I understand there is footage being taken of me on this date __________________
I hereby assign and authorise the producer ___________________________ the right
to record footage of me. I also authorise said producer, without limitation, the right to
reproduce, copy, exhibit, publish or distribute any footage they shoot ofme, and waive
all rights or claims I may have against your organisation and/or any of its Affiliates,
Subsidiaries, or Assignees other than as stated in this agreement.
