Note and Patient Case Presentation
Psychiatric notes are a way to reflect on your practicum experiences and connect them to the didactic learning you gain from your NRNP courses. Focused SOAP notes, such as the ones required in this practicum course, are often used in clinical settings to document patient care.
For this Assignment, you will document information about a patient that you examined during the last three weeks, using the Focused SOAP Note Template provided. You will then use this note to develop and record a case presentation for this patient. To Prepare
· Create a Focused SOAP Note on this patient using the template provided in the Learning Resources. There is also a completed Focused SOAP Note Exemplar provided to serve as a guide to assignment expectations.
PleaseNote:
· Based on your SOAP note of this patient, develop a case study presentation.
· Include at least five scholarly resources to support your assessment, diagnosis, and treatment planning.
The Assignment
The written portion of this assignment is a simulation for you to demonstrate to the faculty your ability to document the complex case as you would in an electronic medical record. The written portion of the assignment will be used as a guide for faculty to review your video to determine if you are omitting pertinent information or including non-essential information during your case staffing consultation video.
In your presentation:
· Present the full complex case study. Include chief complaint; history of present illness; any pertinent past psychiatric, substance use, medical, social, family history; most recent mental status exam; current psychiatric diagnosis including differentials that were ruled out; and plan for treatment and management.
· Report normal diagnostic results as the name of the test and “normal” (rather than specific value). Abnormal results should be reported as a specific value.
· Be succinct in your presentation, and do not exceed 8 minutes. Specifically address the following for the patient, using your SOAP note as a guide:
·
Subjective: What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis? What is the duration and severity of their symptoms? How are their symptoms impacting their functioning in life?
·
Objective: What observations did you make during the psychiatric assessment?
·
Assessment: Discuss their mental status examination results. What were your differential diagnoses? Provide a minimum of three possible diagnoses and why you chose them. List them from highest priority to lowest priority. What was your primary diagnosis and why? Describe how your primary diagnosis aligns with
DSM-5 diagnostic criteria and supported by the patient’s symptoms.
·

·
Plan: describe your treatment plan using clinical practice guidelines supported by evidence-based pr ...

The process of engaged decision-making connected to activities performed in the event of challenging, uncertain, or hazardous conditions is termed as leadership. Management is the purposeful activity of directing individuals through a set of formulated policies and procedures that are scientifically approved to attain the set healthcare outcomes. For example, pain therapy needs an awareness of the fundamental etiology of pain and a collection of therapeutic alternatives that may be deployed to the circumstance at present and may vary from basic to complex activities (Bleich, 2016). By participating to issue detection, accomplishing tasks, and offering input for assessment, followers engage with individuals who are leading or managing. Several structured roles, like a nurse manager or a charge nurse, necessitate an enhanced collection of leadership and management skills to define institutional aims and goals, supervise human resources, facilitate staff with productivity responses, implement change, and solve conflicts in order to achieve client care and institutional specifications. Nursing, unlike other professions, requires nurses to alternate from managing, leading, and following on a regular basis.
Reply to Selena
Nursing is a well-rounded profession. In dealing with roles in nursing they are important for the flow of the flow and hierarchy. The three jobs of leadership, managing, and following all make up different roles. Leadership and management are often viewed as one and the same but that is not the case. Where they differ is leadership is looking forward and thinking of new ways to improve and management makes sure these new policies are being followed. Leadership focuses on “maximizing” effectiveness meaning increasing productivity of the floor (Marquis & Huston, 2017). Management handles in controlling the environment like; salaries, overtime, supplies, etc. (Marquis & Huston, 2017). Those that are followed are the ones carrying out the policies. For example, a leader creates the policy of after exiting a patient’s room to sanitize their hands to decrease the spread of disease. Management’s role is to ensure that this policy is being carried out by those working on the floor and explaining the purpose of the policy. Lastly, those that follow are the ones putting the policy in action and are applying it. They each play a role in creating a cohesive flow. There should always be an open line of communication between all parties so alterations can be made to improve effectiveness even more. In the case of a nurse moving through the positions, it is entirely possible. To better explain picture this; a nurse begins at the follower level (following the policy) and gets promoted to a management position this nurse has a better understanding of how the floor works and what alterations may be necessary to promote a bet ...

