 A transcript is proof of education. It has a detailed
record of all the subjects you have studied with your
scores in the form of marks or grades given by the
institution of study.
 A transcript is an official copy of a student’s
academic record. It records all courses successfully
and unsuccessfully completed and all courses that
were withdrawn after the registration deadline
 A Transcript is a copy of a student's permanent
academic record which usually means all courses
taken, all grades received, all honors received and
degrees conferred to a student.
 A transcript may also contain average grade of the
class.
 A transcript must have the following information-
 1. The overall grade received by the student for the
entire academic course/year
 2. The total number of students in the class (overall
strength of the class to compare where a particular
student ranks)
 3. The average grade of the class specifies where a
particular student stand
 An official transcript is prepared and sent by the
issuing school with an original signature of a school
official. When students are thinking about changing
schools, the cumulative transcripts usually get
mailed from school to school.
 Transcript can be cumulative record file, permanent
record, or simply record
Note: A transcript should always be signed
with a stamp by the registry of the
institution so that it may serve as an official
letter-headed paper.
Official Transcripts
 This type of academic transcript is a detailed record
of your academic advancement. Official Transcripts
usually include your exam credits, transfer work,
and the degrees you have earned over the years.
These transcripts are mailed by the institution and
bear the university seal and the registrar’s
signature. These are sent only at the student’s
request and require the student’s signature.
Unofficial Transcripts
 This transcript is issued by the institute but is
provisional and cannot be used to seek admission.

Health Science can apply for the MUHS
transcript online from the official university
website.
 Log in to the portal
 Fill out the transcript application form
 Upload self-attested scanned copies of the required documents
for MUHS transcript
 Complete MUHS transcript online payment
 Submit the application
 The transcript from MUHS will then be delivered to the
address mentioned in the application form.
 Copies of the mark sheets of graduation course
 Proof of identity of the student
 Transcript application form
 Passport size photograph
 MUHS transcript fee payment receipt
 The university can take up to 25 to 30 days to process the
MUHS transcript application. Depending upon the pending
applications, the university can take a couple of days more or
less to deliver the transcript certificate of the student.
