This document is a location risk assessment form for a music video shoot taking place on 30 October 2013 at a underpass in Norwich. It identifies three main risks: smoking by cast and crew, the underground location of the underpass itself, and the presence of members of the public at the public underpass location. For each risk, it identifies the affected parties, proposed actions to mitigate the risks, and signatures confirming review and agreement of the risk assessment.

Quick shots and My crew will not be taking up the whole pass so we wont be in
anyone’s way and we don’t have any dangerous equipment so as long as we pay
attention and respect the members of the public using the pass there should be no
