The Wicked Problem: Heart Failure
Let’s start with some basic facts. It is estimated that there are 400,000 Canadians living with congestive heart failure (CHF): of those, about 40,000 experience cardiac arrest every year; and
less than five per cent of those who have a cardiac arrest outside of a hospital (the majority of cases) survive. Depending on the severity of symptoms, heart dysfunction, age and other factors, CHF can be associated with an annual mortality of between five and 50 per cent. Between 40 and 50 per cent of people with con- gestive heart failure die within five years of diagnosis.
Now let’s look at what this means for the health care system. A study by the Canadian Cardiovascular Society that exam- ined hospital discharges for fiscal 2000 found that a total of 1.38 million hospital days were associated with CHF. The average hospital stay was slightly less than 13 days. Re-admission to hos- pital was also examined. There were a total of 106,130 discharges for CHF in 85,679 patients – suggesting that there were 20,451 re-admissions among these patients. Empowering rural India is of utmost importance and the government needs to do so by provisioning for broadband penetration and financial inclusion. Access to quality health care is another key to achieving rural empowerment. The budget for this segment was raised marginally last year and it would be good to have an allocation for rural health care programs with provisions for technology that would help modernize this sector to expand its reach through remote healthcare solutions and telemedicine.
Furthermore, the government announced a big budget campaign 'Swabhimaan' in the budget last year to promote banking and provide services to about 20,000 villages. In order to meet this goal, the budget this year too would need to make provisions accordingly. The steering committee on health said that in the 12th plan (2012-17), all district hospitals would be linked to leading tertiary care centres through telemedicine, Skype and similar audio visual media. M-health will be used to speed up transmission of data. Disease surveillance will be put on a GIS platform.
Disease surveillance based on reporting by providers and clinical laboratories (public and private) to detect and act on disease outbreaks and epidemics would be an integral component of the system.India will also put in place a Citizen Health Information System (CHIS) - a biometric based health information system which will constantly update health record of every citizen-family. The system will incorporate registration of births, deaths and cause of death. Maternal and infant death reviews, nutrition surveillance, particularly among under-six children andwomen, service delivery in the public health system, hospital information service besides improving access of public to their own health information and medical records would be the primary function of the CHIS.

Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

20. Telecardiology is a modern medical practice, which uses the power of
telecommunications to achieve remote diagnosis and treatment of heart disease.
This includes coronary heart disease, chronic and acute, as well as arrhythmias,
congestive cardiac failure and sudden cardiac arrest.
21. Pre-Hospital:
Recent advances in telecardiology have allowed for rapid and
reliable remote ECG interpretation. Using a specialized hand-
held ECG unit the user takes a 12-lead ECG in the usual
manner.
22. In addition to the 12-lead ECG device, there is
also a 1-lead variant. These devices are available
in the form of a watch which may be worn on the
wrist quite comfortably for long periods at a
time.

increased frequency between rural small hospitals and tertiary care
centeres

Post hospital telecardiology is an integrated approach that involves the
patient, the family, the GP and specialized cardiac centers. Real-time
transmission of objective data (physiological data and biological signals) in
association with personal data given by the patient is a new approach to the
problem.

Pre hospital telecardiology allows for the use of Ecg outside of the hospital through the use of hand held ecg unit , The device encodes the ECG into sound and, at the touch of a button, plays this over a telephone line to the telecardiology service. This sound is decoded electronically and displayed as a full 12-lead ECG on screen at the telecardiology center. It can then be immediately interpreted by cardiologists and specialist nurses. Within minutes, an oral report is provided by telephone. A full written report is then sent to the general practice by email or fax. This is also stored securely at the telecardiology center, meaning that future ECGs from the same patient can be compared and contrasted with ease.
The patient can then activate a recording at the touch of a button. This recording is stored on the watch, and may be transmitted electronically to the telecardiology center via a device kept at the GP's practice.
telecardiology enables remote cardiologists to guide the sonographer, conduct timely diagnosis and propose effective therapeutic strategies for patients in rural areas where professional cardiologists are rarely accessible
Telemonitoring allows the follow up of patients for long periods. Indeed, telemonitoring and teleassistance through nurse, specialists and GPs can constitute a disease management program.