Changes in culture and technology have resulted in patient populatio.docxromeliadoan
Changes in culture and technology have resulted in patient populations that are often well informed and educated, even before consulting or considering a healthcare need delivered by a health professional. Fueled by this, health professionals are increasingly involving patients in treatment decisions. However, this often comes with challenges, as illnesses and treatments can become complex.
What has your experience been with patient involvement in treatment or healthcare decisions?
In this Discussion, you will share your experiences and consider the impact of patient involvement (or lack of involvement). You will also consider the use of a patient decision aid to inform best practices for patient care and healthcare decision making.
To Prepare:
Review the Resources and reflect on a time when you experienced a patient being brought into (or not being brought into) a decision regarding their treatment plan.
Review the Ottawa Hospital Research Institute’s Decision Aids Inventory at https://decisionaid.ohri.ca/.

Throughout this course, you have identified, examined, and provided individual as well as collaborative analysis on multiple facets of risk management in the health care setting.
Addressing the knowledge you have gained, and building on that knowledge to add your evaluation of the role that the managed care organization (MCO) plays in today's health care environment, develop a 250-500 word reflection to incorporate the following:
What is a health care organization's administrative role regarding oversight of risk management policies and ensuring compliance with managed care organization (MCOs) standards?
What is your assessment of the value provided to an organization that stems from the regulatory statutes of a typical MCO? Consider the establishment of conflict resolution and risk management strategies within the health care organization from the employer/employee perspective as well as in regards to patient conflict circumstances.
What MCO responsibilities pertain to the Patient Protection and Affordable Care Act (PPACA) and Center for Medicare and Medicaid Services (CMS) focus on fraud, waste, and abuse laws?

ASSESSMENT TASK 2
Assessment name: Case Study
Task description: This assignment aims to help you to begin to use your professional and clinical
judgement to think like nurses working in a mental health setting and/or in relation to
the mental health needs of people regardless of the setting.
Relevant to the person in your selected case scenario, your assignment will
demonstrate your understanding of the following:
• Mental Status Examination (MSE)
• A clinical formulation using the biopsychosocial model
• Recognising and responding to the mental health needs of the identified
person by identifying best practice nursing interventions.
• How to engage a person in a therapeutic relationship
NSB204 Assessment Task 2
What you need to do: Choose from one of the two case studies below. Each case study is a person who is
experiencing difficulties related to a mental health disorder.
1. Case scenario one: Natasha
2. Case scenario two: Tom
Please follow the steps outlined below to answer this assignment question.
1. The Mental Status Examination [250 words +/- 10%]
Using the mental state examination (MSE) format from your tutorial guide, complete
an MSE of the person from information provided in your selected case study.
Use health terms correctly (for instance, instead of “talks fast” use the correct term
“pressure of speech”)
The opening sentence must clearly state your chosen case study.
In text references are not required for this section
2. Clinical Formulation [250 words +/- 10%]
Using information provided in your selected case study complete a clinical
formulation identifying relevant indicators using the 5Ps framework: presenting,
predisposing, precipitating, perpetuating, and protective factors. Include
biopsychosocial factors relevant to the person in the case study.
You may choose to present the clinical formulation in a table or in paragraph format,
ensure responses remain in sentence form.
In text references are not required for this section
3. The Therapeutic Relationship [300 words +/- 10%]
The therapeutic relationship is an important part of the nurse’s role. Explain why the
provision of a therapeutic relationship would be a priority for the nurse when working
with the person from your selected case study. Select one strategy you would use to
develop the therapeutic relationship and provide a rationale for use adapted to
respond to the specific needs of the client in the case. You may consider the recovery
model, person centred care or cultural safety in developing your response.
Academic paragraphs integrating evidence-based literature are expected.
4. Nursing care plan [700 words +/- 10%]
Develop a nursing plan of care for the individual in the case study. Base your plan of
care on information identified in your mental state examination, including your risk

