1. The patient presented with fever of 38°C and Gram-positive cocci were found in chains in their blood cultures, identifying Streptococcus bovis as the cause of infective endocarditis. 2. Echo showed vegetation on both mitral leaflets with mild mitral regurgitation and stenosis. Eye exam found Roth spots. 3. The patient is being treated with penicillin and gentamicin for subacute bacterial endocarditis.
