This document contains a questionnaire to help students plan their future by identifying their talents, strengths, weaknesses, and considering suggestions from others. The questionnaire contains questions about students' contact with the workforce, expectations after graduating high school or college, satisfaction with post-graduation activities and choices, and what type of support would help them define goals and careers. It is targeted at students between 14-19 years old to help guide them as they consider their future education and career paths.