Using the Data/Information/Knowledge/Wisdom Continuum
Have you ever gone online to search for a journal article on a specific topic? It is amazing to see the large number of journals that are available in the health care field. When you view the library in its entirety, you are viewing untapped data. Until you actually research for your particular topic, there is little structure. Once you have narrowed it down, you have information and once you apply the information, you have knowledge. Eventually, after thoughtful research and diligent practice, you reach the level of wisdom—knowledge applied in meaningful ways.
Are there areas in your practice that you believe should be more fully explored? The central aims of nursing informatics are to manage and communicate data, information, knowledge, and wisdom. This continuum represents the overarching structure of nursing informatics. In this Assignment, you develop a research question relevant to your practice area and relate how you would work through the progression from data to information, knowledge, and wisdom.
To prepare:
1.Review the information in Figure 6–2 in
Nursing Informatics and the Foundation of Knowledge
.
Figure 6-2. DIKW Framework
2.Develop a clinical question related to your area of practice that you would like to explore.
QUESTION: What is the
Acute Myocardial Infarction
Core Measure
Set.
3.Consider what you currently know about this topic. What additional information would you need to answer the question?
4.Using the continuum of data, information, knowledge, and wisdom, determine how you would go about researching your question.
Explore the available databases in the Walden Library. Identify which of these databases you would use to find the information or data you need.
Once you have identified useful databases, how would you go about finding the most relevant articles and information?
Consider how you would extract the relevant information from the articles.
How would you take the information and organize it in a way that was useful? How could you take the step from simply having useful knowledge to gaining wisdom?
By Day 7 of Week 4
Write a
3- to 4-page
paper that addresses the following:
1.Summarize the question you developed, and then relate how you would work through the four steps of the data, information, knowledge, wisdom continuum. Be specific.
Identify the databases and search words you would use.
Relate how you would take the information gleaned and turn it into useable knowledge.
2.Can informatics be used to gain wisdom? Describe how you would progress from simply having useful knowledge to the wisdom to make decisions about the information you have found during your database search.

Kaggle has hosted a data science competition to predict category of crime in San Francisco based on 12 years (From 1934 to 1963) of crime reports from across all of San Francisco’s neighborhoods (time, location and other features are given).
I would like you to explore the dataset attached visually using Tableau and uncover hidden trends:
Are there specific clusters with higher crime rates?
Are there yearly/ Monthly/ Daily/ Hourly trends?
Is Crime distribution even across all geographical areas or different?

Developing a Basic Logic
Model For Your Program
Drawing a picture of how your program will achieve results
hether you are a grantseeker developing a proposal for start-up funds or a
grantee with a program already in operation, developing a logic model can
strengthen your program. Logic models help identify the factors that will
impact your program and enable you to anticipate the data and resources
you will need to achieve success. As you engage in the process of creating your
program logic model, your organization will systematically address these important
program planning and evaluation issues:
• Cataloguing of the resources and actions you believe you will need to reach intended
results.
• Documentation of connections among your available resources, planned activities and
the results you expect to achieve.
• Description of the results you are aiming for in terms of specific, measurable, action-
oriented, realistic and timed outcomes.
The exercises in this chapter gather the raw material you need to draw a basic logic
model that illustrates how and why your program will work and what it will accomplish.
You can benefit from creating a logic model at any point in the life of any program.
The logic model development process helps people inside and outside your
organization understand and improve the purpose and process of your work.
Chapter 2 is organized into two sections—Program Implementation, and Program
Results. The best recipe for program success is to complete both exercises. (Full-size
masters of each exercise and the checklists are provided in the Forms Appendix at the
back of the guide for you to photocopy and use with stakeholder groups as you design
your program.)
Exercise 1: Program Results. In a series of three steps, you describe the results you
plan to achieve with your program.
Exercise 2: Program Resources and Activities by taking you through three steps
that connect the program’s resources to the actual activities you plan to do.

