The Trillium Bridge project aims to lower the costs of transatlantic engagement in eHealth by building a bridge for the exchange of patient summaries between Europe and the United States. The project has completed a gap analysis between US and EU patient summary standards and developed a terminology service and proof of concept transformer. Current work includes refining interoperability assets and testing with EU countries and US providers to set a precedent for global eHealth interoperability.

DIA 2015 - EMR/EHR Clinical Data Intergration with EDC Systems
It has been a longstanding challenge to integrate patient data from EMRs (Electronic Medical Record systems) with EDC (Electronic Data Capture) systems for clinical studies and trials.
Integration between disparate systems can be done in many ways. Any two systems can be integrated given enough time and money. However, if there is to be wide-scale integration between applications used by clinicians at their practice or in the hospital, it requires the formation and use of standards for the structure and content of data as well as the transport of data between two systems.
A historic problem has been the lack of EMR adoption, particularly in physician offices. Thankfully, this is improving dramatically, something I will touch on later.
There is also a historic problem that Case Report Forms and the databases that contain them in EDC systems are routinely not designed from the start with standardized labels, content or structure. More adoption of the CDISC standards, such as C-DASH (clinical data acquisition standards harmonization) which dictates structure and content, and ODM (operational data model) which is an XML rendering of the CDASH CRFs.. If you take anything away from this talk, please ensure any data capture you do within an EDC system going forward meets the ODM model.
Patient matching between systems is important. There is a whole industry for Community and Master Patient Indexes to link records in disparate systems to the same patient. This gets even more complex when the patients are to be kept anonymous or the studies are blinded.
Lastly, moving data across systems is a challenge. There are two primary methods – 1) asynchronous file transfer in conjunction with an ETL, or 2) APIs where one system is inquiring in real time into another application for data. The latter is usually regarded as a more robust integration, but without standard APIs these are one off solutions. 19 November 2012 - National End of Life Care Programme
The End of Life Care Strategy (2008) identified the need to improve co-ordination of care, recognising that people at the end of life frequently received care from a wide variety of teams and organisations. The development of Locality Registers (now Electronic Palliative Care Co-ordination Systems known as EPaCCS) were identified as a mechanism for enabling co-ordination.
By supporting the elicitation, recording and sharing of people's care preferences, and key details about their care, it is anticipated that EPaCCS will improve the quality of care, with provision meeting people's expressed wishes and preferences. Early findings from the South West SHA Locality Register pilot showed that the vast majority of people on the register were able to die outside of hospital, and in their preferred place of care.
Why EPaCCS?
EPaCCs will contribute to increases in the quality of end of life care individuals receive by improving co-ordination and communication across sectors, ensuring that all those involved in care will be aware of the individuals wishes and preferences as recorded in Advance Care Plans (ACPs) as well as treatment care plans. They contribute to the patient Choice agenda as well as the Quality, Improvement, Productivity and Prevention (QIPP) agenda and improve patient safety by reducing harm through co-ordinated communication in standardised format to reduce the risk of inappropriate interventions.

This document provides an executive summary of a national review of adult elective orthopaedic services in England led by Professor Tim Briggs. The review found significant undesirable variation in orthopaedic practice and outcomes across the country. Emerging themes included low surgeon volume, failure to follow evidence on implants, and variation in pathways, management models, and commissioning relationships. The review aims to improve quality and reduce costs by reducing unwarranted variation and encouraging best practice. Over 200 hospitals were visited and individual reports provided. The methodology was effective and could be applied to other specialties.

Data quality and consistency are critical to ensuring patient safety, communicating delivery of health services, coordinating
care, and healthcare reporting. Assessing the quality and consistency of data requires data standards. This practice brief
provides health information management (HIM) professionals with a clear understanding of data standards as a tool to
enable interoperability and promote data quality.

eHealth Practice in Europe: where do we stand?
eHealth as the use of Information and communication technologies in the practice of health care comprises Electronic health records, Healthcare information exchange cross-jurisdictions, Personal health records, Telehealth, telemedicine and remote monitoring.
There are several efforts to reflect and measure the practice of eHealth including efforts by the OECD and WHO, but in general there is little reported sharing of health data particularly with patients. Specific barriers frequently mentioned are supporting policies and coherent widely implemented standards.
The presentation discusses relevant efforts and programs supported by the European Commission such as the eHealth DSI, eStandards, ASSESS CT, and openMedicine aiming at large scale eHealth adoption It calls for engagement of European Society, its national societies, and its members.