The Mytown Example
Throughout Exercises 1 and 2 we’ll follow an example program to see how the logic
model steps can be applied. In our example, the folks in Mytown, USA are striving to
meet the needs of growing numbers of uninsured residents who are turning to Memorial
Hospital’s Emergency Room for care. Because that care is expensive and not the best
way to offer care, the community is working to create a free clinic. Throughout the
chapters, Mytown’s program information will be dropped into logic model templates for
Program Planning, Implementation, and Evaluation.
Novice Logic modelers may want to have copies of the Basic Logic Model Template in
front of them and follow along. Those read.

1. The first issue that jumped out to me is that the president and two vice presidents were the ones to develop the program. Our lecture notes and the text tell us that safety is one topic where management and employees can usually come to an agreement. Everyone wants a safe work environment. We are also taught that consultation is the best way to approach health and safety at work. Again, this means involving more than three people at the company. For starters, I would recommend that the safety program be dismantled and reconstructed by a committee consisting of at least 50% employees, not just senior leadership. I would keep this committee as small as possible and not have it controlled by one person only. The committee should be formed of employees from all sections and representing all possible departments where health and safety are potential issues.

There could be a number of problems with CMI's safety awareness plan. One major one is that they could not be promoting safety. That is the first step into getting the program to work...employee involvement. First the awareness program was developed by the president and the vice presidents. A safety awareness program can be more successful if employees are involved in the development, and remain involved as it is adjusted and refined. Rules should be in place, and employers must ensure that those rules are followed and enforced consistently. Incentives and competition could be another way to promote safety in the work place. Our text cites that having employees work in teams and have them determine the incentives will keep them involved and promote safety. Also, of course keeping employees up to date on all rules will also promote safety.
2. I think the supervisor's response to employee complaints about John Randall is not appropriate at all. Even thought it is difficult, home problems should not be brought into the work place. Especially if coworkers are complaining about someone's behavior. This does not promote safety at all. To say that Randall will get over it and to disclose that he has personal problems is.

Perceptions Impact
Your Market?
By Nicole Olynk Widmar and
Melissa McKendree, Purdue University
I aintaining existing mar-
kets for pork products,
I cultivating new markets
for existing products and
creating new products for new markets
are some avenues that the U.S. pork
industry has sought, and continues to
explore, for growth. When it comes to
maintaining markets, there are several
relationships that must be considered.
End consumers, whether in restaurant
or supermarket settings, are increas-
ingly interested in social issues and the
production processes employed in food
production. Livestock products (meat
and dairy products) certainly seem
to get the majority of the spotlight in
regard to consumers' concern for pro-
duction processes.
Shoppers in supermarkets and din-
ers in restaurants have increased access
to information via the Internet, and are
in constant communication with one
another via social media and alterna-
tive news sources about perceptions
of animal agriculture. Even though
most U.S. consumers are not directly
in contact with livestock, concern for
the treatment of animals, including
those employed in food production,
is evident — and increasing. While
in the past consumers were mainly
concerned with factors like the fat or
nutritional content of pork, for exam-
ple, today's savvy shoppers are con-
sidering other factors, like the welfare
of livestock (pigs), safety of workers
employed on farms and potential envi-
ronmental impacts (externalities) of
livestock operations.
Large-scale changes in production
practices are taking place in livestock

production due to pressures from vari-
ous interested parties. Changes such
as the discontinued use of gestation
stalls, for example, are being sought
via traditional regulatory channels in
some states, but are also being pushed
via non-traditional market channels.
Consider the cumbersome process
of changing regulations, versus the
oftentimes faster (and perhaps easier)
channel of influencing key market
actors. It is no surprise that consum-
ers' concerns are increasingly voiced to
supermarkets and restaurants which,
in turn, take action to satisfy their
customers by placing pressure on sup-
ply-chain players. Changes sought via
"the market," rather than legislation or
regulation, are increasingly common,
and the use of market channels for
communicating throughout the supply
chain is unlikely to stop anytime soon.

s the global economy has become more integrated and urbanized,
fueled in large part by technology, major cities and metropolitan
areas have become key engines of economic growth. The 123 largest
metro areas in the world generate nearly one third of global output
with only 13 percent of the world’s population.
In this urban-centered world, the classic notion of a
global city has been upended. This report introduces
a redefined map of global cities, drawing on a new
typology that demonstrates how metro areas vary in
the ways they attract and amass economic drivers
and contribute to global economic growth in distinct
ways. New concerns about economic stagnation—in
both developing and developed economies—add
urgency to mapping the role of the world’s cities and
the extent to which they are well-positioned to deliver
the next round of global growth.1
Instead of a ranking or indexed score, which many
prior cities indices and reports have capably deliv-
ered,2 this analysis differentiates the assets and
challenges faced by seven types of global cities.
This perspective reveals that all major cities are
indeed global; they participate as critical nodes in
an integrated marketplace and are shaped by global
currents. But cities also operate from much differ-
ent starting points and experience diverse economic
trajectories. Concerns about global growth, productiv-
ity, and wages are not monolithic, and so this typology
can inform the variety of paths cities take to address
these challenges. For metro leaders, this typology
can also ensure better application of peer com-
parisons, enable the identification of more relevant
global innovations to local challenges, and reinforce a
city-region’s relative role and performance to inform
economic strategies that ensure ongoing prosperity.
This report proceeds in four parts. In the following
section, Part II, we explore the three global forces of
urbanization, globalization, and technological change,
and how together they are demanding that city-
regions focus on five core factors—traded clusters,
innovation, talent, infrastructure connectivity, and
governance—to bolster their economic competitive-
ness. Building on these factors, Part III outlines the
data and methods deployed to create the metropoli-
tan typology. Part IV explores the collective economic
clout of the metro areas in our sample and introduces
the new typology of global cities. Finally, Part V
explores the future investments, policies, and strate-
gies required for each grouping of metro areas. Within
the typology framework, we explore the priorities for
action going forward, including the implications for
governance.

• To understand the power of blockchain systems, and the things they can do, it is important to
distinguish between three things that are commonly muddled up, namely the bitcoin currency,
the specific blockchain that underpins it and the idea of blockchains in general.

WHAT IS BLOCKCHAIN?
• A technology that permits transactions to be recorded
– Cryptographically chains blocks in order
– Allows resulting ledger accessed by different servers
– Information stored can never be deleted
• A digital distributed ledger that is stored and maintained on multiple systems belonging to multiple
entities sharing identical information (Deloitte)
• Bitcoin was the first demonstrable use
HISTORY OF BLOCKCHAIN
T YPES OF BLOCKCHAINS
• public or permissionless blockchains
– everyone who wants to engage in the network can openly see all transactions. The technology is
transparent, and all who want to engage in making transactions on the blockchain can do so.
• private or permissioned blockchains
– closed and accessible only to a selected few who have permission to engage in the blockchain.
BLOCKCHAIN FEATURES
• A blockchain lets us agree on the state of the system, even if we don’t all trust each other!
• We don’t want a single trusted arbiter of the state of the world.
• A blockchain is a hash chain with some other stuff added
– Validity conditions
– Way to resolve disagreements
• The spread of blockchains is bad for anyone in the “trust business”
WHAT IS BITCOIN
• A protocol that supports a decentralized, pseudo-anonymous, peer-to-peer digital currency
• A publicly disclosed linked ledger of transactions stored in a blockchain
• A reward driven system for achieving consensus (mining) based on “Proofs of Work” for

Orientalizing New Spain:
Perspectives on Asian Influence
in Colonial Mexico1
Edward R. Slack, Jr.2
Abstract
This article examines the totality of Asia’s influence on New Spain that resulted
from the conquest of Manila in 1571 and the regularization of transpacific tra-
de – more widely known as the Manila Galleons or naos de China – between the
Philippines and Acapulco. In its wake, a steady stream of Asian immigrants,
commodities, and manufacturing techniques measurably impacted colonial
society and economy through a process the author calls “Orientalization.”
However, “Orientalization” should in no way be equated with Edward Said’s