EPaCCS: Making the case for change
19 November 2012 - National End of Life Care Programme
The End of Life Care Strategy (2008) identified the need to improve co-ordination of care, recognising that people at the end of life frequently received care from a wide variety of teams and organisations. The development of Locality Registers (now Electronic Palliative Care Co-ordination Systems known as EPaCCS) were identified as a mechanism for enabling co-ordination.
By supporting the elicitation, recording and sharing of people's care preferences, and key details about their care, it is anticipated that EPaCCS will improve the quality of care, with provision meeting people's expressed wishes and preferences. Early findings from the South West SHA Locality Register pilot showed that the vast majority of people on the register were able to die outside of hospital, and in their preferred place of care.
Why EPaCCS?
EPaCCs will contribute to increases in the quality of end of life care individuals receive by improving co-ordination and communication across sectors, ensuring that all those involved in care will be aware of the individuals wishes and preferences as recorded in Advance Care Plans (ACPs) as well as treatment care plans. They contribute to the patient Choice agenda as well as the Quality, Improvement, Productivity and Prevention (QIPP) agenda and improve patient safety by reducing harm through co-ordinated communication in standardised format to reduce the risk of inappropriate interventions.

The review found significant undesirable variation in orthopaedic practice and outcomes across the country. Emerging themes included low surgeon volume, failure to follow evidence on implants, and variation in pathways, management models, and commissioning relationships. The review aims to improve quality and reduce costs by reducing unwarranted variation and encouraging best practice. Over 200 hospitals were visited and individual reports provided. The methodology was effective and could be applied to other specialties.

eHealth Practice in Europe: where do we stand?
eHealth as the use of Information and communication technologies in the practice of health care comprises Electronic health records, Healthcare information exchange cross-jurisdictions, Personal health records, Telehealth, telemedicine and remote monitoring.
There are several efforts to reflect and measure the practice of eHealth including efforts by the OECD and WHO, but in general there is little reported sharing of health data particularly with patients. Specific barriers frequently mentioned are supporting policies and coherent widely implemented standards.
The presentation discusses relevant efforts and programs supported by the European Commission such as the eHealth DSI, eStandards, ASSESS CT, and openMedicine aiming at large scale eHealth adoption It calls for engagement of European Society, its national societies, and its members.
In search of a Digital Health Compass Patient Empowerment
1. The document discusses empowering people through improving digital health literacy and using personal health data standards to access one's own health information. It aims to explore how eHealth investments can empower citizens, patients, caregivers and providers.
2. Several emerging solutions are highlighted, including patient-powered research networks where patients share and donate health data. Standards like HL7 FHIR are presented as enabling live integration of personal health data.

In search of a digital health compass: My data, my decision, our power
Knowledge is power. Despite extensive investments in digital health technology, navigating the health system online is challenging for most citizens. Also for eHealth, the “Inverse Care Law” proposed by Hart in 1971, seems to apply. Availability of good medical or social care services and tools online, varies inversely with the need of the population. The low adoption of eHealth services, and persistent disparities in health triggers a call for multidisciplinary action.
Barriers and challenges are not to be underestimated. Culture, education, skills, costs, perceptions of power and role, are essential for multidisciplinary action. This comes together in digital health literacy, which ought to become an integral part to navigate any health system. Patients living with an implanted device or coping with persistent, chronic disease such as diabetes, as well as citizens engaged in self-care, caring for an elderly relative, a neighbor, or their child with illness or deteriorating health, need a digital health compass.
The panel will engage the audience to elaborate on a vision for this personal, digital health compass and drive advancement in health informatics and digital health standards. The transformative power of health data fueled by targeted digital health literacy interventions can be leveraged by open, massive, and individualized delivery. This way, digital health literate, confident patients and citizens join health professionals, researchers and policy makers to address age-related health and wellness changes to shape the emerging precision medicine and population health initiatives.