concept of “Orientalism” because of New Spain’s uniquely intimate historical
relationship with Asia in the early Modern era.
Introduction
Contrary to popular belief, the Philippines Islands were more a colony of New
Spain (Nueva España) than of “Old Spain” prior to the nineteenth century.
The Manila galleons, or naos de China (China ships), transported Asian pro-
ducts and peoples to Acapulco and other Mexican ports for approximately
250 years. Riding this ‘first wave’
of maritime contact between
the Americas and Asia were tra-
velers from China, Japan, the
Philippines, various kingdoms in
Southeast Asia and India known
collectively in New Spain as chinos
(Chinese) or indios chinos (Chine-
se Indians), as the word chino/a
became synonymous with the
Orient. The rather indiscrimi-
nate categorizing of everything
“Asian” under the Spanish noun
for the Ming/Qing empire, its
subjects and export items is easily
discovered in a variety of sources
from that age. To illustrate, the
eig hteenth centur y works of
Italian adventurer Gamelli Carreri and the criollo priest Joachin Antonio
de Basarás (who evangelized in Luzon) nonchalantly refer to the Philippine
Islands as “la China.”3 Additionally, words such as chinería (Chinese-esque,
European/Mexican imitation of Chines.

Published classroom materials form the basis of syllabuses, drive teacher professional development, and have a potentially huge influence on learners, teachers and education systems. All teachers also create their own materials, whether a few sentences on a blackboard, a highly-structured fully-realised online course, or anything in between. Despite this, the knowledge and skills needed to create effective language learning materials are rarely part of teacher training, and are mostly learnt by trial and error.
Knowledge and skills frameworks, generally called competency frameworks, for ELT teachers, trainers and managers have existed for a few years now. However, until I created one for my MA dissertation, there wasn’t one drawing together what we need to know and do to be able to effectively produce language learning materials.
This webinar will introduce you to my framework, highlighting the key competencies I identified from my research. It will also show how anybody involved in language teaching (any language, not just English!), teacher training, managing schools or developing language learning materials can benefit from using the framework.

Total Points for Assignment:
Please copy and paste this rubric table at the end of all
Application Assignments in order for your assignment to be
graded.
Guidance for Application Assignments:
6. Application Assignments require a title page, introduction, body
of the paper, conclusion, and reference page. The title page
needs to follows APA style and includes a title, student name,
course number and section, and date. An abstract is not
required. APA style headings are to be used appropriately to
separate and organize sections of the paper. The use of direct
quotes is discouraged and should only be used when the source
material uses language that is particularly striking or notable.
The introduction should provide an overview of the topic, the
purpose of the paper, and topics that will be addressed. The
body of the paper needs to address all required topics. The
conclusion ought to provide closure for the reader, synthesize
the content, and tie everything together to help clarify the main
points of the paper. The reference page should include all
references cited in the assignment in correct APA format.. Some
course assignments may require the use of more than 2 credible
outside resources. In these cases, the minimum outside sources
specified in the instructions for the assignment will be applied
in the grading of these course assignments. Credible sources
include scholarly peer-reviewed journal articles, evidence based
resources, and professional (.org), educational (.edu), and
government websites (.gov). Commercial websites (.com) are
not considered credible sources. Please note: When selecting
articles for course assignments, you are advised (unless you are
referencing seminal information) to focus on work published
within the past five years.
Late Policies:
Assignments submitted late without prior agreement of the
Instructor, outside of an emergency absence, or in violation of
agreements for late submission, will receive a grade reduction
for the Assignment amounting to a 20% point loss. After 5
days, the Assignment will not be graded. Students should be
aware that late Assignments may not receive the same level of
7. written feedback as do Assignments submitted on time.
There are several theories that explain why juveniles become
delinquent. Some of the sociological theories include anomie,
social disorganization, drift theory, and differential association
theory. Locate a sociological theory (it does not have to be one
of the theories listed previously), and explain that theory with
its relation to juvenile delinquency. Please note that
sociological theories relate to society and external influences
over a juvenile, not psychological influences (which relate to
internalization within a person) or biological influences (which
concern physical problems).
You are a sociologist, and you are noticing that the juveniles in
your area are starting to cause legal problems and are beginning
to overload the juvenile court system.
· Once you decide on the theory, describe how it relates to
juveniles by explaining what the theory believes causes the
juvenile problems.
· Explain what you could do in your position to aid in
alleviating these problems.
Be sure to reference all sources using APA style.

Identifying a Researchable Problem
Walden University
8. NURS - 6052 -03 Essentials of Evidenced-Based Practice
March 13, 2016
Identifying a Researchable Problem
Hundreds of millions of patients worldwide are affected by
healthcare associated infections every year (cite a source here-
how do you know). The infections lead to severe illnesses, long-
term disabilities, prolonged hospital stays and added costs to
patients, their families, and the healthcare system (Cite source).
Hand hygiene is the primary action to reduce the number of
healthcare-acquired infections; a simple action maybe, but the
compliance rates among the healthcare providers is the main
problem (Cite source) . The purpose of this paper is to provide a
summary on the issue of handwashing non compliance amongst
healthcare workers and its relation to nosocomial infections.
This paper will also generate five questions and describe how
they are analyse for feasibility, as well as a PICOT question and
a description of its variable. Additionally, ten keywords will be
developped to help conduct the literature search.
Problem Identification
A crucial factor in selecting a problem is its significance to
nursing, and evidence from the study should have potential to
9. contribute meaningfully to nursing practice ( Polit & Beck,
2012). Nurses comprise s of 55% of the healthcare workforce
and number more than 3 million. , aAs such, they represent the
most critical group in hand higiene compliance as they spent the
most time with the patients (McGonigle & Mastrian, 2012).
Hand hygiene is a critical practice aimed at keeping the hands
clean and free from pathogens or reduce the amounts before
performing any medical procedure or touching a patient.
Practicing hand hygiene is important in the prevention of cross
infection in clinical settings. Healthcare workers need an easy
to use, timely access to both skin protection and hand hygiene
for good hand hygiene practice.
Studies show that most nosocomial infections are transmitted
through the hands of healthcare practitioners. It is well-known
that better hand hygiene practices among healthcare workers
play a critical role in the prevention of transmission of
infectious agents (Pittet, Allegranzi & Boyce, 2009). The
practice of hand washing is one of the most effective ways of
preventing the transmission of infectious diseases. , but
dDespite the implementation of the Centers for Disease Control
and Prevention guidelines for hand hygiene, the compliance
among health practitioners remains low (Pittet, Allegranzi &
Boyce, 2009).
Hand washing is a critical element of patient safety, as it
prevents healthcare associated infections as well as the spread
of antimicrobial resistance (Source? How do you know this?).
The promotion of hand hygiene, therefore, presents a common
challenge that calls for a multimodel strategy. Healthcare
worker’s adherence to hand washing guidelines is poor and calls
for a behavior change. Most studies have recommended
multimodal strategies that include education of healthcare
workers, audit and performance feedback practices, increased
supply of water and soap, reminders, use of automated sinks and
the use of alcohol-based hand rub (Pfoh, Dy, & Engineer, 2013).
Hospital-acquired infections carry a far-reaching and real threat
to every individual admitted to hospitals. The pathogens from
10. the hands of the healthcare workers are readily transmitted to
the patients, hence hand hygiene practices substantially reduce
this chance. Evidence-based guidelines for hand hygiene in
clinical practice are very clear, but the compliance remains
internationally low (Bukhari et al., 2011).
Significance of the Problem of Nursing Practice
Hand hygiene is considered as the most simple and the most
effective way of preventing hospital-acquired infections.
Despite the milestones and achievements made in epidemiology
and infection control, the hygiene message has not been
consistently translated into healthcaremedical practice, and the
healthcare workers adherence to the guidelines for hand hygiene
is unacceptably low. (Bukhari et al., 2011). The average
compliance with the hand washing guidelines varies by from the
patient wards, among health care workers categories, and based
on the working conditions, but also varies according to the
different definitions used in different studies (Pittet, Allegranzi,
& Boyce, 2009). Comment by lisa south: Put the period at
the end of the sentence-after the parenthesis.
Compliance with hand-washing recommendations is a very
significant measure in the prevention of hospital-associated
infections. Studies report that transmission of pathogens from
the hands of healthcare workers is one of the primary sources of
cross-infection in clinical settings and can be prevented by
proper hand hygiene practices (Bukhari et al., 2011). There are
cases of very low or insufficient compliance rates in developing
and developed countries with the mean baseline rates of
between 5% and 89%, making an overall average of 38.7%
(Bukhari et al., 2011). Comment by lisa south: Studies? So
more than one? But only one is cited? Comment by lisa
south: Since you said “studies” cite more than one.
Variation in hand washing behavior across healthcare workers
and hospital wards suggests the need for a strategic educational
program. Nosocomial infections are a significant cause of
complications in the ICU; withand hand-washing being the
single most effective preventive measure, the compliance rates
11. remain low (Bukhari et al., 2011). The association between
handwashing non-compliance and surgical wards may also
suggest a fact that understaffing reduces the quality of patient
care. Hand washing has a proven benefit in the prevention of
cross-infection. Hence the compliance, especially in the surgical
and ICU wards, is paramount (Royal Pharmaceutical Society,
2014).
Studies and estimations by the World Health Organization
(WHO) show hHealthcare-associated infections affect at least
1.4million patients on the global scale at any given time (Cite
the source??? . In the surgical units, the burden of hospital
acquired infections has significantly increased, leading to
increased morbidity and mortality. In some instances, multidrug
resistant pathogens are involved or are part of the infection,
hence pose a challenge to effective treatment (Pittet, Allegranzi
& Boyce, 2009). Proper hand hygiene is the simplest, single
most important and the least expensive means of preventing the
cross infections. The WHO and CDC (2009) guidelines on hand
hygiene also propose the use of alcohol base hand rub as a
means for routine hand disinfection. Comment by lisa
south: Neither of these is on the reference list.
Questions Related to the Identified Issue This information needs
to have an introductory paragraph (at least two sentences).
1. What are the hand hygiene practices among Healthcare
workers?
Rationale
It is important to assess the types of hand hygiene practices
commonly used by healthcare workers so as to determine the
choices available and the recommended practices
2. What are the reasons for non-compliance with hand hygiene
practice?
Rationale
It has been noted that there is moderate to low rates of
compliance with standard hand hygiene practices among
healthcare workers. It is, therefore, important to get to know the
reasons behind the non-compliance.
12. 3. What are the alternative hand hygiene practices?
Rationale
There exist more than one hand hygiene techniques from which
healthcare workers can choose preferred as well as the simplest
and most efficient method of hand hygiene practice.
4. What are the disparities in hand hygiene practices among
different healthcare workers?
Rationale
The non-compliance to hand hygiene practices vary across the
various cadres, ages and experience of health personnel. It is
important to know the personal factors that are attributed to the
variation between the different categories of health workers.
5. What can be done to improve hand hygiene practices in
healthcare?
Rationale
It is also important to assess the actions that can be taken to
encourage hand hygiene practices in healthcare.
The most appropriate question from the above-related questions
is; Ddoes hand washing and appropriate staff dressing among
the surgical ward nurses reduce cross infection during patient
management?
Description of the PICOT Question
The PICOT question is: Does hand washing and proper staff
dressing among the surgical ward nurses reduce cross infection
during patient management? The elements of the table are:
P- (Patient, population, or problem): nurses, hospitalized
patients
I- (Intervention): hand washing and appropriate dressing
C- (Comparison with other treatment/current practice): Nurses
forget to wash their hands when switching from one patient to
the other
O- (Desired outcome): Reduced incidences of cross infection
(Davies, 2011)

10. “Hand hygiene and cross-infections."
Rationale
14. Determining hand hygiene and the relationship with cross
infections
Conclusion
Studies consistently show that better hand hygiene reduces the
risks of cross contamination and nosocomial infections in
Hospitals. Since most of the cross infections are thought to be
transmitted through the dirty hands of healthcare workers, an
assessment of the knowledge, attitude and practice of them is
also necessary. Hospital-acquired infections carry a serious and
real threat to all the patients admitted to hospitals, hence the
assessment of healthcare worker’s hand hygiene practices is
equally very significant.
